| Global dental community meets down under in person after four-year hiatus

T

he World Dental Congress is the
flagship continuing education
event of FDI and a unique opportunity for dental professionals to
strengthen ties and foster collaboration
within the global oral health community. After four years, the congress has
returned to a face-to-face format and
is the first in-person World Dental
Congress since the one that took place
in San Francisco in the US in 2019.
The event last took place in Sydney, the
largest city in Australia, in 2003.

The 2023 congress has been organised jointly with the Australian
Dental Association (ADA). “The ADA
and FDI World Dental Federation have
worked collaboratively over a journey
spanning years of global disruption to
bring the dental community this landmark World Dental Congress, the first
one held face to face since 2019,” said
ADA Federal President Dr Stephen
Liew [read the full interview on page 4].
“We were well into our preparations
for the 2021 congress when the global

pandemic hit and had to make many
tough decisions to delay the in-person
event, pivoting to utilise our assets in
virtual form. It was a difficult journey,
albeit we were incredibly happy with
the virtual congress we held for the
world in 2021,” Dr Liew remembered.
At the International Convention
Centre in the heart of the harbour city,
the 2023 congress brings the global
dental community together to share
ideas, knowledge and experiences.

FDI and ADA have compiled a cuttingedge scientific programme that features
more than 220 sessions. Attendees have
the opportunity to meet with and learn
from leaders in the oral health profession
from Australia and around the globe.

Mr Moed, you have already taken

what makes Romexis the gold standard of radiographic software?
With the all-in-one Romexis software platform, all systems or devices
can be linked together. This means
that all data can be collected,
viewed and processed in one system, making collaboration faster and
more flexible. There are also options
for real-time monitoring.

During the development of
Romexis, the key focus was on excellent user friendliness and intuitive
handling as well as sufficient flexibility. To ensure that it is always
compatible and that the respective
practice management software can
be connected easily, the Romexis
architecture has an open design and
supports the macOS and Windows
operating systems.
Mr Moed, networking and digitalisation will certainly play a decisive
role in the future, but services remain

Mr Kyöstilä, what does this mean
for dental professionals and their
patients?
Workflows in practices are becoming more and more integrated
and therefore smoother and faster.
Treatment planning is also being
optimised, and the treatment results
will therefore be not just better but
also more predictable. I think that, in
future, we will see a steady improvement in patient care and safety,
which is naturally expected of us as
a premium manufacturer. Together,
KaVo and Planmeca are ideally positioned for this and look forward to more
exciting projects and developments
that will help to improve modern

Dr Liew, it has been four years since
the last in-person World Dental
Congress. How excited are you about
the congress, and what are you
personally looking forward to at this
year’s event?
In this period of social reconnection,
it is a great honour and exhilarating
to prepare to welcome thousands of
delegates from Australia and across
the world to our renowned harbour
city of Sydney as the ADA federal
president.

atmosphere.

he countdown is finally over.
Four years after its last in-person
event, the FDI World Dental
Congress is making a return. This year,
the congress will be hosted in Sydney
in Australia and promises a well-crafted
scientific programme and abundant
opportunity for social interaction.
Dental Tribune International spoke with
Dr Stephen Liew, president of the Australian Dental Association (ADA), which
is hosting the event, about the growing
excitement in the run-up to the congress
and discussed the enormous amount of
work that has gone into the planning
and execution of the congress—all to
expand the knowledge and skills of
dental professionals worldwide.

As the global dental profession,
we provide vital oral healthcare to improve the quality of life of the global
population. To do this effectively,
we first must keep our knowledge at
the forefront of clinical excellence via
evidence-based learning. Secondly, our
enjoyment of practising our unique
skill set is enhanced by the warmth
of collegial shared purpose, which
crosses all borders.
The World Dental Congress is the
ultimate manifestation of key professional ideals, and I cannot wait to
share our efforts with all attendees,
overlooking the world-famous Sydney
Harbour and iconic sights.
It takes a tremendous amount of
work and dedication to organise an
event of such scale. Could you tell us
a bit about the ADA’s contributions to
bringing the congress to Sydney?
The ADA and FDI World Dental
Federation have worked collaboratively

acknowledge the substantial efforts of
our national organising committee,
chaired by Dr Shane Fryer; our events
team, led by Oscar van Elten and Shana
Ludwell; and our event partners at FDI,
led by Enzo Bondioni and FDI President
Prof. Ihsane Ben Yahya in delivering an
event of this magnitude.
We aimed to ensure that collegial
connections are enhanced and gained
during the event, facilitated by social
highlights spread throughout the
programme and unique wayfinding at
International Convention Centre Sydney
designed to lead to many spontaneous
catch-ups. Bringing the world of dental practitioners together is a privilege,

“There is so much opportunity to improve the quality
of life of patients with head and neck tumours”
| An interview with Prof. Jonathan Clark, head and neck cancer expert

major impact on the effectiveness of
treatments. There are also new treatments, such as robotic surgery, highly
precise radiotherapy techniques and
new drugs, available. We are now
entering the age of immunotherapy,
which has transformed previously
incurable skin cancers into curable
malignancies. In the future, we will see
the effect of human papillomavirus
vaccination. Australia has been very
progressive in this regard, but it will
probably be another 15 years before
we see the rate of oropharyngeal cancer
decline as a result of vaccination.

| Prof. Jonathan Clark is a leading researcher and surgeon in head and
neck cancer and advanced reconstruction.

Prof. Clark, how did you become interested in head and neck cancer?
I was fortunate to work with the
late Prof. Chris O’Brien, who sadly died
of a malignant brain tumour. He was a
world leader in head and neck cancer
surgery and an inspiration for many
surgeons of my generation. During
my fellowship, I travelled to Toronto,
where I learnt about microvascular
reconstruction. This has transformed
the way that head and neck cancer
surgery is performed by restoring
form and function in patients undergoing radical surgery. I have focused
on reconstructive surgery because it
is at the cutting edge of surgery and
integrates many aspects of surgery
that I enjoy, particularly because
there is so much opportunity to improve the quality of life of patients
with head and neck tumours.
What significant changes in head
and neck cancer research and treatment have you seen throughout
your career?
There have been major changes
in the demographics of those who
develop head and neck cancer.
Traditionally, head and neck cancer
was mostly caused by smoking in
patients of low socio-economic status,

but now the most common head
and neck cancers are not related to
smoking. For example, oropharyngeal
cancer is caused by the human papillomavirus and cutaneous squamous
cell cancer by exposure to ultraviolet
light, and oral cancer is rising in
incidence in young non-smokers,
although we cannot explain this trend.
Australia has had very effective antismoking policies, which we should be
grateful for.
Cancer outcomes are improving
gradually. There are a variety of reasons for this. In part, this is because
cancers not caused by smoking have
a better prognosis, but head and neck
cancer treatment has been a leader
in multidisciplinary care. This has
enabled patients to reliably complete
quite taxing and difficult treatments.
We often under-estimate how important it is to do the basics well.
There continues to be significant
unwarranted variation in how treatments are delivered, and this has a

Research is much more sophisticated in many ways, but we are only
just starting to understand the molecular basis of head and neck cancer.
It is extremely diverse—there are ten
different head and neck cancer sites
and multiple types at each site—so
we have struggled to find drug cures
and targets. This has been frustrating
compared with, say, breast cancer,
leukaemia and melanoma, for which

How has the awareness of head and
neck cancer changed over time?
I do not think that awareness is
very high. Most people think that
head and neck cancer is brain cancer
or that it has something to do with
the spine. We have failed to advocate
effectively or to have high-profile
champions. There is no Movember or
pink ribbon for head and neck cancer.
Most head and neck cancers, individually, are uncommon, but as a group,
it is the sixth most common cancer.
If skin cancer is included, it is the
most common, so it depends on what
you consider head and neck cancer.
In Australia, we treat a lot of skin cancer,
so we tend to include it in the term
even though most skin cancer lesions
are small in extent and are managed
by general medical practitioners (GPs),
dermatologists and plastic surgeons.
Owing to these factors, it is challenging to get a consistent message out in
the community. Our main focus is oral
cancer, and most people understand
what that is.

Screening is challenging. Each
head and neck cancer is different, so
there is never going to be a simple
screening test like mammography,
a prostate-specific antigen test or
faecal occult blood test. We might
have a blood test for oropharyngeal
cancer that could be used for screening in the future. In the meantime,
the education of patients, GPs and
dentists will have the greatest impact.
There are very simple messages that
we can get out into the community—
things like:
• A mouth ulcer that has remained
unhealed for over four weeks needs
referral.
• An ill-fitting denture or loose teeth
require an examination to exclude
gingival cancer.
• A neck lump present for over
four weeks in an adult should be
considered malignant until proved
otherwise.
• A progressive facial palsy is not
Bell’s palsy.

chemotherapy and targeted therapies
have dramatically improved survival
rates. The reasons are multifactorial,
but partly because the diversity
spreads resources thin, the genetic
changes present are not well suited
to targeted therapies. Also, typically,
head and neck cancer research has
been vastly underfunded compared
with research into breast cancer and
prostate cancer.

It has been historically difficult to
diagnose head and neck cancer early.
What are some obstacles to early
detection, and how can screening
for this group of cancers be improved?
Most head and neck cancers do
not cause symptoms until the cancer
is advanced. One of the most common signs is a painless neck lump,
which means the cancer has already
spread to the lymph nodes. Some
early cancers do cause symptoms.
For example, small laryngeal cancer
lesions may cause a hoarse voice, but
they are difficult to see. It is also very
difficult for GPs and dentists to detect
throat cancer and many other head
and neck cancers.
Oral cancer is different. It is often
quite easy to see with a headlight,
though most GPs do not own one and
probably do not see enough cases to
recognise what is concerning or not.
Even dentists do not detect many oral
cancer cases, so they can easily be
missed. Traditionally, the higher prevalence in low socio-economic groups
has meant that patients often do not
see their GP unless they are really sick,
and they are certainly not going
to pay for a routine dental check.
However, as the demographics change,
this is becoming less of an issue.

• Ear pain without an explanation
could be throat cancer.
In your opinion, what is the most
challenging part of treating patients
with head and neck cancer?
We do major surgery which often
exceeds 10 hours, sometimes in
elderly patients with multiple comorbidities, in whom complications can
be catastrophic. I also do a lot of
reconstructive surgery and spend
a large amount of time trying to
improve the function and aesthetics
of patients who have had large
portions of their tongue, jaw or face
removed. I very frequently see patients
with problems, whereas patients
who are doing well do not need
the same intense follow-up. This can
be disheartening. Needless to say,
it is much worse for the patients
who have to live with these problems
than it is for the clinician, but it is still
difficult work.

expectations for the event?
I look forward to four days of learning,
meeting colleagues and planning
future collaborations. To be asked to
speak at so many sessions at a meeting with such an impressive speaker
line-up is truly humbling. I feel deeply
honoured. It is my first visit to the land
down under, and I look forward to the
entire trip!
What topics will you be discussing
in your presentations?
I will be talking about various
topics, including how intricately oral
health and overall health are inter-

woven with the detrimental effects
of vaping and the serious threat of
antibiotic resistance.
Antibiotic stewardship continues
to be a global public health concern,
and various studies have shown that
dental professionals often unnecessarily prescribe antibiotics to patients.
What is the current situation regarding
antimicrobial resistance in dentistry?
Have you noticed a shift in dental professionals’ awareness of the topic?
We are learning more about how
we, as a profession, have contributed
to antibiotic resistance, but progress is
still needed. For example, my patients
ask me every day to recommend a
good mouthwash. There is so much
hype surrounding these products that
we forget that they are antimicrobial
and that using them continually can
negatively affect our oral microbiome
and therefore our overall health.
Tobacco use has a profound impact
on oral health. What recent efforts

have been made to promote tobacco
use cessation in dentistry?
I am the chair of the American
Dental Association Council on Scientific
Affairs, and we have recently conducted
a survey of our expert panel of dentists
on their attitudes and practices towards smoking cessation counselling.
We have learned that most dentists
wish that they had access to more
resources to recommend to their patients and more training to have these
difficult conversations. Smoking is a
preventable cause of disease, and we
need to equip our dental students
with more knowledge on how to help
their patients through counselling and
other intervention.

you talk us through some of the
main takeaways of the discussion?
One of the main things that struck
me in our discussions was that, although we had a very diverse group
from countries with different oral
health systems, the challenges and
some of the potential solutions were
quite uniform for us all, although with
some local contexts and nuances.
The second point that came through
strongly was that we have a real window of opportunity because both the
concept of universal health coverage
and the profile of oral health are high
on the international and national
policy agendas. Finally, it was clear
that the issues and solutions are

complex and multifaceted, including
both the oral health and wider workforce, the role of the private sector, the
information we collect and hold, the
way services are delivered, the financing of oral health services and the
priority of oral health in policy and financial agendas. However, we heard
about many great examples where
these issues had been dealt with, leading
to positive improvements for patients
in access and their oral health.

As a result of the round-table discussion, FDI released an advocacy
white paper that includes a practical
guide on steps that need to be taken
to expand access to oral care worldwide. What are some of these steps?
The first steps include better advocacy at all levels for the importance
of oral health and access to primary
oral healthcare. FDI has prepared an
advocacy toolkit to help with this,
and I will be chairing an Alliance for
a Cavity-Free Future session on the
topic at the 2023 FDI World Dental
Congress.

What are some of the barriers to
integrating oral healthcare into
universal healthcare?
The key barrier is that, historically,
oral health has been separated from
the rest of health. This is evident
in the way we train healthcare professionals, how dental services are
financed, where dental services are
provided and how patients access
oral health services. This means that
there are barriers at multiple levels in
terms of national policy and regional
implementation and at individual
patient and healthcare professional
level. Overcoming these barriers is
not impossible, but does require
action and advocacy at multiple levels
and with many stakeholders.

good practice in terms of innovative
workforce solutions, including inter-

occur. The systemic health effects

when it comes to chewing function, malnutrition and sarcopenia.
Another good example is oral
health-related quality of life and
reduced social participation.

longevity. He has published over
300 research articles and 50 books
on oral health promotion.

s the elderly population
continues to grow exponentially, it is now more
important than ever to address
certain challenges related to the
oral and overall health of older
adults to ensure healthy longevity.
In this interview, Dental Tribune
International speaks with Dr Kakuhiro
Fukai, director of the Fukai Institute
of Health Science in Japan and
chair of FDI’s Oral Health for an
Ageing Population (OHAP) task
team. Drawing on his decades-long
experience of global oral health,
Dr Fukai discussed some pressing
issues related to the oral healthcare needs of older adults and
shared some wisdom on how to
best address them.
Dr Fukai, how does ageing affect
oral health, and how can maintaining good oral hygiene improve the oral and overall health
of older adults?
Ageing leads to a decline in
the physical and mental functions

Maintaining good oral health is
crucial for lifelong quality of life,
and it has been shown to contribute to the prevention of
non-communicable diseases, cognitive decline and frailty. Research
also shows that oral healthcare
maintenance helps to prevent
dental caries, periodontal disease,
tooth loss and a decline in oral
function, thereby contributing to
healthy longevity.
What role do dental professionals
play in managing the oral healthcare needs of an ageing population?
Dental professionals contribute
not only to oral health maintenance but also to the prevention of
non-communicable diseases and
frailty as well as the promotion of
social participation. They do so by
delivering quality preventive care,
including education, and dental
treatment via the life course approach, that is, by preventing and
controlling caries, periodontal disease, tooth loss and a decline in
oral function. It is also crucial that
dental professionals work on improving the early detection of oral
cancer in an ageing population.
Owing to the complex interrelatedness of oral and systemic health,
dental professionals should work
in close cooperation with a variety
of other healthcare professionals.

and treating older adult patients,

dental professionals first need to
be aware of patients’ multimorbidity and medication regimens.
They must assess the physical
and mental status of patients, for
example robust, frail or dependent, and then proceed to the
objective assessment of gradual
changes in oral function owing to
ageing.
To detect oral function decline
as early as possible, it is not enough
to perform these assessments in
the isolated context of outpatient
dental services. Regional community healthcare networks need to
establish collaborative assessment
and/or information-sharing systems that integrate the efforts of
dental clinics, nursing homes and
perhaps even families and companies.
In 2015, FDI launched the OHAP
project to strengthen the role of
the oral health community in
achieving healthy longevity for
patients. What are the overarching goals of the initiative,
and how has it helped spread
awareness of the topic?
The Japan Dental Association
and the World Health Organization
organised the 2015 World Congress,
during which the Tokyo Declaration was issued. This declaration
emphasised that lifelong oral
health is a fundamental human
right, called for the inclusion of oral
healthcare in all healthcare policies and recognised the essential

role of oral healthcare in maintaining quality of life.
Based on the Tokyo Declaration, FDI established the OHAP
project to raise the awareness of
national dental associations of oral
healthcare for an ageing population, to assess national or regional
oral healthcare policies and share
that information globally, to identify solutions and to address these
issues at the global and national
level. Season I (2016–2019) of the
OHAP project produced a guide
for managing the dental treatment of older adults, an oral
healthcare pathway for older people and a road map for healthy
ageing, all of which are on FDI’s
website.
Building on the resources produced during Season I, Season II
(2021–2023) of the OHAP project
seeks to further promote clinical and community initiatives to
achieve optimal oral health in
older adults. The focus of Season II
is on clinical issues such as oral
functional decline. Season II aims
to promote widespread dissemination and use of the Season I
resources and to adapt them for use
in different settings and groups, to
promote awareness of the role of
oral function in elderly oral health
and to provide practical tools for
patients and oral health professionals to prevent functional decline. To achieve these objectives,
the OHAP team has surveyed na-

tional dental associations, published supplementary issues of
the International Dental Journal,
organised symposia at World
Dental Congresses and held various webinars.
What resources are available to
help guide dental professionals
in their journey to better address
the oral health-related challenges
of older dental patients?
One of the products of OHAP
Season I was the Managing Older
Adults: Chairside Guide. This document provides a wealth of prevention-oriented advice for dental
professionals according to each
patient’s level of dependency. In
OHAP Season II, the guide has
been further revised, made more
user-friendly for dentists and developed as a web app. In addition
to the level of dependency, the
guide now includes oral frailty as
a preliminary assessment for older
patients.
Validated scales were used for
these assessments. Additionally,
clear and concise infographics are
provided for patients and caregivers based on these assessments.
These tools increase dentists’ basic
knowledge of and competence
in dealing with older people.
We also believe that they will enable
interactive and shared decisionmaking between dentists and
older patients, ensuring a more
people-centred and functionfocused approach.
Specific details will be presented at a symposium organised by OHAP at the 2023 FDI
World Dental Congress, which
I cordially invite those interested
to attend.

Antimicrobial resistance:
Why should I care?
Antimicrobial resistance (AMR) is
recognised as one of the greatest
threats to global health by the World

expected to be responsible for ten
million deaths each year by 2050. This
issue is of particular concern to dental
teams, as dentists issue around 10% of
all antibiotic prescriptions for humans.

WOHD 2023 marks the final year
of the three-year Be Proud of Your
Mouth campaign, an empowering call

Cleft care:
The beauty of scars and difference
Comprehensive cleft care for those
born with a cleft lip and/or palate can
involve a range of care, including nutrition support, surgery, speech therapy,
orthodontics, other dental care and
psychosocial support. Treatments, such
as those involving multiple prosthetic
appliances, can be extensive and tiring.
They are, however, essential to help
babies and children swallow, eat, grow
and learn to speak. Digital technologies
are key to improving universal access to
cleft care, especially in remote areas.
In this episode, Ashley Barbour, a
teacher consultant from the US who was
born with a bilateral cleft lip and palate,
and Dr Juan Pablo Gomez Arango, an
associate professor of orthodontic biomechanics at Universidad Autónoma
de Manizales in Colombia, explore the
potential of digital technology to increase access to cleft care. They also delve
deeper into how growing up with cleft
and receiving treatment throughout life
can help people have successful, meaningful and purposeful lives.

Oral healthcare access:
Insights and solutions
Oral health is recognised as a basic
human right and a fundamental aspect of overall health and well-being.
Despite all the evidence, oral healthcare services are not included in
universal health coverage programmes
in many countries. In countries where
some coverage exists, essential preventive and clinical services show large gaps,
especially for vulnerable, disadvantaged,
rural or poorer populations.
In March, FDI organised a full-day
round table to review existing evidence
on the challenges of universal access
to oral care. Participants discussed
what constitutes appropriate access to
oral care and reviewed replicable best
practices for expanding access to oral
care through integration into primary
healthcare systems, including underserved populations.
In this episode, three of those
participants—Dr Irving McKenzie, chief
dental officer in the Jamaican Ministry
of Health and Wellness; dental surgeon
Prof. Manu Mathur from Queen Mary
University of London; and Dr Grace Dubois,
senior policy and advocacy manager
at the NCD Alliance—represent the
perspectives of their various fields,
including clinical practice and service
management, policy analysis, advocacy
and academia. Their conversation
provides a robust picture of the key
elements of appropriate access to oral
care.

FDI’s Sustainability in Dentistry initiative:
Paving the way for a greener future

As part of the project, FDI released
the groundbreaking Consensus Statement
on Environmentally Sustainable Oral
Healthcare, a comprehensive report that
identifies the challenges faced by the
dental profession and industry in adopting sustainable practices and proposes
solutions to overcome them. Moreover,
the initiative has gained widespread support from and participation by dental

care. The summit is available to watch on
demand, ensuring that dental professionals
and stakeholders worldwide can access
and benefit from the valuable insights
shared during the presentations and
through the poster abstracts.

acutely aware of the pressing
need to address climate change
and adopt sustainable practices across
all areas of society. The healthcare sector,
including oral healthcare, is also a contributor to global greenhouse gas emissions and therefore has the responsibility
to mitigate its environmental impact.
FDI’s Sustainability in Dentistry project,
launched in 2021, strives to promote
high-quality dental care with a focus on
prevention of oral disease, which is at
the core of ensuring an environmentally
sustainable practice. By equipping dental professionals and organisations with
valuable resources, the initiative aims
to guide them towards reducing their
environmental footprint.

toolkit, which provides dental professionals with practical guidance on improving sustainability in their practice.

tainable Dentistry, demonstrating their
dedication to reducing their environmental impact.
In 2023, FDI launched the new phase of
the project, bolstered by valuable support
from industry partners, including Colgate,
Dentsply Sirona, Haleon, Kulzer, SDI and
Sunstar, highlighting their commitment to
this vital initiative. Its primary focus is to
develop educational resources and advocacy

guidelines to support the dental profession
in their sustainability journey.
As part of the new phase, the Sustainability in Dentistry Summit was held on
World Environment Day in June, presenting captivating case studies, innovative
ideas and beneficial research projects that
put the spotlight on the dental profession
and industry’s commitment to reducing
the environmental impact of oral health-

Exploring the bidirectional
­relationship between
oral and general health

ral disease affects almost half the
world’s population. Nearly 3.5 billion
people suffer from oral health con­
ditions, making oral disease the most pre­
valent non-communicable disease globally.
Although it is known that oral health issues
can have an impact on overall health,
well-being and quality of life, oral health has
been isolated within health systems in many
countries, separating the mouth from the
body and under-estimating the importance
of oral health for general health.

health, and poor oral health can increase the
risk of developing other non-communicable
diseases. Poor oral health and untreated oral

each is able to influence and worsen the
other. Periodontitis during pregnancy is
linked to low birth weight, preterm birth
and pre-eclampsia.

The impact of poor oral health on the specific condition has been graded as causal,
worsening or association. This information
can be used to advocate for enhanced

ICD transforms lives in Vanuatu

T

he International College
of Dentists’ (ICD’s) Global
Visionary Fund and Australasia section have united their
efforts to bring lasting change to
underserved communities in the
South Pacific nation of Vanuatu
through their support of the
country’s healthy teeth school
programme, Gudfala Tut Skul. This
collaborative initiative has not only
improved oral health among children but also positively affected
families, communities and local
healthcare systems.

| Currently, Gudfala Tut Skul is operating in schools and kindergartens at
Port Vila, Luganville and Sulphur Bay on the island of Tanna in Vanuatu.

In 2017, the charity Medical
Sailing Ministries, which transports medical and dental teams
to all corners of the country,
conducted a national oral health
survey in Vanuatu, revealing alarming rates of dental health issues,
particularly among the young
population.

12–14 October 2023

Recognising the urgent need for
intervention, the Gudfala Tut Skul
programme was born, aiming
to instil lifelong oral hygiene
habits and reduce dental ailments.
This visionary project began as a
20-week supervised toothbrushing trial using fluoride toothpaste
among Grade 1, 6 and 9 students
at the central school in the capital,
Port Vila. Supported by the ICD
and other organisations, Gudfala
Tut Skul’s impact has been nothing
short of transformative.
Starting with just six schools
and early childcare and education
centres in 2019, the programme
has expanded its reach year after
year. Thanks in part to grants from
the ICD’s Australasia section and
Global Visionary Fund, the programme now reaches 72 schools
and early childcare and education
centres—over 4,000 children and
their families!
This initiative has improved
oral health and has increased
awareness about the importance
of proper oral hygiene and the
adverse effects of excessive sugar
consumption on overall health. It
has instilled a sense of community
responsibility, each child’s influence extending to at least four
family members. The ripple effect
has multiplied the programme’s
impact, fostering healthier habits
within families.
The ICD’s support, alongside
contributions from FDI, Rotary and
others, has been instrumental
in driving this initiative forwards.

FDI and Henry Schein working to introduce
integrated electronic health records to dentistry

and elevate patient care to new heights.

E

lectronic health records (EHRs)
have the potential to greatly
improve the overall quality of
dental care and research. FDI is undertaking an ambitious project that
aims to lay the foundation for EHRs
in dental offices around the world,
and the project is being generously
supported by Henry Schein. Dental
Tribune International (DTI) spoke
with experts at FDI and Henry Schein
about the project and about why EHRs
are important and how they can
improve patient outcomes.
FDI’s Integrated Electronic Health
Records in Dentistry project aims to
lay the groundwork for enhanced
interprofessional collaboration and
integrated healthcare in dentistry.
The organisation has convened an
international group of experts from
dental and other medical fields who
will review the literature and existing
examples of EHRs. This team will
work together with national dental
and health associations to identify
and understand the needs of end
users and consider barriers to and
facilitators for the use of EHRs in daily
practice. Based on this information,
the team will propose a recommendation for relevant and acceptable
oral health indicators to be included
and routinely recorded in EHRs.
The team includes experts from
the fields of preventive and restorative
dentistry, oral medicine, preventive and
general medicine, internal medicine

and health outcomes measurement.
Henry Schein is supporting the
project with its global network, its
broad portfolio of technological
solutions and its expertise in digital
health records.
Improving health outcomes
According to Prof. Mark Wolff, dean
of the School of Dental Medicine at
the University of Pennsylvania, there
has been global recognition that
oral health is intimately involved in
general health and well-being and
that the opposite is also true—general health affects oral health and
quality of life. Prof. Wolff is part of
the project team which began work
earlier this year, taking what FDI
said was a momentous step towards
improving patient care and fostering
collaboration between healthcare
professions.
Prof. Wolff told DTI: “It is very clear
that oral health affects our ability
to concentrate and study at school,
to attend work on time and do a
good job. To separate out whether
a patient is having cardiac, diabetic
or arthritic problems and oral health
problems is foolish. A patient’s medical health, and the medications they
are taking, directly influence how a
dentist can deliver care and how effective or ineffective it is. Oral health
and general health outcomes really
can be negatively affected without
communication between healthcare
providers and social workers.”

One of the strengths of EHRs is
that they can positively influence
patients’ engagement with their treatment, improving treatment acceptance
and healthcare outcomes. Prof. Wolff
explained: “One of the things that I
think we have learned in dentistry
is that we cannot cure dental disease without patient and caregiver
participation, without families and
individuals and an informed public.
When people have access to their
own records, where they can look at
the interactions between the prescriptions and the medications that
they receive, these individuals have
a much better opportunity to improve their health in concert with
their dentist or primary healthcare
provider.”
The time for EHRs
in dentistry is now
David Kochman, chief corporate
affairs officer at Henry Schein, told
DTI that EHRs are overdue. “In the US
every year there are an estimated two
million visits to hospital emergency
departments for dental pain. This
increases costs for all involved and
ultimately leaves a massive gap in the
patient’s care journey,” Kochman
pointed out. “There have been numerous studies linking chronic oral
diseases—such as caries and periodontal disease—to chronic diseases such
as diabetes. The mouth is a part of the
body. We need to treat it as such, and
there is no better time to integrate
dental and medical than now.”

Kochman said that there has
been a substantial push in recent
decades to create the technology
needed for interoperable EHRs that
share patient information between
EHR systems and healthcare providers. He pointed to examples such
as Fast Healthcare Interoperability
Resources, a standard for healthcare
data exchange developed by Health
Level Seven International (HL7), and
said that artificial intelligence and
open application programming interfaces have allowed companies like
Henry Schein to develop innovative
solutions and partner with industry
leaders, bringing interoperability and
integration to the forefront.
It takes time to build consensus
in medical fields. However, Kochman
said that the culmination of several
factors has made Henry Schein confident that the time has arrived to
empower the health professions to
collaborate more closely. He said:
“For some time now, people have
been shouting ‘This is where we have
to go, why are we not moving faster?’
But the bulk of healthcare professions still have not adopted an integrated care model, not even those
who do agree with it. We have
reached the inflection point where
the energy has built up, and the consensus is there. From an academic
standpoint, universities are starting
to drive a holistic care model to the
extent where students who are graduating expect there to be integration
in the way that they practise and the
proper technology to support it.”
Prof. Wolff pointed out that the
use of EHRs in US dental offices
emerged as a means of practice
management, helping clinics to bill
patients and oversee financial aspects,
rather than as a means of benefiting
the health of the patient. He said that
this was no longer the case: “I think
that health records are emerging so
that a patient who has high blood
pressure in the medical record has
high blood pressure in the dental
record. This is where EHRs are starting
to have an elevated impact. Dentistry
emerged with its own record, medicine emerged with its own record,
and it is just at this time that we are
starting to see these records speaking to each other.”
EHRs are already in use in some
countries, and Henry Schein stands
out as an expert in this emerging
field. Henry Schein One’s Dentrix
Enterprise, for example, already interoperates with more than 40 EHRs
though HL7 integrations, offering
single or bidirectional care transfer
of patient information that includes
demographics, clinical notes, medication information and pharmacy

orders. Exan, a Henry Schein One company whose software is integrated
with the US health information exchange Carequality, is currently being
used by the University of Pennsylvania
School of Dental Medicine.
A project on a global scale
The task ahead is significant.
Prof. Wolff explained: “You have to
recognise that we are doing this on
a global scale and that we need to
understand all of the different records
that are out there. We are already getting a better understanding of what is
happening globally, and I think that
there are a series of health items—
things like dental caries and periodontal disease with inflammation that
could influence heart health or diabetes—that should be shared between
the health professions. And then we
also need to talk about social determinants of health, such as financial,
psychological and family issues. These
determinants cross the professions
and need to be shared.”
Kochman said the work being
done will change dental care: “It
won’t happen overnight, but the
generation of practitioners graduating today (and in the future) are going
to have a more holistic approach.
They will also call on the industry to
provide scalable, innovative, interoperable technology to play a meaningful role in helping them put
the patient first. Post-pandemic, consumers are now expecting the same
level of access, personalisation and
fluidity in healthcare as they experience in other areas of their lives.
There’s no doubt interoperability
and integration will help shape the
future of care, bringing the healthcare industry up to speed with consumerism and providing comprehensive care that closes the loop in
the patient journey.”
Emphasising the significance of
the project at the time of the launch,
FDI President Prof. Ihsane Ben Yahya
commented: “If harnessed effectively
across all healthcare sectors, EHRs
hold the potential to foster interprofessional collaboration and elevate
patient care to new heights. We are
thrilled to embark on this journey to
comprehend the needs and preferences of dentists and dental teams concerning EHRs to enhance quality of
care and fuel impactful research.”
FDI expects the results of the literature review and end user research
by early 2024, and the project’s international expert advisory group is expected to convene throughout 2024
for the consensus process. FDI and
Henry Schein will keep the international
dental community updated as the
project progresses.

BISCO’s tried and true calcium-releasing family
| Remember the adage “A penny saved
is a penny earned”? How about “Little
strokes fell great oaks”? It is time to add
another adage to the list: “If it works,
keep using it”. Whereas this saying has
countless uses in everyday life, it is
particularly applicable to work in the
dental community. Here, when something is efficient and reliable, we turn to
it time and time again. That is why dentists across the country continue to rely
on the BISCO line of therapeutic, calcium-­
releasing materials for their restorative
needs.
The newest addition to this family
of products, launched last year, is
­TheraBase—a dual-cure, calcium- and
fluoride-releasing, self-adhesive base/
liner designed to be stronger and more
durable than other base materials.
Here is how it works: when water is
introduced, the non-dissolving matrix
releases calcium hydroxide and fluoride ions. Then, within minutes, the ongoing ion exchange that results raises
pH levels,* and the ensuing alkalinity
promotes pulp vitality.1 TheraBase is
dual-curing, which means that, even
though it can be light cured, the ma­
terial can fully polymerise chemically
throughout, and this includes the area
in deep restorations where light cannot
reach. The calcium-releasing base and
liner features BISCO’s proprietary hydrophilic matrix, which aids in protecting tooth structure. In addition, the
material is self-adhesive and does not
require bonding agents, thus saving
time and costs.

A much touted benefit is that
­TheraBase is highly radiopaque, paving
the way for improved radiographic
identification and more accurate dental
diagnoses. The product also has high
flexural strength, absorbing stress and
shock from occlusal forces.
Well suited for pulpitis risk
“I’ve been using TheraBase when
I am very near to the pulp and where
there’s a risk of pulpitis,” Dr Aliisha Choucair,
a dentist at Laurel Dental in Leander in
Texas, explained. “It’s very easy to place
and has good retention. I was able to
restore a case that had reversible pul­
pitis—the tooth remained vital, the
nerve settled down, and the restoration
appears to be well sealed,” she con­
tinued. “I like the science behind it—
releasing calcium and fluoride—and it
has good handling properties.”

Meet the rest of the family
TheraBase joins three other Thera
products, which all use the THERA technology in their formulas. TheraCal PT,
for example, is a biocompatible, dual-­
cured, resin-modified calcium silicate
that is used for pulpotomy treatment.
Dr David Martin, a dentist in
Totowa in New Jersey, said that
­TheraCal PT is particularly useful in
cases where patients cannot tolerate
long chair times. The material has a
working time of 45 seconds and a
setting time of 5 minutes. “I recently
had a young child with a large carious exposure,” he explained. “I was
able to treat the lesion in a much

that contains MDP, an adhesion-­
promoting monomer that enhances
bond strength to zirconia, metal and
tooth structure without the need for
additional adhesive or primer.

| The single or combined action of
agents such as bacterial plaque, food,
acidic substances, chewing and in­
appropriate oral hygiene practices, if
sufficiently prolonged, may result in
a loss of mineral substances from the
surface of the tooth, leading to weakened dental structure and reduced
masticatory capacity. A 2010 review
on the remineralisation of tooth
enamel published in the Journal of
Dental Research states that the goal
of modern dentistry is “to manage
non-cavitated caries lesions non-­
invasively through remineralisation
in an attempt to prevent disease

­ rogression and improve aesthetics,

strength and function”.1
Remineralisation is a natural
process in the oral environment.
However, it is limited owing to various circumstances. Nowadays, there
are many products on the market
that claim to enable remineralisation.
Remineralisation means that the
constituent minerals of the teeth,
mainly calcium and phosphate ions,
can be sourced to promote ion depo­
sition into the crystal voids of the
demineralised enamel. However, the
substances that claim to have this

and mouthwashes for children and
adults contain a mixture of two dif­
ferent hydroxyapatites that allow the
biomimetic properties necessary for
effective remineralisation to develop:
a hydroxyapatite substituted with
magnesium, strontium and carbonate conjugated with chitosan—
called Bioactive Complex—and a
fluoro-hydroxyapatite—called H.A.F.
Natural hydroxy­apatite is a highly stable
mineral that is unable to release active
ions even when in contact with damaged
tooth areas. Products that contain only
this type of hydroxyapatite are not
particularly effective in remineralising, although they can still claim to
have desensitising properties. Natural hydroxyapatite, without added
minerals, does not have any particular capacity to remineralise. The substitution with magnesium, strontium
and carbonate, however, makes the

to true remineralisation, thus exploiting the normal biological processes.
In addition, fluoride helps to create
a mineralised surface that is more
resistant to acid attacks and more
able to better protect the teeth
from erosion, abrasion and brushing
damages. The formulation has been
further enriched with strontium salts
to help rapidly reduce pain in cases of
dentinal sensitivity.
In patients particularly predisposed
to strong demineralisation, such as
those suffering from Sjögren’s syndrome, diabetes or dry mouth or undergoing chemo- or radiotherapy, and
wearers of orthodontic appliances,
patients with white spots or those
who have frequent caries, the additional
use of Curasept Biosmalto Mousse is
recommended. The foam is based on
amorphous calcium phosphate enriched with carbonate and fluoride and
is externally citrate functionalised. In
contact with saliva, the ACP-F complex

dissolves immediately, rapidly releasing
the active substances. In this way, a
large amount of calcium, phosphate
and fluoride is released directly on to
the dental surface and in proximity to
the demineralised lesions, while citrate
facilitates the penetration of the ions
inside the tooth. The result is a rapid
and intense remineralising action.
Curasept Biosmalto Mousse is
also recommended for children. To
guarantee an effective remineralisation process, the Curasept Biosmalto
Mousse should be used together
with products from the Curasept
Biosmalto toothpaste line, selected
according to the patient’s age.

Embracing the next century of growth

| Shofu was established in Kyoto in

Patients prefer Guided Biofilm Therapy
| The basis of good oral and
general health is a clean mouth.
Well-informed patients know this
and visit the dentist for professional prophylaxis regularly.
An ongoing global survey by
EMS on patients’ preferences
regarding the method used
showed that around 92% of the
76,000 patients who completed
the survey between May 2021
and March 2022 favoured
Guided Biofilm Therapy (GBT).
To date, the survey findings have
been confirmed by more than
250,000 participants.
The World Health Organization
has identified oral disease as a
key health problem.1, 2 Patients can
avoid developing diseases such
as caries and periodontitis by
adopting a healthy lifestyle and
implementing an effective personal oral hygiene routine to
remove plaque. 3, 4 Undergoing
professional prophylaxis is also
of great importance. Methods,
instruments and materials used
for this purpose should therefore
be state of the art.5

Method comparison
EMS’s GBT is a systematic, comprehensively documented clinical
protocol developed in collaboration with researchers and practitioners. Its aim is to clean all oral
surfaces as thoroughly as possible
using a minimally invasive method.6
In GBT, after disclosing of plaque,
the biofilm is gently removed with
the AIRFLOW handpiece.7 Only then
is the painless piezoceramic ultrasonic instrument PIEZON PS NO PAIN
used for removal of calculus. In
contrast, conventional professional
prophylaxis procedures focus on
removing calculus with invasive
sonic or hand instruments. The
subsequent polishing unnecessarily
removes enamel.8 In addition, in
order to save time, disclosing of
plaque is often neglected. As a
result, significantly more biofilm
remains compared with GBT, for
which a standardised disclosing
agent is used.9, 10

overall assessment of GBT, surface
smoothness achieved, usefulness of
disclosing plaque prior to treatment
and direct comparison with the
conventional prophylaxis method,
among others.

Discussion and conclusion
The survey finding that around
92% of patients preferred the
GBT systematic prophylaxis protocol
compared with the conventional
approach is consistent with that
of previously published studies.5, 11
Based on the risks and needs of the
individual patient, GBT is suitable
for both primary prophylaxis and
secondary prophylaxis for periodontitis and peri-implantitis therapy.

benefit, in combination with the
comprehensive scientific
documentation supporting
GBT, it is recommended

Queen Victoria Building is a fivelevel shopping centre located in the
heart of Sydney’s central business district.
Originally built as a market hall between
1893 and 1898, the building now boasts
over 200 high-end shops, Australian
designer boutiques, gift stores and
galleries. Besides serving as a hub for
shopping, eating and entertainment,
the building is celebrated for its ornate
late nineteenth century Romanesque
architecture and features impressive stainedglass windows and mosaic floors that
exude timeless charm.

Darling Harbour consists of a
large recreational and pedestrian
precinct and offers many tourist attractions. Here, people can enjoy
