Osstem Implant Gears up to captivate
global customers with its award-winning
unit chair ‘K5’

K3, the unit chair of Osstem implant, has been exported to 72
countries, including the United
States and China, and its accumulated sales exceeded 40,000 units
in the global market as of last November. And Osstem’s next generation unit chair, ‘K5’ has been officially released at the ‘IDS 2023’. Osstem Implant is confident that K5
has captured the hearts of visitors
from all over the world.
Six divisions at Osstem Implant concentrate on developing
the ‘unit chair’ on which all dental treatment activities are conducted.
The unit chair is an agglomeration of mechanical engineering and
electronic engineering. It is largely
developed through a mechanical
engineering approach, such as in
its implementation of stable engine torque and strong power, and
in its improvement of driving speed
and adjustability, including upward
and downward mobility and rolling. It also requires electronic engineering technologies to enhance
the functions of attached instruments, such as handpiece, intraoral
camera and suction machine, and
the structural design of internal
pipes for water supply and drainage, as well as a communication
module that controls instruments,
and sensing processes.
For developing and manufacturing the unit chair, five divisions
at Osstem Implant cooperate systematically with the Unit Chair R&D
Center and each other. Collaboration divisions include the Marketing Division and Design R&D Center for product design and development, the Dental Equipment
Production Division that manufactures and manages product quality,
the Overseas Smart Dental Service

Division and Smart Dental Education R&D Center that provide
speedy after-sale customer service
and technical and equipment training.
Osstem Implant focuses on
achieving the ‘world’s top quality’
and a ‘robust design’ in developing
unit chairs. To achieve the highest
level of quality, Osstem Implant’s
unit chairs undergo strict assessment procedures. The company
evaluates 48 items in ‘product reliability’ and ‘use convenience' areas.
For example, in order to conduct a
‘durability test’, it repeatedly lifts
and drops a 150 kilogram-weight
75,000 times upon the unit chair
seat where patients lie down. It
measures the corrosion resistance
of metal components and plated or
painted parts by spraying salt water
on them. In addition, performance
of the unit chair is tested under extreme conditions in a ‘room chamber’ being usually used in developing vehicles. “These are basic tests
to provide products with confidence in regions with different climates, including South America,
Europe, the Middle East, and
Africa,” explained a manager at
Osstem Implant. The company believes that the excellent quality of
unit chairs achieved through such
rigorous product assessments contributed to concluding a contract to
supply 1,500 units of ‘K3’ to the
Egyptian government in 2022.
Placing emphasis on user experience (UX) as much as mechanical design, Osstem Implant
“reinforces user-oriented design”.

And it is designed to prevent a patient from moving during treatment by adding the tilting function
that raises the lower board when
the seat back descends. For comfortability of patients as well as for
convenience of dentists in treating,
the thickness and hardness of
sponge are meticulously calculated. To help patients not feel discomfort despite treatment lying
down for a lengthy time, Osstem
Implant developed an ergonomic
seat by improving body pressure
distribution. The persistence of Osstem Implant is also demonstrated
in the fact that it improved the fixation power of the dispenser so
that several paper cups are not extracted at one time when patients
pull out a paper cup for gargling by
repeating extraction tests over
70,000 times with different quality

enamel.1, 2 The main mechanism of
pain is because of the fluid movement inside the dentinal tubules,
which may result in pulpal nerve
stimulation. It is important to remember that not all cases of exposed dentin will necessarily mean

the presence of exposed dentinal
tubules, and that’s because of areas
where sclerotic dentin might be
present.
The consumption of acid-containing foods and beverages (such
as acidic drinks and fruits) can also
increase the risk for sensitivity. Bulimia and acid reflux can also increase and/or at least participate in
enamel erosion; and it should always be on the dentist’s radar for a
proper diagnostic.

Regarding dental whitening,
the occurrence of a mild temporary
sensitivity is something that can be
associated with this kind of treatment. However, it is also usually
gentle and transient in most cases. 3
It is important to note that this
temporary sensitivity does not
usually have a permanent impact.
As mentioned before, besides
areas of exposed dentinal tubules
(usually present in areas of gingival
recession), sensitivity can also
occur with poorly fitted or aging
restorations, in areas where non
carious cervical lesions are present,
or with non-detected caries (especially in interproximal regions).
In relation with the kind of whitening agent, a comparative systematic review of tray-delivered
carbamide peroxide gels versus hydrogen peroxide products (for athome whitening) found that both
methods had relatively equal levels
of tooth sensitivity and gingival irritation.4
It has also been reported that
transient mild to moderate tooth
sensitivity can occur in up to twothirds of users during early stages
of whitening treatment.5 Therefore
it is something that can be considered a normal occurrence.
It is fundamental to know that
this kind of tooth sensitivity can
and should be treated, and there
are many ways for a professional to
do it. Treatments range from sealants to diode lasers, including potassium nitrate gels and fluoride.

Chemical neural agents such as
potassium nitrate are an easy, fast,
and cost-effective method, and
usually the first option for most
cases. Ultradent Products offers
UltraEZ desensitizing gel as such a
method. The gel can be used on the
patient’s custom trays, or can also
be used through its exclusive prefilled trays. Potassium nitrate also
has the advantage of providing
faster results compared to other
methods.
Another desensitizing method
is the use of fluoride varnishes,
such as Enamelast varnish. The
mode of action is different from
potassium nitrate, in the sense that
fluoride is expected to temporarily
block the entrance of exposed dentinal tubules.
As mentioned before, another
additional method is the use of
diode lasers. These can be helpful
because of photobiomodulation
therapy, also called PBM.
Below there’s a guide that
might be helpful in treating patients with dentin hypersensitivity:
Pre-whitening sensitivity
treatment
1. Patient has history of sensitivity or tests positive to touch or
air exam.
2. Suggest dental check-up to
check for decay two weeks
prior to whitening. Create custom trays as needed.
3. Patient brushes with Opalescence Whitening Toothpaste
Sensitivity Relief prior to and
during whitening treatment.
4. Apply an Enamelast* fluoride
varnish treatment. Whitening
can begin the following day.
5. Trays are evaluated for fit, gingival irritation, and ease of removal.
6. Patient begins whitening with
low concentration whitening
gel.
7. Patient wears tray with
UltraEZ* desensitizing gel for
15–60 minutes.
During whitening treatment
1. Provide reduced concentration whitening product if possible.
2. Patient reduces wear time.
3. Patient skips a day or more of
whitening.
4. Patient brushes with Opalescence* Whitening Toothpaste
Sensitivity Relief.
5. Patient uses UltraEZ* desensitizing gel for 15–60 minutes
before and/or after whitening
treatment.
6. If gingival irritation occurs, review technique for filling and
inserting tray. If necessary, patient may use OTC oral pain relief products as directed.
7. If patient is unable to manage
whitening sensitivity, discontinue whitening treatment.
Finally, it is a known fact that
the use of UltraEZ desensitizing
agent for 30 minutes prior to whit-

ening helps decrease tooth sensitivity.⁶ The use of an active 3% potassium nitrate and 0.11% fluoride
desensitizing agent for 30 minutes
prior to whitening decreased tooth
sensitivity when compared with a
placebo in a population at risk for
tooth sensitivity.
It can be concluded that the desensitizing agents are effective in
relieving dentin hypersensitivity.
The short-term treatment of dentin
hypersensitivity with UltraEZ gel
showed a statistically significant reduction in the sensation of pain
when compared with the placebo.7

Dental students say artificial intelligence
should be included in curricula

Artificial intelligence (AI), a rapidly
evolving technology, is increasingly
relevant in various medical fields, including dentistry, for tasks like diagnosis, treatment planning and data
handling. A new study seeking to
understand how AI is perceived by
dental students in India has identified potential obstacles to its integration into dental practice. While a
majority found AI in dentistry exciting and believed it would lead to
major advancements, there were
mixed feelings about AI replacing
dentists and its role as a definitive
diagnostic tool.
The cross-sectional descriptive
study was conducted across eight
randomly selected dental schools in
India, and the participants included
both undergraduate and postgraduate dental students. The study,
which ran from August to October
2022, used a questionnaire that included both close-ended and openended questions covering socio-demographic information, sources of
AI knowledge and perceptions of
AI’s scope and application in dentistry. Out of the 937 respondents,
the majority were female (67.7%),
and undergraduates made up 84.3%
of the participants.
The study found that awareness
and basic knowledge of AI in dentistry were relatively high among
both undergraduates and postgraduates, 62.8% understanding AI’s

working principles, but that awareness of its dental applications was
divided. The primary source of information about AI for these students
was social media (55.4%). Based on
this finding, the researchers recommended integrating AI into current
dental curricula to ensure dental

students are actually receiving relevant, evidence-based information.
According to the study, while the
students saw AI as a significant advancement in dentistry, they did not
believe it would replace dentists,
mainly owing to the sensory and interpersonal aspects of dental care—

although very few said that AI is not
patient-friendly or that it has a limited future. Indeed, the majority of
the dental students found AI exciting and saw it as a valuable tool for
diagnosis, prognosis and treatment
planning. They recognised its potential in areas like radiographic di-

agnosis, soft-tissue lesion diagnosis,
3D implant positioning and forensic
dentistry.
Significant barriers to AI adoption in dentistry were also reported,
including insufficient training at dental school and a lack of awareness
and of technical resources. Additionally, concerns about the cost-effectiveness of AI and its inclusion in
the dental curriculum were noted.
Most students agreed that data
used in machine learning must be
handled carefully to comply with
data protection regulations, ensuring patient data confidentiality. The
findings also suggest that combining data using AI requires collaboration among clinicians, researchers,
policymakers and the industry to
maximise benefits and minimise patient harm.
Limitations of the study include
its sample size and methodology,
which might have influenced the results. The authors recommended
that future research should focus on
developing diagnostic models of
greater accuracy and guiding policymakers on integrating AI into
dental education.

Dr Fatma Banday graduates with merit the
“Master of Science Specialist Practise of
Pulpal Therapy and Endodontics”

Dental Tribune Middle East and Africa interviewed
Dr. Fatma Banday, a General Dentist who previously
lived and worked in Karachi, Pakistan. Dr. Fatma recently
obtained, with merit, a Master's degree from the University of Bolton. This accomplishment was achieved
through an Approved Prior Learning (APL) process via
the Centre for Advanced Professional Practices (CAPP)
and City of London Dental School (CoLDS). Dr. Fatma has
now moved on to pursue another Master's in Periodontics at Rutgers.

It is a pleasure to feature an experienced dentist
like yourself in the Dental Tribune Middle East and
Africa edition. First of all, tell us a little bit about
yourself, your educational background, and your
work experience?

I earned my dental degree from Ziauddin University
Dental College in Karachi in 2018. Following my graduation, I completed my internship and subsequently joined
Banday's Dental Clinic, our family-owned practice,
where I devoted two years to enhancing my clinical expertise. During my tenure there, I recognized the importance of continually improving my skills to provide the
best care to our patients. That's when I decided to enroll
in the Clinical Endodontics Professional Training organized by CAPP, a pivotal decision that greatly enriched
my professional journey. CAPP played a significant role
in honing my abilities and helping me achieve a higher
level of excellence in my practice. This commitment to
continuous improvement is something I remain dedicated to as I continue to serve my patients today.

You graduated from the Clinical Endodontics PG
Diploma program awarded by the British Academy of
Restorative Dentistry (BARD), CAPP-Tipton Dental
Academy, and organized by CAPP in Dubai in September 2021. Can you tell us about your experience in

the program and what it consisted of? How has the
program helped you in your daily clinical life?

My experience with CAPP-Tipton Dental Academy
was truly transformative. This program provided me
with invaluable knowledge and skills that have had a
profound impact on my daily clinical practice. The program was comprehensive, covering a wide range of advanced endodontic techniques, treatment modalities,
and the latest advancements in the field. It emphasized
hands-on training and case-based learning, enabling
me to gain practical experience in challenging clinical
scenarios. Additionally, the exposure to renowned experts and specialists in the field was inspiring and expanded my horizons, such as Prof. James Prichard, who
is an amazing teacher – both academically and clinically.
The knowledge and skills I acquired during the program
have significantly enhanced my daily clinical life. I now
approach endodontic cases with a more in-depth understanding, enabling me to diagnose and treat patients
more effectively and with greater confidence. The techniques and insights I gained have not only improved the
quality of care I provide but also allowed me to achieve
better patient outcomes.

After completing the Clinical Endodontics PG
Diploma, you also applied through the Approved
Prior Learning (APL) process via CAPP and CoLDS
for a UK Master’s degree. Recently, you successfully
completed the Master of Science in Specialist Practice of Pulpal Therapy and Endodontics with merit,
awarded by the University of Bolton. Could you share
more details on how you achieved this and what your
overall experience was? What advice do you have for
dentists who wish to follow your steps and achieve
this remarkable recognition?

I had the privilege of earning a Master's degree in
endodontics, a significant milestone in my career journey, with invaluable guidance and support from the
dedicated coordinators at CAPP. They played a pivotal
role in connecting me to CoLDS, where I embarked on a
transformative educational path. The program was
thoughtfully designed with self-paced learning in mind,
featuring three comprehensive modules that delved
into various aspects of research methodology and article interpretation. With the unwavering assistance of our
accomplished tutors and mentors, we successfully navigated through each module, diligently completing the
tasks assigned. This rigorous process culminated in the
crowning achievement of my academic journey – the
completion of my dissertation. In retrospect, this experience was instrumental in refining my research skills,
and it deepened my understanding of the endodontic
field. It instilled in me the ability to critically evaluate and
contribute to the ever-evolving body of dental knowledge. I am immensely grateful for the support I received
from CAPP, CoLDS, and my mentors, as they have been
instrumental in my professional growth and prepared
me to make valuable contributions in my field.

You have lived in various places around the globe,
such as UAE, Pakistan, and the USA. What are the
similarities and differences in dentistry between the
places you have lived in?
Living in various countries, such as the UAE, Pakistan,
and the USA, has given me a unique perspective on the
field of dentistry. Each place has its own distinct characteristics and nuances when it comes to dental practices.
In the UAE, I observed a strong emphasis on state-ofthe-art dental technology and a diverse patient population. Dental clinics often incorporated the latest advancements in treatments and equipment to provide
high-quality care. There was also a focus on cosmetic

dentistry due to the diverse and
cosmopolitan nature of the population. In Pakistan, I noticed a deeprooted tradition of family-owned
dental practices. The close-knit relationship between patients and their
dentists is a hallmark of the dental
landscape. Dental care in Pakistan
often centers on affordability and
accessibility, making it an area
where dental professionals need to
be resourceful and versatile in their
treatment approaches. The USA, on
the other hand, has a robust dental
education system and a stringent licensing process, which sets high
standards for dental professionals.
The emphasis on continuing education and specialization is significant
in the United States, resulting in a
wide array of dental specialists in
various fields. Despite these differences, the underlying similarity in all
these places is the dedication of
dental professionals to patient care.
Regardless of the location, dentists
share a commitment to improving
oral health, and this common goal
transcends borders. My diverse experiences have given me a broader
perspective and a deep appreciation for the richness and variety in
dental practices around the world,
enabling me to adapt and integrate
the best practices from each place
into my own clinical approach.

Dr. Fatma, what are your hobbies, and what do you like doing in
your private time?

In my personal life, I find great
joy in spending quality time with my
family, fostering strong connections, and making cherished memories together. Outside of work, I am
an enthusiast of various sporting
activities, with golf being a favorite
pastime that allows me to unwind
and stay active. Additionally, I have
a profound passion for exploring
different cultures around the world,
and I am always excited to embark
on new travel adventures. The opportunity to discover unique customs and traditions is truly enriching. Moreover, I have a deep appreciation for culinary experiences,
often indulging in the pleasure of
exploring diverse cuisines and discovering new restaurants. This is a
special treat that I reserve for weekends, where I can savor delightful
flavors and share the experience
with friends and loved ones. These
activities provide a well-rounded
balance in my life, nurturing my love

Restoration of a fractured ceramic
crown with a digital workflow

The following case outlines the
restoration of a fractured ceramic
crown with a three-unit bridge utilizing a digital workflow. A 38-year-old
female patient visited our dental
practice, reporting discomfort in the
maxillary left area. Upon clinical examination, a fractured ceramic crown
on the posterior abutment (tooth
#27) of a three-unit bridge (from
tooth #25 to tooth #27) placed seven
years earlier by her previous dentist
was identified (Fig. 1).
The initial step involved removing the old bridge and assessing the
abutment preparation. A temporary
resin bridge was then placed using
Temp-Bond (Kerr; Fig. 2).
One week later, a digital impression was captured using an intra-oral scanner (DEXIS IS 3800; Fig.
3) and transmitted via the DEXIS IS
Connect platform to the dental laboratory. In the laboratory, the design and milling of a new zirconia
bridge were executed. Four days
later, the new layered zirconia
bridge, accompanied by 3D-printed
models, was delivered to the practice (Fig. 4). The final zirconia bridge
highlights the quality and precision
achievable through a digital workflow (Fig. 5).
Following the try-in, the ultimate restoration was cemented in
the mouth using glass ionomer cement (Fuji I, GC). Six weeks later, the
patient returned for a postoperative
check-up, revealing perfect contact
points and excellent adaptation
with the soft tissue (Fig. 6).
This case illustrates the successful integration of cutting-edge dental technology, precise craftsmanship, and meticulous care in providing our patient with a durable, functional, and aesthetically pleasing
zirconia bridge, ultimately enhancing her oral health and comfort.

Achieving a higher standard in
implant therapy for your patients

As the Baby Boomer population ages, the numbers of edentulous and partially edentulous patients increases, since tooth loss
and age are related. Whether it is
due to neglect, caries, medications
or other systemic reasons, patients
are presenting to practices all over

the country needing extractions
that can eventually lead to missing
teeth of full arches that need implant
supported
restorations.
These patients often have aesthetic
concerns from missing teeth, or
ill-fitting or loose partial or full
dentures and the inability to eat or
function as they once did with
teeth. Because of these concerns, it

is important to incorporate some
type of implants into the plan. Implants, whether narrow or traditional width, allow patients to
smile, eat and function much more
effectively.
Case 2
A 43 year old female presents
with missing premolar and molar

upper right side (#15,16) and missing premolar upper left (#25). These
teeth have been missing for 15+
years. A flapless approach was selected to minimize trauma and to
facilitate quick healing. The Axiom

emergence profile that would be
difficult to detect from the natural
dentition.

Conclusion
In conclusion, by utilizing a

Intra-oral scans may present more humane
option for evaluating clefts in infants

and alveolar bone are the most

common congenital anomalies of
the head and neck, resulting in
feeding, psychological, craniofacial, and speech challenges. In infants, care may involve preopera-

tive appliances, for which impressions of their clefts are required.
Conventional
impression-taking
techniques pose risks such as ingestion and suffocation. A study at

pressions to be as accurate but
more acceptable for guardians.
The study involved seven infants aged 0–28 days diagnosed
with complete unilateral cleft lip
and palate. Impressions of their
clefts were taken with the conventional method using an irreversible
hydrocolloid impression material
and with an intra-oral scanner.
Stone models of the conventional
impressions were scanned, creating virtual 3D models, and the intra-oral scans were saved as virtual
3D models and 3D-printed.
The virtual models from both
methods were superimposed to
compare the alveolar arch width
and alveolar cleft defect. The maximum alveolar arch width and the
maximum distance between the
premaxillary segments were measured on the physical models from
both techniques using vernier calipers. The superimposed 3D scans
of the conventional and digital impressions showed significant differences in three of the cases. However, the caliper measurements
showed no significant variation between the conventional and digital
impressions.
Additionally, the infants’ guardians completed a questionnaire on
their acceptance of both impression techniques, and their answers
revealed a distinct preference for
the digital method. Two significant
findings were that the guardians
felt that the conventional method
was more invasive and that they
believed their infant had suffered
during its application.
The study indicates a shift away
from traditional impressions owing
to associated risks and the stress it
places on both patients and guardians. Digital impressions emerged
as safer and preferred because
they minimized risks to infants as
well as eased guardians’ concerns.
The study also showed that digital
impressions are accurate and efficient. Digital impression-taking
also offers the advantage of creating reliable models for future treatment planning and provides visual
aids to parents that showcase the
potential improvements in their infant’s condition.

Hyaluronic acid fillers:
Can fuller lips be too full?

Lips come in various shapes and
sizes, with fuller lips often perceived
as a physical manifestation of youth
and beauty. Consequently, many
women opt for lip augmentation
procedures each year. However, do
lip fillers consistently enhance lip attractiveness?
Numerous studies highlight the
significant role facial appearance
plays in social interactions, contributing to the mainstream acceptance
of facial aesthetic treatments. Lip
augmentation, in particular, is
widely accessible and trendy, especially among celebrities and individuals with naturally thin lips.
According to the 2017 American
Society of Plastic Surgeons statistics
report, surgical lip augmentation increased by 60% between 2000 and
2017. In 2016 alone, 2.6 million filler
procedures were performed in the
US, with the numbers projected to
rise annually. Hyaluronic acid, a minimally invasive treatment for lip
augmentation, is deemed safe, predictable, and enjoys high patient
satisfaction. To aid clinicians in tailoring hyaluronic acid use to indi-

vidual patients, researchers from
Brazil investigated whether natural
lip thickness before filling affects attractiveness post-lip augmentation.
Dentists, facial harmonization specialists, and laypersons were enlisted to evaluate the attractiveness
of the lips of 16 women, grouped
into eight with thin and moderately
thick lips and eight with thick and
full lips, both before and after filling
with hyaluronic acid.
The researchers reported that
respondents perceived participants
with thinner lips pre-treatment as
having significantly improved lip attractiveness after the filling procedure. However, thicker lips were
considered more attractive than
thinner lips both before and after lip
augmentation, although they decreased in attractiveness after the
filling procedure.
The findings suggest that
greater lip volume may not always
be more aesthetically pleasing. The
researchers noted, "There seems to
be a limit to the amount of filler to
be injected in patients who already
have a certain volume in the lips before filling."
In light of these findings, the researchers recommend that dental

professionals base their recommendations on the amount of filler to be
injected on the patient's lip volume
pre-treatment and consider other
factors that significantly impact lip
shape preferences, such as ethnic
background and profession. They
concluded, "Clinicians must be
aware that beauty is an ever-evolving concept subject to trends. The
patient expects his doctor to be up
to date with the latest scientific literature published in the field and
aware of beauty trends. A good talk
between the professional and the
patient will awaken trust between
them."

CBCT data could help create patient-specific
scaffolds for periodontal tissue regeneration

Leveraging advancements in 3D
printing, Greek researchers have
developed a methodology for designing patient-specific 3D scaffolds

for periodontal tissue regeneration
using CBCT data. Preliminary results
indicate that utilizing CBCT data to
create 3D models of the hard tissue
around periodontal defects shows
promise for personalized periodontal treatments. The ultimate goal is

to
produce
bioabsorbable
3D-printed scaffolds that also function as sustained-release drug carriers, targeting periodontitis.
Periodontitis leads to osseous
defects that compromise tooth support. Traditional methods like graft-

ing use donor bone tissue as scaffolds for new bone formation, but
achieving a perfect fit to prevent
soft-tissue proliferation remains a
challenge. The emergence of CBCTbased 3D modeling offers a promising solution. This innovative approach aims to design patient-specific scaffolds, paving the way for
personalized periodontitis treatment and broader clinical applications.
The process involves two key
scaffold design methods: one for
periodontal
defect-customized
block grafts and another for extraction socket preservation customized grafts. The significance of
these methods lies in the personalization of treatment using CBCT
data to model the teeth and alveolar
bone in regions with periodontal
defects.
Both models of the scaffolds
and models of the hard tissue
around the periodontal damage
were 3D-printed using a fused
deposition modeling 3D printer.
These advancements lay the foundation for 3D printing bioabsorbable scaffolds tailored for periodontitis treatment that can potentially
act as drug delivery systems.

The novelty of the research lies
in its emphasis on detailing the scaffold design process and its ability to
capture intricate osseous defects,
thereby producing highly accurate
scaffolds. This research opens avenues for future exploration and clinical applications, including improved image segmentation algorithms, comparisons of 3D-printing
techniques, utilization of biodegradable materials for drug delivery, and clinical testing of 3D-printed
grafts in patients.
However, a significant limitation
is the study's inability to compare
the accuracy of the CBCT models
with real patient scenarios, highlighting the need for future collaborations with clinical researchers. As
biotechnological
advancements
continue, it is anticipated that related ethical and regulatory challenges will be addressed.
