As the dental profession has
come to realize that undiagnosed
and untreated injuries caused by
ergonomically unsound practices are
often the cause of physical pain and
early retirement, there have been
numerous strategies employed to prevent such injuries.
Today’s dental professionals are
trained to provide dental therapy
in a seated position, which varies
from earlier training that allowed for
standing only. The incidence of musculoskeletal disorders (MSDs) among
dental professionals has increased
noticeably in recent years.
Because dental treatments often
involve working in awkward positions and holding those positions
for extended periods of time, many
MSDs, especially those involving the
neck and shoulders, result from what
is known as Prolonged Static Posture
(PSP). In addition, the application of
a high percentage of Maximum Voluntary Contraction (MVC) of muscles
required for many procedures also
contributes greatly to the high incidence of MSDs.
To address these problems, such
things as improved lighting, magnification and chairs that support proper
posture have been introduced. Now,
another technology, free motion
elbow supports, has emerged to add to
the resources available to protect us.
Free-motion
elbow

encourage proper working posture
and enable you to work from an
ergonomically optimal position. They
are a key component in the reduction of work-related musculoskeletal
disorders of the back, shoulders and
neck among dental professionals.

syndrome, which can be a serious

Look to the science

A comprehensive study published by
ASSTSAS1 used electromyography
(EMG) to measure the actual amount
of force exerted during the performance of key dental activities.
The study measured hygienists’
levels of stress when performing

under their normal working conditions and using equipment, postures
and techniques they had become
accustomed to.
The study found that even when
ideal posture was maintained during
work, significant muscle stress was
observed without the use of freemotion elbow supports. It then followed up those results by testing the
same group of hygienists after they
had been retrained and provided with
seating that made use of gel-padded
free-motion elbow supports to reduce
specific types of stress.
The results of this study demonstrate that a combination of changes in
work habits and posture, coupled with
the use of seating equipped with freemotion elbow supports, significantly
reduces the stress of PSPs and the percentage of MVC of muscles required to
perform dental work.
Because the study used EMG
devices attached to its subjects to
directly measure the stress exerted on
specific muscles during the performance of dental tasks, it provides direct
evidence based on physiological measurements rather than questionnaires
and observation alone in support of
its conclusions about the best ways to
dramatically reduce MSDs and their
associated negative consequences.

n Periodontal tissue regeneration
has been demonstrated to be promoted either when the Nd:YAG laser
or Er:YAG laser is being used individually as an adjunct to mechanical
therapy. The same or presumably
even better result is expected when
both laser therapies are performed
in combination using the TwinLight®
protocol. My clinical experience confirms this conclusion: Bone regeneration is observed as one of the positive
outcomes of the LightWalker ® laser
TwinLight treatment.
In this paper, I report on one of my
more difficult cases, in which the periodontal TwinLight laser treatment
was additionally combined in tandem
with the endodontic PIPS® laser treatment, both exclusively available with
the LightWalker dental laser.
Radiographs taken before and
three-years post-op demonstrate significant bone regeneration. My clinical experience confirms the conclusion of a recently published five-year
retrospective study that concluded
that bone regeneration is observed
as one of the positive outcomes of the
TwinLight treatment.

Case

A 72-year-old male in otherwise good
health presented with an endo/perio
lesion of the mandibular left second
molar (Fig. 1).
The tooth was the distal abutment
of a four-unit fixed partial denture.
The patient’s chief complaint was
soreness to chewing pressure and a
chronic dull ache on the left side of his
jaw. He had recently seen his family
dentist and an endodontist, both of
whom had advised him the tooth was

hopeless. A single appointment of
1.5 hours included PIPS laser endo,
TwinLight periodontal treatment and
bridge repair.

Conclusion

It is the author’s contention that the
success of this case can be attributed largely to the gentle nature of
the TwinLight and PIPS treatments
available in tandem with the Fotona
LightWalker laser.
Traditional root-canal treatments
and periodontal surgery both create
a great deal of mechanical stress
on the periodontium as endodontic
instruments engage dentin within

the canals, and scalers and ultrasonic
instruments debride and root plane
the diseased cementum.
Laser energy does not create mechanical stress, and its bactericidal
nature as it works within the canals
and on the perio defect is an additional
benefit. The radiographs taken before
and three-years post-op demonstrate
significant bone regeneration following the combined TwinLight laser
periodontal treatment.
