By Lyne Noiseux President, Posiflex Design n Musculoskeletal disorders (MSDs) represent an assembly of symptoms found in the workplace. They range from locomotive discomforts and pain related to certain work activities up to annoying pains that interfere with all activities, all the time. Rose-Ange Proteau, an ergonomist with the Association for Health and Safety in the Work Place for Social Affairs, said at a recent conference, “Often, the person is not conscious of the effect of wear. As we worked to the amplitude limit of the articulations on a regular [basis], we are much more at risk.” She used the example of a young dentist who after only three years in practice declared, “Me, I have no problem, I go to the chiropractor every week!” Proteau completed her master’s degree in ergonomics on this subject and has created the guide, “Prevention of Work-Related Musculoskeletal Disorders (MSDs) in Dental Clinics.” She said that scientific studies have demonstrated that even with a good posture, with arms held close to the body, muscular tension remains high because of the strength and the control required for precision work. (Photos/ Provided by The Canary System) By Stephen Abrams, DDS n Caries detection around the margins of restorations is a major challenge in clinical practice. Typically, the restorative material will impede the detection system from accurately examining the margins for caries. However, The Canary System, powered by an energy conversion technology (PTRLUM), provides the solution. The Canary System™ directly examines the status of the tooth crystal structure. Rapid, safe pulses of laser light combining infrared and luminescence allow users to examine lesions as small as 50 microns and subsurface caries up to 5 mm below the tooth surface or along the margin of composites and amalgams. Detection of caries around the margins of a Class V amalgam restoration can be a challenge. Figures 2 and 3 illustrate a patient who was at very low risk for developing caries. The existing restoration was placed more than 30 years ago; the margins were intact; oral hygiene was excellent; and no restorations had been placed or replaced in the last 15 years. Radiographs could not image the restoration margins, and the amalgam restoration on the buccal or occlusal part of the tooth may have created a silver stain or halo around the margin. The Canary scan number, 76, resulting from scanning around the margin, indicated advanced caries around the restoration. Upon removal of the amalgam, a large carious lesion was found extending well beyond the margins of the restoration. Therefore, the subsurface caries that would have otherwise gone undetected, possibly requiring a root canal, was successfully detected by The Canary System by examining for decay around the margins of this restoration. Research shows that PTR-LUM technology used in The Canary System can detect: • Occlusal pit and fissure caries1-3 • Smooth surface caries4,5 • Acid erosion lesions6,7 • Root caries8,9 • Interproximal caries lesions10,11 • Demineralization and remineralization of early caries lesions12-15 The Canary System captures both signals and images of the tooth surfaces being examined. These images are displayed on an accompanying interactive, touch-screen monitor for immediate chairside review with the patient who can also take home a printed Canary Report. The Canary System creates an opportunity for dialogue and codiagnosis, two strong motivators for a new long-term, recurring patient relationship. Odontograms also are added to patients’ files so treatment progress can be monitored over time. The data is saved on the Canary computer and merged into detailed user-friendly reports. Kurtzman, DDS, MAGD, FACD, FPFA, FADI, DICOI, DADIA n The Miratray Implant Advanced Tray simplifies the process of taking open tray implant impressions. The tray is provided in three maxillary and three mandibular sized trays, and the trays are unique in their design. The occlusal surface is covered by a transparent foil. This allows easy identification of the heads of the pins intraorally. Retention slots and an internal rim provide mechanical retention to keep impression material in the tray. Should the practitioner choose to supplement the retention with a PVS adhesive, it is recommended that it not be applied to the foil surface because this may obscure visualization of the pins when inserting the tray to proper depth. The technique involves filling the tray with an appropriate impression material. The tray is then inserted over the open tray impression heads intraorally and pressed down crestally until the top of the impression pins are visible through the transparent foil. The practitioner then presses the tray further until the pins puncture the foil and are visible protruding through the foil. This contains the impression material within the tray, eliminating the potential problem often seen with use of custom or modified stock trays of the impression material obscuring the tops of the pins. Upon setting, the pins are rotated in a counterclockwise fashion and removed from the impression, and the impression is removed intraorally. Because of the design of the tray, it can be used in all implant impression situations, whether the arch is partially dentate or fully edentulous. About the author Gregori M.
