through its activities and those of our national members. We support member associations in oral health

Health Organization (WHO) and a member of the
World Health Professionals Alliance (WHPA).
Our main strategy is, with the support of all of our associate members, to be the Voice of Dentistry, leading

Planmeca expands on its 40th anniversary
The Finnish dental equipment manufacturer Planmeca Oy enlarges its premises
in Helsinki, Finland. After completing the
new 118,000 square feet annex building,
the company owns and occupies a total of
475,000 square feet of production and office space.
Planmeca’s product lines – dental care
units, dental x-ray units and advanced
imaging software – are known for their
user-orientated and functional design, as
well as latest technology, uncompromised
hygiene, and advanced usability. The company has been awarded several product
design awards in international design competitions.
During the past 40 years Planmeca has
grown into one of the leading companies in
the dental field. Its groundbreaking innovations stem on one side from close relationships with leading universities and have
lead to major advances in dentistry. Basing
its innovations on the other side on solid co-

operation with the users and understanding
details of their workflow, the company has
created several revolutionary concepts.
The new facility contains both offices and
production facilities. As in its products, the
company utilises the newest technology
in its production. The technologically advanced machining as well as the new powder painting department will be located in
the new building. In addition, the ground
level warehouse is all automated with two
robotic forklifts handling the logistics.

more effectively respond to the increased
demand”, explained Mr Heikki Kyöstilä,

FDI Statement on Classification
of Caries Lesions

C

aries is a major global oral disease and its management should be based
on our current knowledge of the disease process, its aetiology, prevention and control. Health promotion and well being across all communities are linked to caries control.

The original classification of carious cavities in exposed tooth surfaces was proposed by GV Black in the early 1900s for
use in operative procedures. A century
later, this system is still in widespread use
by the majority of dentists.
Over the last 30 years there have been
marked improvements in the under-

standing of the aetio-pathology and the
nature of caries and the caries process.

La clasificación original de la caries en las
superficies dentales expuestas fue propuesta por el GV Black en el año 1900
para procedimientos quirúrgicos1. Un
siglo más tarde, este sistema es todavía
de uso generalizado por la mayoría de los
dentistas.
Durante los últimos 30 años han habido
importantes mejoras en la comprensión
de la pato-etiología y la naturaleza de la
caries y el proceso de caries. La caries
dental es la destrucción localizada del
tejido dental duro susceptible a los ácidos derivados de la fermentación bacteriana de hidratos de carbono. El proceso
de la caries es la secuencia dinámica de
las interacciones del biofilm dental, que
puede ocurrir con el tiempo y dentro de
la superficie del diente. Las mejoras en
la comprensión de la caries se extienden
también al papel de la transmisión bacteriana en niños pequeños, la importancia de los factores de riesgo comunes y
su potencial impacto en la salud general.
La FDI World Dental Federation recomienda:
• Elaborar y aprobar una clasificación de
las lesiones de caries y un sistema para
su manejo, incluyendo la evaluación de
riesgos y la prevención, que sean capaces de describir y documentar la experiencia total de la caries (clínica) en una
población y a nivel individual.
• Que el sistema de clasificación y manejo de la caries se utilice como base
para comunicar y educar a los pacientes,
profesionales de la salud y gobiernos
acerca de la caries, su prevención, control y manejo.
• Que el sistema de clasificación y manejo de la caries proporcione un marco
para el diseño de políticas apropiadas de
salud, la toma de decisiones de planificación y la aplicación costo-efectiva de la
planificación del manejo de la caries.
• Desarrollar sistemas de apoyo y de registro para mantener una vigilancia adecuada y apropiada.
• Implementar la investigación presente
y futura para mejorar los sistemas y que
su cumplimiento sea una prioridad.

for temporomandibular disorders, which in their experience, simplifies
the diagnosis and treatment process.

Temporomandibular joint screening prior to
orthodontic rehabilitation is indispensable
for preventive, therapeutic, and forensic reasons.
Daily practice calls for a simple and effective
concept in order to treat TMD patients, and to
avoid iatrogenic mishaps during orthodontic
therapy. Furthermore, planning and progno-

sis may be improved in many cases.
Most diagnostic methods, from expensive
axiographic and electromyographic measurement, through to sophisticated imaging
techniques (MRI, ct) entail considerable
costs in terms of time and expense. As these
techniques mostly focus on the examination
of special parts and structures, they cannot

be considered as universal standard examination methods.
The Manual Functional Analysis according to
the Kiel concept (Bumann et al. 1989) provided the first substantial progress towards a
practice-oriented concept (Fig. 1)
On this basis, the AquaSplint concept was
developed. In compact form the established
techniques of the Kiel concept are utilized for
tissue-specific diagnostics. Subsequently,
TMD etiology and differential diagnosis
are determined using the AquaSplint. This
significantly affects therapy decisions and
treatment planning.
The previous hypothesis implying that temporomandibular joint disorders / TMD originate
from the stomatognathic area is only partially
correct. Opinions in the current literature widely differ on this topic. We believe that malocclusion may be among the causes of cer-

nuevo dispositivo para los trastornos temporomandibulares que, en su
experiencia, simplifica el proceso de diagnóstico y tratamiento.

La detección de trastornos de la articulación
temporomandibular antes de la rehabilitación ortodóncica es indispensable por razones preventivas, terapéuticas y forenses.
La práctica requiere de un concepto simple y
eficaz para el tratamiento de pacientes con
Trastornos Temporomandibulares (TTM) y
para evitar accidentes iatrogénicos durante el
tratamiento ortodóncico.
La mayoría de los métodos diagnósticos, desde las mediciones axiográficas y electromiográficas a las técnicas de imagenología (MRI,
CT), implican costos considerables en tiempo
y dinero. Como estas técnicas se centran en
el examen de piezas y estructuras específicas,
no pueden ser consideradas como métodos
universales estándar para el examen de los
TTM.
El Análisis Funcional Manual, siguiendo el
concepto de Kiel (Bumann et al. 1989), ofrece
el primer avance importante hacia un concepto orientado a la práctica (Fig. 1).
El Aqua Splint se ha desarrollado siguiendo
este concepto. Las técnicas de Kiel se utilizan
para el diagnóstico de tejidos específicos y

posteriormente la etiología y el diagnóstico
diferencial de los TTM se determinan utilizando el Aqua Splint. Esto afecta significativamente las decisiones y la planificación del
tratamiento.
Sin embargo, la hipótesis anterior que implica
que los trastornos de la ATM provienen de la
zona estomatognática, es sólo parcialmente
correcta. La literatura científica difiere sobre
este tema. La maloclusión puede ser una de
las causas de ciertos trastornos de la ATM;
sin embargo, rara vez es la única. Como es
sabido, hay pacientes que a pesar de presentar maloclusión aguda no muestran ninguna
molestia de la ATM, mientras que afecta significativamente a otros con discrepancias
oclusales menores.
Los trastornos cráneomandibulares (TCM) resultan de la interacción de múltiples factores:

tain TMJ disorders, however rarely appears to
be the only one. As generally known, there are
patients who, despite extreme malocclusion,
show hardly any, or no TMJ discomfort, while
other patients are sometimes severely affected despite minor occlusal discrepancies.
Craniomandibular disorders (TMD) result
from an interplay of multiple factors:
• malocclusion, overloading of the temporomandibular joint (compression, forced
position)
• hyperactivity of masticatory muscles
(bruxism/clenching)
• psychosomatic disorders, stress syndrome
• joint hypermobility, particularly in combination with general connective tissue
weakness (Fig. 2, 2a).
• trauma
• neck disorders / cervical spine syndrome
• internal diseases (hormonal, circulatory
disorders, rheumatism)
TMJ internal derangement results from joint
hypermobility rather than from malocclusion.
In a 12-year long-term study (1997-2009),
by our orthodontic practices, all patients with
diagnosed disc displacement (n = 535) were
manually examined for their bite position,
including the wrist joint. In cases of doubt,
magnetic resonance images were obtained.
Anterior disc displacement, with reduction,
was found in 421 patients, while 114 patients
showed displacement without reduction.
The association with connective tissue weakness was highly significant. Concomitant general connective tissue weakness was observed in 92.5 per cent of the patients (Fig.2b).
In contrast, no significant association was revealed between malocclusion and the abovementioned disc disorders.
Therefore, some doubts are raised about the

indication of disc repositioning with occlusal
rehabilitation. This type of therapy is often
complicated, time-consuming and expensive, and not without risk in the case of surgical
repositioning. Moreover, this therapy does
not necessarily protect from complete disc
displacement, without reduction, at a later
stage.
Even when therapy achieved optimum occlusal conditions and complied within narrow
limits of indication, there were usually intact
posterior ligaments. This is true in partial anterior disc displacement, and the presence of
a distal bite in need of treatment.
According to our current AquaSplint concept,
clicking alone does not constitute an urgent
need for therapy. On the other hand, existing
pain symptoms have to be examined as to
whether they actually originate from the temporomandibular joint/masticatory muscles.
Phantom or referred pain has to be excluded
by tissue-specific manual diagnostics. The
next diagnostic step should aim to determine whether malocclusion is the main cause
of pain. This is achieved most effectively
through (temporary) neutralization / disengagement of the existing malocclusion using
the AquaSplint. Thus, a possible forced bite is
eliminated and the muscles become relaxed.
In more serious cases, manual and physical
adjunct therapy should be applied.
A malocclusion-related TMD will usually respond due to AquaSplint therapy within four
weeks. Pain relief of at least 50 per cent is
a favourable precondition and indicator of
promising orthodontic rehabilitation. Use
of the AquaSplint often changes the bite.
Subsequently, the resulting new and almost
painless bite position may require orthodontic rehabilitation, or at least stabilization, by
means of an adjusted splint for night time
use.

• Hiperactividad articular, sobre todo en combinación con debilidad general del tejido conjuntivo (Figs. 2 y 2a)
• Trauma
• Trastornos del cuello/síndrome de la columna
cervical
• Enfermedades internas (trastornos hormonales, circulatorios, reumatismo).
En la ATM los trastornos internos son provocados por la hiperlaxitud articular, no por la maloclusión. En un estudio de 12 años (1997-2009),
largo plazo en nuestras prácticas de ortodoncia,
todos los pacientes con desplazamiento discal
diagnosticado (n=535) fueron examinados
manualmente para determinar su posición de
mordida, incluyendo la articulación. En caso de
duda, se recurrió a exámenes con imágenes de
resonancia magnética. Se observó desplazamiento del disco anterior, con reducción, en 421
pacientes, mientras que en 114 pacientes se
observó desplazamiento sin reducción.
La débil asociación con el tejido conectivo fue
muy significativa. Una debilidad general concomitante del tejido conectivo se observó en el
92,5% de los pacientes (Fig. 2b). En cambio, no
se obeservó una asociación significativa entre la
maloclusión y los trastornos del disco mencionados.
Por lo tanto, existen dudas sobre la indicación
de reposicionar el disco durante la rehabilitación oclusal. Este tipo de terapia es a menudo
complicada, lenta y costosa, y no sin riesgo si
es necesario el reposicionamiento. Además, no
protege contra el desplazamiento del disco, sin
reducción, en una etapa posterior.
Incluso cuando obtiene condiciones oclusales
óptimas y respeta los estrechos límites indicados, se oberva que los ligamentos posteriores
generalmente permanecen intactos. Esto es
cierto en el desplazamiento parcial del disco
anterior y cuando la mordida distal debe corregirse.

AquaSplint

The AquaSplint is a self-adjusting, customisable, pre-fabricated TMD splint which can
be inserted immediately without preparations, impressions, or registrations. The device consists of two water pads connected by
a tube (hydrostatic aquabalance). The water
pads are provided with an acrylic saddle,
which can be relined with a long-term silicone
material. Such individualization enables immediate optimum stability and comfort and,
unlike conventional hard bite splints , offers
the following advantages:
• immediate pain relief without laboratory
procedures or preparations;
• universal use and insertable in the patient’s
mouth, within a few minutes;
• self-adjusting, no grinding or frequent recall appointments required (thanks to the
hydrostatic balance between the two water

pads)
• replaces the relaxation and distraction
splint (no increased compression within
the joint in contrast to splints with frontal
bite plane)
• high accuracy and comfort of fit through
individual relining
• no more than 10 hours of daily wearing (8
hours by night, 2 hours by day)
• excellent clinical results and patient satisfaction owing to rapid pain reduction,
comfort, and advantageous repositioning
of the mandible
• higher stability and less visibility compared
to a maxillary splint
In comparison to similar aqua pads the
AquaSplint offers the following advantages:
• increased muscle relaxation: i.e. conven-

tional products must be held in position
by permanent mouth closure or muscular
activity, which may interfere with sufficient
muscle relaxation. The immanent stability of the AquaSplints (relining) and the
favourable positioning of the mandible
enable improved muscle relaxation.
• the only self-adjusting customisable TMD
bite splint applicable even during orthodontic treatment (“prior blocking out of
brackets with protection wax”) (Fig.3, 3a)
• longer life time (4-6 weeks) with hardly any
loss of fluid
• devoid of sharp edges

Indications

• Rapid pain relief, particularly in acute cases (e.g., accidents, trauma, and disc displacement, without reduction, associated
with markedly limited mouth opening and

difficult impression-taking)
• Muscle relaxation, particularly in cases
of acute or intermittent teeth grinding
(Bruxism/clenching), e.g. during stress
periods such as exams and acute psychological trauma (not suitable for chronic
bruxism)
• Identification and elimination of hidden
functional disorders, premature contacts,
forced bites
• Simple pre-prosthetic / pre-orthodontic
bite determination
• High differential diagnostic validity and
supportive therapy in associated diseases
with multifactorial aetiology (neck disorder
syndrome, tinnitus, migraine headache,
stress syndrome and sleep apnoea)
• Adjunct treatment supportive of osteopathic/physical therapy after trauma and
surgery
Due to the hydrostatic effect of the two water
pads, the jaw position and occlusal contacts
become levelled and harmonious, without
grinding-in and adjustment (conventional
bite splint), i.e. the mandible achieves a selfbalanced position.
This new pain-free / pain-reduced therapeutic
position provides the basis for subsequent
occlusal rehabilitation (prosthetic in vertical
discrepancies, orthodontic in transversal,
and sagittal discrepancies).
In addition, the three primary tasks of classic splint therapy are addressed: decoupling
of pathological occlusion, tone reduction of
hyperactive masticatory muscles, and reduction of enamel abrasion.

Convenient and simple handling

The saddle width can be adjusted, when necessary, through expansion, compression or
cutting. This makes the AquaSplint applicable to any tooth shape, crown/bridge, and
brackets or bands.The Aqua Splint can not
be used as a permanent splint without an
additional 0.5 mm thin stabilizing material
(Fig.9), as undesired occlusal alterations/
changes cannot be ruled out. The additional
0.5 mm splint has proved a good compromise solution especially for patients who are
unable or unwilling to wear a rigid bite splint,
after AquaSplint therapy. Furthermore, those
additional thin splints clearly prolong the lifetime of the Aquasplint, particularly in bruxism
cases. Usually, prosthetic/ orthodontic rehabilitation or an adjusted permanent splint
(as a compromise) should follow AquaSplint
therapy. If the malocclusion has been the
main reason of the pain, and if there is a huge
discrepancy between the old and the new therapeutic/painless occlusion, then permanent
phase 2 changes are required. In some cases,
occlusal rehabilitation is unnecessary, especially if the existing TMD mainly results from
trauma or systemic disease rather than from
the malocclusion.
Should AquaSplint therapy fail to yield considerable improvement after 4-6 weeks, stomatognathic factors can be ruled out as a main
cause of TMD. Unfortunately, such patients
cannot be helped in the dental practice, except for abrasion prevention. In these cases,
interdisciplinary consultation involving other
disciplines such as ENT, orthopaedics or psychology is recommended.

Conclusion

Our experience with the AquaSplint concept
has shown excellent clinical results and patient satisfaction, particularly due to rapid
pain reduction, wear comfort, and favourable
repositioning of the mandible. Great advantages for the provider are the simple and quick
diagnostic procedure, the effective therapy,
and the chance to dispense with impressions,
registrations, laboratory procedures and
grinding-in of hard splints. The new concept
does enable us to provide exact diagnosis,
and targeted therapy, at a reasonable time
and expense.

De acuerdo con nuestro concepto actual, el
bruxismo de por sí no requiere de tratamiento urgente. Cuando el paciente experimenta
dolor debe examinarse si los síntomas provienen de la ATM y de los músculos masticatorios y excluirse, mediante diagnóstico
manual de tejidos específicos, la posibilidad
de dolor fantasma.
El paso subsiguiente es determinar si la
maloclusión es la causa principal del dolor.
Esto se logra con mayor eficacia mediante la
neutralización/desconexión (temporal) de
la maloclusión existente con el Aqua Splint,
eliminando la posibilidad de una mordida
forzada y relajando los músculos. En casos
más graves se debe aplicar terapia manual y
terapia física adyuvante.
Los TTM relacionados con maloclusión responden por lo general a la terapia con Aqua
Splint a las cuatro semanas. El alivio de por
lo menos un 50% del dolor es una condición
favorable previa y un indicador para rehabilitación ortodóncica. El uso del Aqua Splint a
menudo modifica la mordida. Posteriormente, la nueva posición indolora de la mordida
suele requerir rehabilitación ortodóncica o
estabilización por medio de una férula de
uso nocturno.

Manejo fácil y cómodo

El grosor de la almohadilla se puede ajustar
expandiendo, comprimiendo o cortándola.
Esto permite colocarla en cualquier forma de
diente, corona/puente, bracket o banda.
El Aqua Splint no debe utilizarse como férula

permanente sin agregar una delgada capa
de 0,5 mm de material estabilizador (Fig.
9), ya que puede provocar alteraciones oclusales no deseadas. Estaa capa adicional es
una buena solución, especialmente para
pacientes que no pueden o quieren usar una
férula rígida después de la terapia con Aqua
Splint.
Por lo general, se debe seguir la terapia con
Aqua Splint con rehabilitación prótestica/
ortodóncica o, (como compromiso) la colocación de una férula ajustable permanente.
Si la maloclusión es la principal causa del
dolor, y si hay una gran discrepancia entre
la previa y la nueva terapéutica/oclusión
indolora, es necesario realizar cambios
permanentes de fase 2. En algunos casos
la rehabilitación oclusal es innecesaria, especialmente si los TTM existentes se deben
principalmente a traumatismo o enferme-

dad sistémica y no a maloclusión.
Si la terapia con Aqua Splint no produce una
mejora considerable a las 4-6 semanas, se
puede descartar que la causa principal de los
TTM sean factores estomatognáticos. Desafortunadamente, la odontología no puede
ayudar a estos pacientes excepto en prevenir
la abrasión. En estos casos se recomienda
una consulta interdisciplinaria con otorrinolaringólogos, ortopedas o psicólogos.

Conclusión

La terapia con Aqua Splint nos permite
proporcionar un diagnóstico exacto y una
terapia específica, a un costo y en un plazo
razonables.

El Aqua Splint

El Aqua Splint es una férula prefabricada, autoajustable y adaptable para la ATM que puede colocarse inmediatamente sin necesidad
de preparativos previos, toma impresiones o
