Most of the dental implants in the market nowadays are made of a single material, although these two kinds of issues require different materials. Therefore, the TBR Group decided to rethink the definition of a dental implant and to develop a hybrid system to optimise both the integration in bone tissue and gingiva. At a recent press conference in Cologne, they also rejected fears of economic cutbacks due to the ongoing financial crisis. According to Dr Martin Rickert, Chairman of the Association of German Dental Manufacturers (VDDI e.V.), there is still a steady demand for medical and dental services. Today Editor-in-Chief Daniel Zimmermann spoke with Dr Rickert about the current state of the industry and his expectations for IDS Cologne 2009. Daniel Zimmermann: Dr Rickert, the economic valuation of the German dental market in 2008 was fairly positive. Has this changed with the current financial crisis? Dr Rickert: Not really. As you correctly point out, it is predominantly a financial crisis. The industry’s outlook for 2008 was good and currently there is a trend towards a stable return which does not fulfil all our expectations but still promises to deliver satisfactory profits. There are certainly fluctuations in various market segments and world markets that stem from different influences and are not attributable to a single cause. There have been initial reports on redundancies in the United States. Should we be preparing for cutbacks or restructuring measures in Germany as well? The US health sector differs from that in Germany and in most European countries and, therefore, many private investors have been affected by the financial crisis. Mass redundancies in key sectors mean that many people can no longer contribute towards company health insurance schemes in the US. This will AD definitely not be the case in Germany because the statutory health insurance provider and private health insurance companies are prepared to cover the healthcare costs for their clients in the future. Even if we are in for job losses in various economic sectors, there are not likely to be any negative effects on the German healthcare market. What positive effects will IDS have on the industry? The 33rd IDS will again exhibit a comprehensive range of modern dental products and technologies. Visitors will be able to see and experience an abundance of new product and service innovations. Technological developments in the medical field and our industry’s investment in research and development continually produce new and improved system solutions for dental practices and laboratories, which are of great financial interest to dentist and technician alike. IDS has always been a driving force behind the dental market and will remain in this role in the future. Moreover, patients’ demands for health-related services are mostly governed by acute health problems and the desire for a return to health and recuperating quality of life. Dental implants and automated fabrication of dental restorations are currently the fastest growing sectors in dentistry. Is this boom reflected in the products and services that are going to be presented at IDS? Implant-borne restorations are in fact one of the fastest growing sectors, with more than 700,000 dental implants placed in Germany last year and an annual growth-rate of around 10 to 15 per cent. The results of our latest member questionnaire underline the fact that export figures in East European markets, especially Russia, and Asian markets have shown a positive trend. Incidentally, the growth in dental implants in Germany can largely be attributed to the fact that the statutory health insurance scheme has subsidised this treatment since the introduction of a fixed coverage system in 2005. IDS will exhibit the entire range of modern implantology systems, presenting both innovations and developments in tried-and-tested systems. What other sectors do you view as having potential for growth? Apart from implantology, I see the most potential in the all-porcelain sector, in which around 25 million restorations worldwide were fabricated using CAD/CAM technology. Modern laser scanners are able to read more than 100,000 reflecting points a second, and image-capturing has become quicker and more precise. Modern high-performance materials provide the best possible stability for a wide range of indications and are able to guarantee biocompatibility at the same time. The latest digital fabrication technology using laser metal sintering of, mostly precious metal-free, alloy powders can now also be considered for prosthetic restorations. These dentures stand out because of their incredibly precise fit and durability. German dental industry export activities again increased by approximately two per cent in 2007, which comprises 55 per cent of the total turnover. Is the domestic market losing its value? The shift of sales is a result of an expansion of our market position overseas disproportionate to growth in the domestic market. It is important to guarantee dentists and dental technicians in the German market acceptable remuneration for their services. This is the only way to ensure that young people find their way into the health service, to prevent professional migration, and to ensure patients gain access to the latest developments in general dentistry and dental technology. German patients have to carry more medical expenses due to the introduction of the national health fund. Is this having an effect on dentistry as well? I expect not only an added burden for the insured, but also an increase in bureaucracy that we will all have to tolerate. Calculations by a council of experts for the national health fund predict a shortfall of €440 million in 2009 alone! I think notable negative effects are to be expected due to the revamped dental practitioners’ fee schedule, which is the new basis for private dental billing in 2009. In their calculations, the Association of German Dentists has concluded that large cutbacks in salaries could be the outcome of these regulations. If one considers that a significant portion of the annual turnover in many practices already comes from patients who pay out of their own pockets and are, therefore, essential for practices’ survival, one cannot understand why the dental profession should be content with a slight increase in re- growth development in this market is still at a very low level. A slight weakening in the outlook for the North American market has been reported. Regardless of this, the number of members who predict an increase in business prospects for the first half of 2009 is higher than those who predict a decline in business prospects. On average, Western Europe presents itself as a very stable market environment and remains one of the most important sales regions for our members in terms of turnover. Onefifth of our members expect an improvement in their profits within the first half of 2009 compared with the same period last year. An increasing number of companies from emerging markets, such as India and China, are beginning to gain market shares. How do you size up the competition? It is important to remember that IDS is also a global marketplace. These countries do not necessarily come to Germany to attend IDS, in order to compete in these highly developed markets. For most companies, markets in developing countries are of primary importance, as the world’s largest dental marketplace is a meeting point for suppliers from developing countries and their consumers from other developing countries. However, this doesn’t mean they don’t have anything to offer European consumers. The German dental industry has enjoyed an international reputation as one of the most innovative dental industries. Our exceptionally high standard of quality, service-friendly prod- ucts, and high level of innovation, as well as distinct customer orientation with a high quality of service, make our products globally competitive. We confidently meet the challenge of all competition in any market worldwide. During the last two years you have travelled abroad to promote IDS in other regions. Can you tell us more about perceptions of the show in different parts of the world? IDS is THE international meeting for the dental sector. For years, it has been an established communication and marketing platform for the international dental community. This recognition has not changed very much, as was evident during our several international press conferences last year held in collaboration with Koelnmesse subsidiaries in Japan, China, Russia, and the United States. Participants consistently displayed much interest in and support for IDS. We were also given credit for the high quality of accommodation, careful organisation, and the show’s international character. Only at IDS is such a comprehensive display of the international dentistry market presented—the complete spectrum of dentistry and dental technology is available to be seen and experienced. All products and services that make the work of dentists easier, help them work economically, and provide their patients with the state-of-the-art in dental technology are available at the show. Unlike other international meetings, IDS seems to grow in popularity each year. In your opinion, what are the main factors contributing towards this growth? IDS has an appealing and powerful concept that was developed in cooperation with our partner Koelnmesse. In particular, the sheer amount of state-of-the-art innovations makes IDS the most comprehensive dental exhibition and one of the liveliest and most significant marketplaces in the dental world. It is important that we plan this growth carefully to keep up with market demands. We do not want the show to expand too rapidly and possibly outgrow its market. We already have a high level of international participation at IDS from both exhibitors and visitors. Demand for dental products and services remain healthy due to growing incidence of cosmetic treatment, new products that reduce patient discomfort, increased health consciousness and growing number of dental implants. However, growth in the conventional supplies category may be tempered due to emerging procedures and technologies that require fewer supplies and materials. According to the report, dental care markets vary widely across different countries. Rapid growth in living standards has induced greater realisation about the quality of medical and dental services in countries such as India and China. Consequently, these countries are exhibiting soaring demand for modern and sophisticated technology and equipment in the dental market. United States, Japan, and Europe collectively dominate the worldwide dental supplies market, with over 80 per cent share of sales in 2008. Most implants found to be successful Dental implants are 98 per cent successful and cause little or no bone loss, according to new research published in the Journal of Oral Implantology. Authors Zeev Ormianer, DMD, and Ady Palty, DMD, reviewed 60 charts of patients who received a total of 267 implants in two private dental practices in Israel and Germany. They found that 98.5 per cent of the implants survived and there was no discernable bone loss in 88 per cent of the implant sites. The mean follow-up time was 7.5 years. The study goal was to determine the level of bone loss over time at the implant sites in the jaw. A key clinical issue was not whether bone loss would occur, but how AD much bone loss should be considered normal and acceptable. Commenting on the findings, Jamie Lozada, DDS, president of the American Academy of Implant Dentistry (AAID), said the study adds to a growing body of compelling clinical evidence supporting dental implants as the most successful method for replacing missing or compromised teeth. “With an estimated two of three Americans having at least one missing tooth, implants are becoming the preferred toothreplacement option. Implant surgery is one of the safest, most precise and predictable procedures in dentistry,” Lozada said. Show Preview IDS Cologne 2009 Men drive cosmetic trends According to the British Academy of Cosmetic Dentistry (BACD), more and more men are opting for cosmetic dental treatment. Men, who used to account for 28 per cent of all veneers, now account for nearly a third of all procedures. Similarly, men used to account for less than a fifth of all orthodontics. They now represent almost a quarter, with 400 cases this year. Bridges are another procedure that has become more popular with men, who used to account for 42 per cent and now are edging ever closer to women at 46 per cent. Overall, orthodontics (which includes both visible ‘train track’ as well as invisible and removable braces) has boomed in popularity, with the number of cases more than tripling since 2006. Women are still the big spenders on cosmetic dentistry, accounting for 61 per cent of all procedures. Women also still account for the majority of whitening procedures. According to Dr James Goolnik, dentist and BACD board member, “These results show that men have become more accepting of cosmetic treatments in general—reflecting the importance we now place on a healthy smile. The advent of new procedures, such as the removable adult braces and more realisticlooking veneers, which require much less drilling, also means people are less likely to choose invasive and irreversible treatments.” Dr David Bloom, dentist and president of the BACD, added, “This audit has highlighted some very exciting trends, such as an impressive increase in the number of orthodontic cases. It’s also (DTI/CURAphotography) interesting to note the overwhelming preference for less invasive treatments, such as onlays, which are porcelain fillings to cover part of the tooth, over crowns, which involve drilling to achieve full coverage.” Economic fears in the US affect dental care With the economy in the United States declining, preventive dental care can be one of the first things to go. The correlation between rising unemployment and a drop in preventive dental care, however, is not necessarily due to people being short of cash, according to a study in the online edition of Health Services Research. The researchers analysed 10 years of information about visits to dentists’ offices in metropolitan Seattle and Spokane from Washington Dental Ser- vices, USA, the largest dental insurer in the state, which covers roughly one-third of its residents. They compared this information to unemployment data from the Bureau of Labor Statistics and Washington’s Employment Security Department, and ruled out other possible explanations for a correlation. In the Seattle area, for every 10,000 people who lost their jobs, there was a 1.2 per cent decrease in visits to dentists for checkups. The drop was higher in the Spokane area, where the same increase in unemployment was associated with a 5.95 per cent decrease in preventive visits. This is notable as the study looked at people who had dental insurance that covered routine care. “We see that high communitylevel unemployment exacts a psychological toll on individuals,” said lead study author Brian Quinn. “Even for people who are working, or who have a working partner or spouse, there might be an impact if they’re stressed about themselves or their significant others losing their jobs.” Quinn, a program officer for the Robert Wood Johnson Foundation, said the distraction of worrying about not having a job could make dental care drop off a person’s radar. “During stressful periods, those things that don’t seem as urgent may be ignored,” he said. Ma Yun Xiu: Most Chinese dental labs I had the chance to visit have a serious consciousness for service and product quality. But contaminated bridges and crowns from China have recently appeared in the United States. Are these individual cases? I think they are. By now a majority of dental labs in China are using raw materials as well as equipment from high-level brands in the world, such as Ivoclar Vivadent, Dentsply or Shofu. Graeme Milicich Lasers have been used in different medical fields for many years and have revolutionised many treatments, notably eye surgery and hair removal. The technology is also an established aspect of modern dentistry and is widely used in Europe and the USA. DTI editor Anke Schiemann had a chance to speak to Graeme Milicich, who is a fellow, diplomate, and founding board member of the World Congress of Minimally Invasive Dentistry (WCMID), prior to the recent FDI Congress in Stockholm in Sweden. Anke Schiemann: In a nutshell, what are the benefits of using laser in clinical dentistry today? Graeme Milicich: Lasers have many applications in clinical dentistry. My research in the last four years focused on the clinical applications of hard tissue Erbium lasers. The broad range of laser applications has benefits for both the patient and the dentist. Many hard and What is the advantage of lasers over rotary cutting instruments, and are there limits to what a laser can do? Yes, there are some limitations as to what can be done with a laser, like the removal of metal restorations and crowns. But if you consider the totality of the types of treatments offered by general dentists, these limitations are far less compared to conventional rotary instrumentation. For example, you could run into problems with soft tissue contours or bone levels associated with a deep cavity while cutting tooth structure. With a laser you can remove both bone and soft tissue by simply changing a setting, and are therefore able to complete the procedure in one appointment—something that cannot really be done with a highspeed drill. Generally speaking, what can be done with a drill, scalpel, or bone bur can also be done with a laser. Additionally, many patients have a fear of dentistry based on the sounds and vibrations associated with rotary instruments. These sensations do not occur with the laser, meaning the patients find treatment much more acceptable. With the advent of the KaVo DIAGNOdent more than ten years ago, the first general dentistry application of laser fluorescence was introduced. Like with any new technology, it had to be understood first, in order to achieve the best results. In order to provide patients with accurate treatment recommendations based on the results of early cavity detection, an understanding of minimally invasive concepts is essential. Otherwise, the profession can be open to claims of over treatment. These charges often derive from a lack of understanding of the technology, its accurate application, and the concepts and applications of minimally invasive techniques. Often, astute clinicians are at the forefront of the application of new technology and techniques, and the research literature struggles to keep up with the clinical pioneers. This leads to a period with a shortage of validation for what eventually becomes a new and accepted standard of care. Further developments in the field are occurring and, as they filter into general practice, the standards of diagnosis will continue to improve. For new diagnostic technology to be readily implemented in general practice, it has to be both cost effective and time efficient. 5:39:20 PM What are the chances of treating peri-implantitis with lasers? There are many case studies showing excellent results when treating peri-implantitis with a laser. The laser’s use for debridement and disinfection gives the competent clinician a tool that previously wasn’t available. The laser is safe to use around implants with little risk of damage to the implant. Personally, I have only treated one case of periimplantitis so far, and it was a complete success. The use of laser in fields like endodontics or periodontology is highly controversial. What are the main issues here? Once again, competent laser clinicians are ahead of the research in these fields. Clinical results are being achieved that are now only beginning to be validated by research, and until the research results are available, use of lasers in these fields is going to remain controversial for many. Those that are using lasers and are observing the clinical outcomes, have little doubt as to the efficacy of their treatments. Personally, I have been involved in research using the Waterlase (Er,Cr:YSGG) in endodontics. The ability for complete debridement of the canals following conventional canal preparation using radial firing tips in a non-ablative mode is significant, and ad- dresses the issues of air and fluid entrapment at the apex that are associated with conventional techniques used for final canal debridement and rinsing. In your FDI lecture you talked about new concepts associated with laser therapy. Can you give our readers a brief overview and explain these concepts? it requires a mental awareness to slow the motion of the tip, to allow ablation to occur. As competence increases, this phenomenon is used to control ablation rates, without having to alter laser settings, by increasing or decreasing the motion of the tip. The third concept is focusing and defocusing the beam, to alter ablation rates without (DTI/Creation/Shafeev) The most common complaint from a new user is that it will not cut fast enough. The most significant contributor to slow ablation rates is the user, not the technology. The single biggest hurdle a dentist faces when beginning with laser is the difference between rotary instrumentation and lasers. When this is understood, a new laser dentist can become competent in a very short time. If these concepts are not well taught, then the new user will become frustrated and may fail to integrate their new laser into their treatment regimes. The first concept is that lasers are end cutting. We have all become very competent using rotary instrumentation and have developed reflex motions as a result. The natural tendency is to apply these ‘reflexes’ when using a laser and this leads to frustration for the new user. When using a high-speed bur, we tend to move the bur laterally to extend a cut. This does not work with a laser because it is end cutting, not side cutting. Therefore, the operator needs to learn a new way of pointing the laser directly in the direction where a cut needs to be extended. Anyone who has become competent in the use of air abrasion masters the use of a laser very quickly because the same concepts apply to both technologies. The second concept is that slow is fast. Once again, this concept is associated with our reflex motions associated with using high-speed hand pieces. We tend to use a fast painting motion on the surface when contouring a cavity. Exactly the opposite applies when using a laser. Ablation rates are stalled by this rapid painting motion, and initially having to change power settings on the laser. This technique, in combination with slowing or speeding up the motion of the tip, allows the operator to finesse ablation rates to create very smooth contours. The final concept is the clinically observable ablation threshold. Many new users focus on power settings and how far the tip should be from the surface, depending on what they want to do. Absolute distances in relation to operating parameters are impossible to give because there are so many variables involved, including the tip being used, the state of the tip, the air/water ratios, and the surface being ablated. As a tip is moved towards the tooth, it reaches a point where the operator can begin to see the commencement of ablation. This then gives a reference maximum operating distance in relation to the current settings and tip being used. New users are taught to start out of focus and move towards the tooth until the clinically observable ablation threshold is reached. This distance can range over several millimetres, depending on the various parameters. Understanding the concept helps new users avoid inadvertent high fluence effects at the ablation surface. There are two other issues that will be dealt with as separate topics in the lecture in regard to ablation rates in enamel. This is the area that new users find most frustrating, because they tend to use rotary cutting movements with an end-cutting device. Firstly, because laser ablation is a non-contact technique, magnification is essential. Secondly, enamel ablation rates are related to the orientation of the long axis of the enamel prisms in relation to the plane of the ablation face. Ablation rates are 40 per cent greater when enamel prisms are ablated from their sides, rather then on their ends. This requires an understanding of the orientation of the long axis of enamel prisms in different surfaces of a tooth. The culmination of this understanding is epitomised in the time it takes a new user or a competent laser clinician to cut a slot preparation, with a new user often taking more than three times as long to complete the same procedure. Recent research on shorter wavelength lasers or the so-called blue lasers has shown them to be effective in diagnosing cancer cells. With this in mind, how do you see the use of the laser in general dental practice developing in the years to come? Many dentists focused on minimal intervention have embraced laser fluorescence in the diagnosis of demineralisation of tooth structure. With these new applications, such cancer screening becomes cost effective and will become a common part of general dentistry. The main issue is that laser effects are frequency specific, and as applications develop, this will lead to a plethora of technology that becomes difficult for the clinician to incorporate into a practice. I envisage, in the near future, a diode-based laser that will have multiple, switchable frequencies that will allow one unit to accomplish varied tasks that require different frequencies. As uptake of laser technology increases, costs will decrease, making it more attractive to more of the profession. Do you expect lasers to be an essential part in every dental practice in 10 to 15 years? The multiple applications of lasers are only going to expand in the future. At the moment, the lasers with the most clinical applications in one unit are the Erbium family, and many dentists have embraced this technology and are constantly expanding its clinical applications. If we look back over 60 years to the initial introduction of the high-speed hand piece, there was initial and significant resistance to the technology, and it took over 10 years before it was readily accepted into general practice. Lasers have had a slower journey, mainly because of the need for advanced technology to make them more applicable in the field of dentistry and the associated research and development costs that are reflected in the price of lasers. Taking the cost of a laser out of the equation, it is very easy to visualise a laser in every practice in the near future. Many of the innovations created by endodontics experts in the last few years have been further developed by businesses in the dental industry, resulting in mature instrument and tool systems. Now lesions of the root canal are diagnosed and, above all, treated better. For example, modern ultrasound imaging procedures are used to locate the root canal precisely and measure its length right to the apex, as well as to provide exact control of the file position during preparation. High-resolution intraoral cameras that are connected to monitors and which enable documentation and diagnosis of endodontic treatments using views of the root canal are also available. “The fascinating scientific and technological advances in the area of endodontics have greatly improved the chances of long-term tooth preservation and also make this specialist area one of the significant components of prophylaxis-oriented dentistry,” said Dr Martin Rickert, Chairman of the VDDI (Association of German Dental Manufacturers). Tooth preservation using endodontic measures has become which increasingly use electronically controlled torque limiting or variable oscillations. This means the files remain intact over their lifetime. (DTI/Nikola Spasenoski) increasingly important in modern dentistry with treatments that have been carried out in compliance with regulations achieving success rates—based on evidence—of between 68 and 85 per cent. The spectrum of methodologies in conservative treatment ranges from manual or mechanical root canal preparation, disinfection and obturation, to the revision of the root canal should it become necessary and minimally invasive, microsurgical concepts for the treatment of particular endoperiodontic lesions. Dentists have a number of different options available for root canal preparation: classic manual file systems or rotating instruments, particularly those made of break-resistant nickeltitanium alloys, as well as timesaving mechanical preparation. Preparation is carried out safely through the use of motors, Root canal rinsing for cleaning and disinfection using effective chemical preparations can now be supplemented by hydrodynamic procedures. For obturation, as well as the classic methods, there are modern sealer adhesives and cements made of composites, which also enable resilient post restorations if needed. Fibreglass-reinforced polymer or metal post systems for stable fixation can also be used, in addition to any necessary prosthetic treatments. For clinically complex problems, endodontologists are offered complete systems that also contain files with special geometries for the revision of treated root canals. As a last resort—for root tip resection, for example—there are powerful magnifying glass systems or surgical microscopes available for minimally invasive endodontic microsurgery for skilled experts. “For dentists that are wanting to start in endodontics as well as specialists in tooth preservation, a visit to the 33rd International Dental Show is an exceptional opportunity to become more familiar with the entire spectrum of this fascinating area,” said Dr Markus Heibach, Managing Director of the VDDI. Osseogenesis and stem cell technology find their way into specialty There is now doubt that implantology has become the fastest growing sector in dentistry. Within only five years, the number of implants inserted in Germany has doubled to more than 700,000 per year. Total annual growth rates of some 10–15 per cent also reflect the growing demand of aging patients for a modern implant-based dental prosthesis. The development towards looking after the remaining teeth has been the result of immense research and development activities in both academia and in the dental industry. At IDS Cologne, dentists and dental technicians will have the opportunity to gain a full overview of these latest concepts regarding implant geometries and materials, as well as the range of suprastructures currently available. These methods have become alternatives to conventional procedures, for example the sinus-lift. lutions adapted to the respective indications and enabling them to choose the right ones for their practice. Optimised procedures for preserving the alveolar process, but also for augmentation or alveolar distraction osteogenesis are on display. Another focus will be on state-of-the-art biotechnological procedures for osseogenesis including adult stem cell technology. Several manufacturers will be showing their latest developments for using bone marrow stem cells and other osteogenic factors and their integration in what are referred to as scaffolds (support membranes). Intramedullary resorbable rods or membranes, in some cases made of collagen or mucous membrane material are just some of the new topics to be addressed during IDS. Furthermore, there has been progress in the field of implantological instruments, such as atraumatic forceps, that are gentle on alveolar tissue or improved hollow cylinder osteotomes. Furthermore, state-of-the-art, high-resolution computer tomographic navigation procedures for use in combination with cast laser scans have made their way into implantology. Used in combination with other high-tech diagnostic methods, such as digital x-ray, implantologists are able to receive extremely precise stereo-lithographic drilling templates that enable the preparation of the implant bed with the highest precision. Visitors to the show in Cologne will also be confronted with a wealth of so- Implantology specialists will be presenting their latest curricular projects at the IDS thereby offering interested dentists the perfect opportunity to enter and train in this increasingly important discipline. A new German-Israeli study found that 98.5 per cent of all implants are successful and there was no discernable bone loss in 88 per cent of the implant sites they observed. The study goal was to determine the level of bone loss over time at the implant sites in the jaw. The high-performance ceramic zirconium oxide in particular is characterised by a broad range of indications and high aesthetics with practically no visible difference compared to natural teeth. CAD/CAM processes are most frequently used to process it, but the substructure construction can also be done using traditional wax modelling followed by the CAM process. Metallic prosthetic materials have always been important as bake-on alloys with a high gold content for crowns and substructures and are used mainly with the established casting technology. What is the latest in CAD/ CAM development? Anyone well acquainted with the field predicted early on that manufacturing centres would play a crucial role: high capacity utilisation, specialised staff, in milling procedures and, just recently, in the laser sintering of metals is often completely forgotten. The manufacture of metal restorations (eg, non-precious metals, titanium, or gold alloys) will thus eventually become a domain of CAD/CAM technology. What does the future of CAD/CAM technology hold? Only a few years ago, discussion focused on exactness of fit, the reduced costs for dentists and patients, and user-friendliness. The quality of CAD/CAM restorations was viewed with cynicism, and only a few pioneers gave scientific attention to this technology. At present, the situation is quite different. The hesitant and doubtful attitude towards computer-manufactured dental prostheses has been discarded, and an accepted, standard procedure has taken its place. Many companies now invest immense resources in the further development of this technology. What impelled this rapid change? On the one hand, the value of zirconium dioxide ceramic in particular, which can only be processed with computer-assisted techniques, became evident. This material made all-ceramic fixed partial dentures possible for the first time. Other ceramics, too, exhibited better material properties after automated milling because the blanks used could be industrially manufactured under optimal conditions. On the other hand, the technology of CAD/CAM systems has clearly improved. Based on more powerful computers centralised material purchasing, and high quality standards for ‘standard care’ facilitate an economical throughput, which in turn enables the amortisation of the investment in highly developed manufacturing machines, while increasing economic efficiency. Mid-sized and smaller labs will use their core competence in the computerassisted manufacture of high quality, aesthetic restorations and special fabrication of partial prostheses. Another important current trend is the chairside manufacture of inlays, onlays, partial crowns, and single crowns. The dentist is this CAD/CAM procedure’s target group. The oneappointment treatment has a time-saving benefit for the patient and eliminates provisional restoration, which additionally minimises the risk of cusp fracture, enamel-margin chipping, and weakening of the dentine bond. The biogeneric formation of occlusal surfaces enables the reconstruction of missing occlusal surfaces for inlays, onlays, and partial crowns according to nature’s designs (Figs. 2, 3). CAD/CAM and all-ceramics are often mentioned in conjunction with each other, which is understandable given the discussion above, but this doesn’t represent all the options.
