Global dental community meets in Spanish

| FDI World Dental Congress opens in Feria de Madrid | Se inaugura el Congreso Dental Mundial de la FDI, celebrado en la Feria de Madrid
Over 200 Spanish and international
providers have registered for the trade
exhibition to present their latest equipment, materials and oral hygiene products. In addition, a scientific programme
featuring 130 well-known experts from
over 30 countries is being held over the
course of the next four days. Furthermore, attendees will have free access to
special sessions, such as this year’s
World Oral Health Forum and the FDI
Science Committee Forum, which will
focus on global periodontal health and
antimicrobial resistance, respectively.
The event has been organised by
the FDI World Dental Federation in partnership with the Spanish dental association. Its president, Dr Óscar Castro
Reino, said that this year’s congress will
be a remarkable and rewarding experience and an important milestone for
dentistry in the southern European
country.
“We hope the FDI World Dental Congress in Madrid will be the dentistry
event of the decade,” he told World
Dental Daily.
For those who have not already registered, on-site registration for the congress is still open. After the recent terror
attacks in Barcelona, the organiser has
said it is taking all necessary security
measures to ensure a safe event. Attendees can obtain more information
about the FDI and its work and latest
projects, such as the World Oral Health
Day 2018 campaign, being launched
here in Madrid, at the FDI booth in Pavilion 5. The latest news and information
from the congress are available daily
in the World Dental Daily newspaper,
published in cooperation with Dental
Tribune International, and online at
www.dental-tribune.com.

Effectiveness of 21-day Brush Day & Night
programme worldwide La eficacia a nivel
mundial del programa ‘Brush Day & Night’
T
his year, the FDI and Unilever
celebrated the tenth anniversary of their partnership to improve oral health literacy and practice
on a global scale. A special symposium
during the FDI World Dental Congress
(29 August, 3.00–5.30 p.m., Retiro
room) will reflect on the 21-day programme, part of Brush Day & Night,
that concluded at the end of 2016.
For the Phase 3 programme, children were asked questions in advance
on their brushing habits, especially frequency, timing and usage of fluoride,
then followed a supervised brushing
routine to teach them the correct way
to brush. At the end of 21 days, the
same questions were asked to measure
the impact of the programme on the
children’s brushing habits. The results,
which demonstrated very positive ef-
fects, will be presented at the symposium.
The symposium will also detail how
the FDI–Unilever partnership is evolving, improving and providing invaluable information on oral health behaviour. It will discuss the effectiveness
of a one-year programme in terms of
the behaviour and knowledge about
oral health of children between 2 and
12 years of age as reflected by the impact on the presence of plaque and
dental caries and how the experience
acquired in Phase 3 has paved the way
for the move to Phase 4 of the partnership.
In addition, the relationship between the presence of plaque and dental caries and the behaviour and knowledge achieved by these children will be

presented. The experience acquired
and these results provided the necessary information to move to the next
phase of the partnership that started in
2017.
“We’ve really been astonished by
the excellent results achieved in Phase 3
of the Brush Day & Night partnership.
Children learn quickly and good habits
acquired in childhood last a lifetime.
That’s why it is so important to develop
oral health literacy at an early age, so
people can look forward to a lifetime of
oral health,” said Profs. Jo Frencken and
Paulo Melo, experts appointed to oversee Brush Day & Night Phase 3.

‘Brush Day & Night’ compone las fases
3 y 4 de la alianza FDI–Unilever, lanzada
en 2007 con el nombre de “Live.Learn.

4 apoya programas dirigidos a escolares
de Nigeria e Indonesia y colabora con

Chairside guide for clinicians available in Madrid

F
ollowing up on its already successful White Paper on Dental Caries Prevention and Management,
launched during the 2016 World Dental Congress, the FDI is this year issuing a chairside guide to advise clinicians on restorative interventions for
their patients. It is conceived as a visual aid, with easy-to-follow guidelines suitable for the dental practice.
The goal is to reduce the impact of caries as early as possible by preventing
further tooth destruction and reversing the caries process in favour of remineralisation and to ultimately empower patients to improve and maintain their own oral health.
“We believe this chairside guide is
an easy reference tool for practising clinicians and facilitates their decision-making on treatment options,” said
Dr Sophie Dartevelle, member of the
FDI Public Health Committee. “Dentists

see numerous patients in one day all
with their specific needs and anticipated outcomes. The guide is a reminder of the objective of patient care
for caries, which has to be the least invasive and must necessarily involve the
patient in the maintenance process.”
The guide will provide a greater understanding of lesion activity and determining caries risk according to varying
risk factors throughout life. These risk
factors are classified in the context of
five age groups, three covering children
and adolescents, one for adults, and
one for seniors. For each age group,
there are a caries risk assessment and
recommendations for professional
maintenance and for patient maintenance and education.
The chairside guide was developed
in the context of the Caries Prevention
Partnership, an international effort to
help shift the focus of dental practitioners from a restorative approach
to preventative dental medicine.
Launched in 2015, this joint FDI–
Colgate initiative seeks to enhance oral
disease prevention through high-quality educational communication, material and activities regarding caries that
target dental professionals, patients
and the public.
Copies of the chairside guide are
available at the FDI booth. Information
can also be found online at www.

L
Dentales (Dental Caries Prevention

tive communication, education and
training. It is widely recognised that
the education of medical professionals in appropriate prescribing and dispensing of antibiotics is critical to address misuse and overuse, a known
driver of the development of resistant
microbes.
As a first practical follow-up to the
GAP, WHO organised an initial meeting with medical professional associations in March 2016 to discuss ways of
scaling up education on antimicrobial
prescribing and dispensing, including
on how improved antibiotic prescribing practices can be achieved.
AMR AND THE

For its part, the FDI held a hot topic
session on AMR at its World Dental

existing national guidelines on AMR
through its member national dental
associations.

Bearing in mind that dentists are
responsible for up to 10 per cent of antibiotic prescriptions worldwide, the
FDI’s latest contribution to their awareness and understanding is the Science
Committee Forum at the 2017 World
Dental Congress in Madrid, themed
“Antimicrobial resistance and antimicrobial stewardship in clinical dentistry” (29 August, 3.00–5.30 p.m., Pavilion 3, Velásquez room). It will feature lectures on the scope of the problem of AMR, the dental clinical
perspective and what dentists can do
to promote antibiotic stewardship
and reduce antibiotic resistance. A further lecture will focus on the antimicrobial drug pipeline, what dentists
need to know about the challenges in
developing new antibiotics and promising new developments, as well as
provide current information, to enable
dentists to discuss potential new
treatment approaches.
E

World Dental Daily: What aspects will
your presentation highlight?
Dr Mariano Sanz: It will deal with
the efficacy of regenerative therapies
in the treatment of periodontitis. In
this presentation, I will review the results of both graft-based and membrane-based technologies, which
have been the standard of treatment
for many years, as well as new biological approaches using growth factors
and cellular therapies within the
framework of tissue bioengineering.
My fundamental message will be that
we currently have regenerative technologies that allow us to maintain the
natural dentition and to save teeth
even when they are seriously compromised by periodontitis.
What are the latest developments in
this area?
Regenerative therapy is a field in
which much progress has been made
in recent years, not only owing to technological advances like the development of new biomaterials, but also

And on a personal level?
Firstly, the magnitude of the event.
This congress is of global significance
for our profession, and therefore it is a
privilege to have the opportunity to
participate in the scientific programme. Secondly, that it is taking
place in my home city of Madrid is an
additional advantage that will allow
me not only to share my work, but also
to promote the Complutense University of Madrid, where I work and of
which I am very proud.

gress in our country, and general dental practitioners should make use of
the opportunity.
Likewise, much progress has been
made in our knowledge of the organisation and physiology of the communities of microorganisms that colonise
the oral cavity and are responsible for
most of the pathologies we deal with,
including dental caries, periodontal
disease and peri-implant disease. This
knowledge has allowed the development of new antimicrobial strategies
that are more effective and offer great
potential in the prevention of oral disease.
What does it mean to attend an event
of this nature?
It is a great opportunity to update
one’s knowledge. An international
event like the 2017 FDI World Dental
Congress in Madrid covers all areas of
dentistry, and for this reason, it is very
attractive to general dentists seeking
to update their knowledge in their respective fields. It is a significant advantage to be able to attend such a con-
2017.
World Dental Daily: ¿Qué aspectos

asistir.
¿Y a nivel personal?

Caries prevention is not just more fluoride
La prevención de la caries
no es simplemente usar más fluor
| By FDI presenter Prof. David Manton, Australia
| Por Prof. David Manton, Australia
A
behaviourally driven bacterial
disease, dental caries is one of
the most prevalent primarily
preventable conditions in the world.
Even in developed countries with
water fluoridation and ready access
to fluoridated toothpaste, a large
proportion of children still have experience of advanced dental caries.
In the past 50 years, the prevention
of dental caries has focused mainly
on the delivery of fluoride, whether it
be via reticulated water, toothpaste
or professionally delivered products,
such as varnishes and gels.

A sound knowledge of the causative factors of dental caries, however,
is important in the process of developing a holistic preventative programme
for the individual. Demineralisation
occurs when the fluid at the surface of
the tooth becomes undersaturated
with respect to tooth mineral. This is
normally due to a decrease in pH below the often-stated critical pH of
around 5.6. The critical pH is not a constant, but relates to the amount of calcium and phosphate present, as well
as the pH. The microbiome of the dental biofilm develops after birth, and is
influenced significantly by the inges-
tion of fermentable carbohydrates, especially sucrose and fructose. The production of organic acids from the metabolism of sugars decreases the pH in
the biofilm, creating an amphibiotic
change to an aciduric and acidogenic
microbiota, also known as Marsh’s
ecological shift. Dietary change to infrequent ingestion of sugary foods
and drinks therefore is the first step in
prevention of dental caries.
In addition to dietary control, fluoride is still considered the gold standard for caries prevention. However,
the action of fluoride, especially for
remineralisation, is limited by the concentration of bioavailable calcium and
phosphate in the local environment
10
(biofilm fluid). Remineralisation is only
possible in the presence of bioavailable calcium and phosphate, with the
intrinsic source being saliva, a limited
resource. Recently, several products
containing calcium and phosphate
have been developed and are available commercially. While a number
of claims of efficacy have been made,
the evidence for many is still limited.
For example, casein phosphopeptideamorphous calcium phosphate (CPPACP) is a milk-derived product that utilises a peptide to stabilise calcium and
phosphate in a bioavailable state—reacting to acidity and saturation with
respect to tooth mineral. Incorporation of CPP-ACP into products such as
a topical crème can provide high concentrations of calcium and phosphate
ions that can diffuse into demineralised tooth structure down a concentration gradient. This remineralisation
is enhanced by the presence of fluoride ions.
The inhibition of demineralisation
and subsequent remineralisation of
early carious lesions (white spot lesions) is sometimes considered unlikely owing to an inability of the patient to reduce risk factors, such as a
sugary diet, poor oral hygiene habits,
including interdental cleaning, as well
as poor salivary flow and possibly developmental defects of enamel. In
these instances, the use of sealing or
infiltrating agents, such as fissure
sealants or resin infiltrate, can be effective, although remineralisation of
the treated area may not be possible
afterwards. These products either seal
the surface of the lesion or infiltrate
the porosity of the lesion to decrease
the loss of mineral from the tooth, and
can prevent the lesion progressing to
cavitation.
The prevention of dental caries is
far more than just fluoride. A sound
knowledge of the caries process assists the clinician in formulating an individualised preventative programme
for patients.
L

evidencia sigue siendo limitada.
|Prof. David Manton heads the Growth and Development section (paediatric
dentistry and orthodontics) at the University of Melbourne’s dental school. Tomorrow, he is presenting a paper titled “The prevention of dental caries—It is a
lot more than fluoride” as part of this year’s congress programme in the Miró
room.

Aadva IOS 200: A sneak peek into the future of oral scanning
© GC
© GC
| Some things are so exciting they just
can’t wait, and one is the world’s most
advanced intraoral scanner, made by
global dental specialist GC. The potential for digital impressions is huge and
extremely attractive for dentists, and
the Aadva IOS 200 will make it easier
than ever to take the plunge. The developers at GC place great importance
on ergonomics and an intuitive workflow, and Aadva IOS 200 will excel in
these areas. The system features a
completely redesigned handpiece as
well as highly accurate scanning and
fast scan speeds. The control and display features are also designed with intuitive operation and optimal ergonomics in mind.
Among the user-friendly features
of the Aadva IOS 200 are the removable autoclavable tip, which contributes to a hygienic practice; the innovative “open-mirror” design, which
allows for easy scans of the distal parts
in the molar areas; and the anti-fog
function, which guarantees fog-free
recordings.
One of the most obvious benefits
of the Aadva IOS 200 in daily use will
be its convenience. For instance, the
system requires no powder, so the
scan can be initiated immediately, saving time and work and patient discomfort—a key advantage of digital scanning over traditional impression-taking. The entire scan flow is visualised
on the screen in real time and the process can be paused at any time to
check the result. You can then continue the process or start again if you
are not satisfied.
With an intuitive workflow being a
must-have, the Aadva IOS 200 designers have made sure that just a few simple steps are needed from scanning to
exporting the data. The updated
Aadva IOS 2.0 software has an uncluttered user interface that guides operators through the various steps, from
validation and defining the preparation margins through to data export.
And another major plus: because the
bite registration happens in real time,
the manual step of bite alignment is
no longer necessary. Intraoral scanners like the Aadva IOS 200 allow dentists to enter easily the digital world
and simplify the workflow between
the treatment room and the dental
laboratory.
© GC
The Aadva IOS 200 is designed as
an open system, so the scanner uses
the STL standard. The Aadva Digital
Services Platform (DSP) allows the
structured management of scanned
cases and data transfer to dental laboratories, partners and milling centres.
The status of the case can be tracked at
any time. Alternatively, the open STL
data can be easily and immediately
exported on a USB, without having
to send or upload the file to external
parties for validation.
| Algunas cosas son tan emocionantes
que no pueden esperar. Una de ellas es
el escáner intraoral más avanzado,
fabricado por GC, especialista dental
mundial.
las partes distales de las áreas molares;

Greater safety and precision in 3-D
Más seguridad y precisión en tercera dimensión
| Very fine structures have to be detected for endodontic procedures, for
example the number, location and
shape of root canals. Therefore, commonly used radiographic systems
have to be as precise as possible. CBCT
has become indispensable for many
endodontic practices, as this imaging
technique supports optimal planning
and helps dentists provide comprehensive information to their patients.
Morita’s 2-D/3-D combination system Veraview X800 makes an impression not only because of its modern
design, but also because it provides
extremely sharp 3-D, panoramic and
cephalometric radiographs. According to the company, it delivers brilliant
image quality with a voxel size of
80 μm and resolution of 2.5 LP/mm
MTF 10 %. Highly precise scans—both
in 180° and 360° mode—can be obtained with the unit. The wide spectrum provided by 11 fields of view
covers many fields of dentistry, particularly endodontics.
To increase the sharpness of the
images and to minimise artefacts,
Veraview X800 uses a horizontal X-ray
beam that shifts by 5°. Thus, disruptive
shadows on the hard palate are pre-
18
vented in panoramic scans. The X-rays
can be recalculated afterwards using
the zoom reconstruction function. An
80 μm voxel image, for example, can
be reconstructed from a 125 μm voxel
scan without having to take a new
scan.
In addition, panoramic scans have
been optimised. The Adaptive Focal
Point analyses several layers of acquired images, then chooses the optimal panoramic layer for each region
and pieces these layers together to
form a new image. The Adaptive Gray
Scale function further ensures perfect
contrast.
The time-proven Reuleaux format of the predecessor model,
Veraviewepocs R100, has also been
integrated. Its cylindrical shape significantly reduces the radiation dosage.
imagen. La Escala Adaptativa de Grises
(AGS) garantiza además un contraste
perfecto.
También se ha integrado el formato
Reuleaux del modelo predecesor
Veraviewepocs R100. Su forma cilíndrica reduce significativamente la dosis
de radiación.

to apply for this award.
The Sunstar Foundation will deliver the 6th edition of the award event
in 2018 under the EuroPerio9 framework, the international congress of
periodontics. In addition, this year the
Foundation will present an innovative
initiative by adding a new award to its
research project.
In its quest to continue the task of
scientific and social dissemination regarding periodontal diseases and
their impact on global health, in 2017
the Sunstar Foundation also launched
another important prize. The
purpose of the Perio Link™
Award is to raise awareness of
the relationship between oral
health and general health,
communicating scientists’ latest research on periodontics to
the general public.
Through this new award,
the Foundation wants to communicate the importance of
considering the human body
as a whole, as an organism
whose functions are interconnected and impact on one another. In addition, the Foundation would like to ensure that
this message reaches society
at large, to also include it as a
participant in the selection of
the winners for this award.
The scientific papers presented at the 1st edition of the
Perio Link™ Award will be
available on the Sunstar Foundation website so that the
public can choose the most
interesting, playful or useful
research they believe will
encourage social awareness
and discussion in these health
areas.
These two awards add to the
many initiatives that Sunstar has
been working on for more than 20
years. For this reason, the Foundation
presents itself as a key driver of research and social outreach, with its
purpose of making the public more
aware of their health and the scientific activities that are currently being

Spark intraoral scanner
Escáner intraoral Spark
| French start-up Innixim combines
50 years of know-how with innovation. Established by industry experts,
including Sopro founder and former
ACTEON Imaging CEO Pierre Montillot, and a new generation of extraordinary engineers, the company has
taken radical steps to design the future of dental imaging to offer dentists efficient tools and provide better comfort for patients. The result of
this philosophy is Spark, the first personal intraoral scanner, which is on
display at the FDI World Dental Congress.
Innixim has combined the benefits of different scanning technologies, but eliminated their weaknesses.
To achieve this performance, the
company used patented Line Stream
Scanning technology, which avoids
all of the problems experienced with
common scanners. With dimensions
of only 11.5 × 18.0 × 5.5 cm, Spark is
very small, allowing the dentist to
keep it at the workstation and removing the need to share scanners. By far
the fastest unit on the market, according to the company, it scans an imaging plate in only three seconds. Moreover, Spark only requires dose exposures as low as 80 ms when using the
company’s special high-definition imaging plates. Moreover, it features
outstanding image quality, providing
a 35 LP/mm resolution.
With Spark, dentists will receive
the performance and effectiveness of
a sensor and patients will appreciate
the enhanced comfort, Innixim said.
The scanner is available worldwide
through the company’s dealer network.

PracticeDent no precisa instalación
