Small turbine handpieces for
top-level requirements
By W&H
Small, but mighty! The TK-94 L is
the smallest of W&H’s Synea Vision
turbine handpieces, convincing with
modern technology, intelligent design, and optimal power. The turbine
handpieces are particularly suitable
for minimally invasive procedures
thanks to their extra small head
sizes. They are mainly used in geriatric and paediatric dentistry as well as
for nervous patients as they allow
stress-free and gentle treatment.
The TK-94 L microturbine is the
smallest turbine handpiece in the
W&H range. With a diameter of just
9 mm and head height of 17.2 mm,
including mounted 16 mm bur, it ensures safe and efficient treatment in
the smallest spaces. “As a dentist
who treats patients, the main benefits of the TK-94 L are the ergonomics and functionality resulting from
its slim design”, explains Salzburg
-based paediatric dentist Dr Dinah
Fräßle-Fuchs.

Small head, high-quality features

When it comes to features, the
TK-94 L can easily keep up with
W&H’s larger dental turbines. The
5× spray constantly cools the bur tip
in every application position, while
the innovative LED technology perfectly illuminates the treatment site.
Despite the smaller turbine head,
the integrated Power Blade Rotor
boasts an impressive 14 Watts of
power and thus provides remarkable torque. The precision instrument is also equipped with the patented W&H hygienic head system.
The circulating air in the turbine
head prevents aerosol particles
from being sucked in as the rotor
runs down.

Sustainability through
quality

Even after numerous sterilization cycles, the ScratchBlocker coating of the Synea Vision turbine
handpiece doesn’t show any signs of
wear, an indication that the W&H
microturbine also impresses when it

comes to easy maintenance. A
high-quality ceramic ball bearing is
integrated into the turbine head. All
in all, the W&H turbine handpieces
are robust, long-lasting, and reliable
quality products with high-value retention.

Stress-free and gentle treatments

Patients benefit from precise,
safe, and gentle treatment thanks to
the microturbine’s light weight and
non-slip profile. The ceramic ball
bearing ensures dentists can work
quietly and without vibration, which
is particularly advantageous when
treating children or elderly patients.
“I don’t want my patients to be
nervous or in pain during treatment.
I consider the TK-94 L to be ideal for
paediatric dentistry – it simply unbeatable when it comes to carrying
out treatment in hard-to-reach
places or if patients cannot open
their mouths that wide”, says
Dr Dinah Fräßle-Fuchs when discussing the benefits of the W&H microturbine.

World-class Osstem Implant Dental
Clinical Education System
By Osstem Implant
Osstem Implant, the world's
No. 1 implant sales company, invites
1,500 dentists from 22 countries, including China, Japan, Germany, and
Australia, to its headquarters in
Korea to provide clinical training this
year.
Osstem established South Korea’s first implant training centre in
2001 and provided dentists in South
Korea and other countries with free
training on implant surgery procedures.
By the end of 2022, the Osstem
training centre had conducted implant surgery training for more than
110,000 dentists across the globe,
85% of whom were foreign dentists.
Osstem expects some 160,000 dentists to complete its clinical training
programmes by 2026.
Osstem’s confidence in conducting invitational training comes
from over 20 years of offering clinical training to dentists and having
established
an
environment
equipped with the industry’s best

educational facilities and practice
equipment.
A systematic curriculum that
covers both theory and practice,
and the best educational environment
Osstem has established Osstem
training centres at its 32 overseas
subsidiaries in 36 countries to operate more than 100 regular education
sites and deliver online lectures on
live surgery and other relevant
areas.
Osstem’s Central Research Institute has a separate training facility.
The institute provides specialised facilities for each training course, including a digital seminar room, implant seminar room, operating room
set up for live broadcasting and
technician seminar room. Dental
professionals can also experience
the latest devices in practical
demonstrations, including SMS, the
implant surgery engine, the OneMill
5x five-axis dry milling machine
(Osstem), the Osstell Beacon implant stability quotient measuring

instrument and the TRIOS 4 wireless
intra-oral scanner (3Shape).
Osstem‘s clinical training programme takes both theory and
practice seriously. In particular, the
emphasis is on practical training so
that what they learn can be put to
use in practice. This year as well, we
will offer to German dentists a lot of
short-term hands-on courses based
on Osstem Implant products to
make difficult procedures, or those
that have never been attempted
previously, easy.
The most distinguishing feature
of this course is that it allows dentists
to experience the sinus procedure,
which is avoided by many dentists
unless they are oral surgeons,
through hands-on practice so that
any dentist can easily perform it. Osstem has made it easy for 10,000
dentists to perform sinus surgery
through
short-term
hands-on
courses for the past ten years. Sinus
surgery, which is always encountered while performing maxillary
molar implants, is in many cases
avoided as it is difficult for doctors

who are not inclined. With the crestal approach sinus (CAS) kit or the
lateral approach sinus (LAS) kit developed by Osstem Implant, the
sinus membrane can be raised easily
without being torn, and implants
can be placed in the desired area
after bone grafting.
Expanding the proportion of
digital education-centered curriculum and Supporting the popularisation of implant surgery
around the world
Osstem Implant selects lecture
topics according to the needs of
each country and organises courses,
but considering that digital planning, which can effectively respond
to any clinical condition and produce accurate results, they plan to
increase the proportion of lectures
that help in understanding digital
dentistry.
Osstem Implant provides basic
concepts and usage methods of
'OneGuide', Osstem's implant placement template, and applied technologies in various placement envi-

ronments using 'OneCAS' and
'OneMS' kits and templates through
excellent lecturers already recruited
from each country. lectures, etc.
Osstem explained the basic concept of a digital workflow to dentists
taking training and emphasised that
an in-office all-in-one system can be
built by utilising an intra-oral scanner, CBCT device, dental 3D printer,
and milling equipment. Osstem
commented, “It’s time now to get
started in the fascinating field of digital dental medicine”, underlining
the advent of the digital era. They
added, “Digital planning that can effectively respond to any clinical condition and produce accurate results
is now a key factor for a successful
implant surgery.” The company’s T2
CBCT unit is also demonstrated to
dentists to aid their understanding
of digital dental equipment.
Osstem Implant plans to invite
dentists from all over the world to its
head office in Korea to continue clinical training, and to provide various
practical training and seminars at
each overseas corporation.

A multi-tool for your office:
J-Temp Temporary Resin
By Ultradent Products
“If anybody has had to move a
dental office, you'll find out it's ten
times worse than moving a home,”
says Ultradent VP of Clinical Affairs,
Dr. Jaleena Jessop. “Often, there
are so many different products that
just have one indication for what it
can be used for.”
Closets full of single-indication
products can certainly create headaches during a move, but they can
also be expensive, both on time
and actual cost. Products that can
be used in multiple ways add simplicity to the complex workings of a
dental office. J-Temp™ temporary
resin offers multiple solutions in
one syringe. “J-Temp is a product
that kind of covers a variety of different needs,” Jessop says.
Dentists can utilize J-Temp
temporary resin for everything
from bite ramps and temporary occlusal buildups, to build structure
for isolation clamping, for temporary restorations, and for splinting
between implant copings.
“The first time I started thinking
about J-Temp, I was doing a root
canal on a maxillary molar, and the
mesiolingual cusp was fractured
below the gingiva. And I was just
really struggling to get isolation
before I irrigated the canals,” says
AD

Jessop. At that time, she was doing
a good bit of ortho work, and she
took an ortho product to use as a
barrier for irrigants and to help
support the cusps that were in
place before the clamp was put on.
Sure enough, it worked well. “I
thought about doing some composite and bonding, but when you
get down below the gingiva and
you try doing etch and then adhesive in this fractured area, it can be
difficult if not impossible to get isolation. I wanted something that
would remain until the final
build-up and restoration could be
placed. This was the idea for the
first of the indications for this product.”
The more Dr. Jessop started
using it for different niches, the
more she discovered how nice it
was to have one product capable of
covering a gamut of different needs
as opposed to five, six, or more different materials that may all expire
before they were used again after
opening.
And thus, J-Temp temporary
resin was born.
Emergency calls are nothing
new to dentists, and Dr. Jessop says
she’d continually receive them
from patients saying they’d just
had a root canal and the temporary
would be gone. “They would never

(Image: Ultradent Products)

call the endodontist; they always
call the general dentist. And so,
nine times out of ten, it had just
washed out. It wasn't gone. The
seal was still there, but it was incredibly washed out. So they'd get
food packed in there and they
struggled with the stress of a fracture—thinking they were going to
have to now have the tooth extracted or have to pay to have the
root canal retreated, this doesn't
happen with J-Temp. You put it in,
you make sure that the occlusion is
good, and you'd let them go.”
Temporaries are unique in that
there can be an unforeseen consequence to them being too good.
It’s a strange concept, to be too
good—but the perfect temporary
may lead to a patient feeling like
their problem is fixed and they
don’t need to return for a final restoration. “He didn't come back for
seven years. It looked too good,”
Jessop says of a patient she saw

soon after graduating dental
school. It was a particular situation
where her father, Ultradent Founder
and CEO Emeritus, Dr. Dan Fischer,
was out of town and needed
Dr. Jessop to place a temporary before he could do the full restoration. “I'm like, no problem, Dad. I
got this. I made the most beautiful
temporary. I mean, I was so proud
of myself. I took a picture of it and
I sent it to my dad and he's like, “Oh,
J, what did you do?” The patient
didn’t return for the full restoration
for the better part of a decade.”
When it comes to its unique
color, “J-Temp is light purple because we want just a subtle reminder to the patients that this is
still just a temporary restoration,
that they need to get back into
their doctor and get the final restoration placed.”
A common theme with the versatile J-Temp temporary resin, the
light purple color serves multiple
purposes. “For the doctor, it's also
a color that will allow them to determine what's temporary and
what's tooth structure, so that they
know all of the temporary is removed before they do the final restoration.”
“It's just lightly light purple [...]
you can use it on the lingual of an
anterior tooth. It doesn't show
dark. You can use it on the occlusal
surface, and it doesn't show too
dark. So that's why the light purplish color,” Jessop says.
J-Temp resin is a polymerizable,
temporary material. “It's nice because it doesn't require an adhesive
to be placed. It's self-leveling. It's
something that has a little bit of
shrinkage, but not much. It's a resin
that isn't really affected by peroxide,
so it can be used with the walking
bleach technique. There's almost no
oxygen inhibition layer to it.”
“You're not supposed to be
able to put resins over peroxide because oxygen inhibits polymerization. So you normally use more of a
putty type or a self-adhesive resin
cement like UltraTemp™ material
and it comes out within a few days.”
Jessop continues, saying she was
searching for something that had
more support, and structure to it,
so it didn’t fall out in between appointments.
You can also use J-Temp temporary resin to build splints between implant copings or with the
floss technique to help prevent implant distortion. “I have this patient
who years and years ago was
changing the tire on his car and he

got hit and knocked over an overpass and he got a facial impact, and
it knocked out a lot of his teeth,”
Jessop says. Following the accident, her father, Dr. Fischer, restored the patient with a bridge.
Over time, with perio and bone deterioration, the abutting teeth for
the bridge had to be extracted and
implants had to be placed. “Many
may already be aware of how difficult it can be to seat the framework
on implant abutments. This is often
due to the flexing of the implant
impression abutments during the
impression making procedure.
Being able to isolate and stabilize
the copings while the impression
material sets will be a game
changer,” Jessop says. She then
took another impression to test the
seating before final restorations
were made. “I just took a little bit of
floss, went around each one, and
then I reinforced with J-Temp. Place
the J-Temp along the floss covering
all but a very small segment, then
light cure. Only connect the J-Temp
to the coping once most of the
resin has already covered the floss
and been polymerized. This will reduce the polymerization stress,
thus reducing the distortion on the
coping. The next coping came back
and seated beautifully."
Dr. Brigham Stoker practices in
Utah while also working on Ultradent’s clinical team. He used
J-Temp to splint five implants together for a hybrid prosthesis impression. “Because of the implant
coping distortion that can occur
during the impression, the lab usually has to send me back a "jig" to
try so that we can assure the fit is
perfect. Then some sort of adjustment is often necessary, and a new
impression is taken. This adds several appointments into the process
and can be frustrating,” Stoker says.
“This time, I took the impression
with the J-Temp splint and told the
lab that I effectively made my own
jig with the J-Temp, included underneath this first impression. It
eliminated 2-3 appointments in the
process and the prosthetics fit perfectly!”
Ultradent Products was created
more than four decades ago when
Dr. Fischer saw a need for a new tissue management product and created Astringedent™ hemostatic.
Today, the tradition continues with
Dr. Jessop and Ultradent’s latest
revolutionary product, J-Temp
temporary resin.

Why the colour of your instrument matters
By HuFriedyGroup
In the world of dentistry, small
improvements can make a big difference. The same can be said for
dental instrumentation - a small
detail may have a major impact on
your ability to conduct a procedure
with ease and efficiency. Consider
the decisions you make when selecting the appropriate hand instrument for a procedure – the
shape of the working end is certainly important, but so is the
weight of the instrument, the surface of the handle which you feel
while gripping it, and more.
When small improvements
make a big difference, every element of the design matters. This includes the instrument finish. When
operating in the mouth, visual acuity is critical, and glare from bright
operatory lights or headlamps can
be detrimental to your accuracy
and outcomes.
This is one of the many reasons
why we developed the Black Line
collection of instruments. The
product portfolio has grown tremendously over the years. Not only
does it include surgical instruments, but diagnostic, endodontic,
and restorative instruments as well.
You may be curious how much of
an impact the dark coating can
truly have, and here’s how:

AD

01

The science behind the Black
Line Collection

Black Line instruments are engineered to optimise clinical outcomes by delivering efficiency
throughout the entire procedure.
The unique matte black coatings,
which have been applied to a plethora of HuFriedyGroup instruments,
reduce light reflection and enrich
visual contrast. This is a gamechanger when it comes to improving accuracy and patient care. The
following sections explore the impact of our Black Line Collection in
each area where they have been introduced.

Surgical instrumentation

The surgical Black Line collection consists of scissors, atraumatic
extraction instruments, periodontal instruments, and implant instruments.

The dark coating provides outstanding contrast and enables enhanced visual clarity while reducing
light reflection. Having the ability
to see the instruments through tissue during surgical procedures
boosts your confidence in the accuracy of your movements. The
performance-oriented coating also
provides superior edge retention.
For example, Black Line scissors
feature an extremely sharp blade
and a serrated edge to reduce tissue slippage.
Additionally, surgical Black Line
instruments improve accuracy and
control due to their sleek, lightweight, ergonomic handles.

Endodontic instrumentation

The sense of precision and accuracy that Black Line instruments
provide in surgical instrumentation
becomes even more apparent

when considering endodontic instruments. The highly precise nature of endodontic procedures demands instruments that exhibit exceptional attention to detail while
remaining durable enough to handle various endodontic procedures, ranging from basic to intricate microsurgery. Endo Black Line
instruments feature the same performance-engineered coating that
enhances lubrication, minimises
light reflection in harsh lighting
conditions, and optimises edge retention.
Black Line Rubber Dam Clamps
offer another excellent method to
further reduce light reflection and
enhance visual clarity. This is
achieved through the striking contrast provided by the dark clamp
throughout the procedure.

Diagnostic instrumentation

The HD Black Line Mirrors are
the latest addition to the Black Line
collection, and they highlight the
advantages of the black matte
coating for instruments, given the
critical importance of visual acuity
in accurate diagnoses. The frames
and handles of HD Black Line
Mirrors are crafted using a
Diamond-Like Carbon (DLC) coating engineered to enhance clinical
outcomes by providing superior
visibility during any dental procedure. The robust black matte coating reduces glare by up to 80%
compared to a standard metal mirror head and handle1. This black
coating creates a distinct contrast
between the instrument, the tooth,
and/or the surrounding tissue, allowing for easy identification intraorally and facilitating quicker and
more accurate visibility. The reduced glare can also enhance the
accuracy of 3D intraoral scans for
better modelling.
Don't overlook a diagnosis. It's
also noteworthy that the material
used in a dental mirror significantly
affects visibility. Most mirrors are
coated in rhodium or feature a
resin (plastic) casing. In contrast,
the High-Definition Mirror Glass
used in the HD Black Line Mirror is
38.5% brighter than rhodiumcoated mirror glass and 50%
brighter than other front-surface
mirror glass, offering true-to-life
colour accuracy. The matte finish
ensures that this brightness does
not increase glare, but instead, it

facilitates a sharper, distortion-free
image for quicker and more accurate visibility.

Small changes can make a big
difference

If this review of the Black Line
collection emphasises one thing,
it's that small innovations, such as
the dark coating on these instruments, can have a significant impact in the operatory. Whether
you're a specialist focusing on oral
surgery or endodontic procedures,
or a general dentist or hygienist
performing diagnostics, the quality
of your instrument can greatly enhance your experience.
HuFriedyGroup is honoured to
sponsor Dr. Olivier Carcuac's lecture and hands-on course on "The
Modified Free Gingival Graft - an
innovative approach for treating
gingival recession defects in lower
anterior teeth" during the CAPP
Conference & Exhibition on 27 and
28 October.

The success mindset: Why it is very
important to only work with the best
By Dental Tribune MEA
A passionate advocate for
high-quality dental care, Dr Miguel
Stanley created the No Half Smiles
treatment philosophy. He is committed to education in the dental
community, serving as vice president of the Digital Dentistry Society, an expert on the online education platform DentalXP and an adjunct assistant professor at the University of Pennsylvania School of
Dental Medicine in the US. He is
also involved in charitable work
through the Global Child Dental
Fund and Humble Smile Foundation. On 27 October, he will be presenting in Dubai at the CAD/CAM
Digital & Oral Facial Aesthetics
Conference and Exhibition. The
Centre for Advanced Professional
Practices (CAPP), which is organising the event, spoke with him about
his treatment focus, what attendees can expect from his presentations, and the importance of the
positive dentistry mindset now and
in the future.

Could you tell us about your
journey and what led you to become a dentist?

I was born in South Africa in the
seventies. Unfortunately, back
then, knowledge on the dangers of
consuming processed and sugary
foods was not what it is today, so I
had a lot of dental caries before I
was 12 and suffered quite a bit at
the hand of dentists—this gave me
a fear of dentists, so it’s ironic that
I ended up becoming one. Back in
the nineties, it was easier and faster
to become a dentist than to become a cardiologist, which was
what I really wanted to do. I never
thought I’d be a dentist, but then I
discovered that there was a huge
opportunity to grow within what
was a relatively unknown profession, and I’ve never looked back.
I’m very grateful to have become a
dentist in the nineties and to have
had the experience of the following
decades of great change. It’s given
me an amazing opportunity to see
the evolution of the field.

What areas of dentistry do
you specialise in, and what motivated you to focus on those particular areas?

I don’t think that in healthcare,
and in dentistry in particular, anybody is ever truly specialised, although of course one may have a
qualification or an accreditation
that says one is. I understand
though that the question really is
about the kind of a dentist I am. I
think that this can be explained
better by focus of interest. I am
trained in implant dentistry and
prosthodontics and have a particular focus on implant surgery and
complex bone regeneration, which

is one of the topics I’ll be talking
about in my lecture at the CAPP
event. I’ve been placing implants
now for over 22 years, and within
that field, my focus is comprehensive complex case management,
the difficult stuff using surgical
guides, software planning, 3D
printing and so on. In prosthodontics, I very early on started focusing
on occlusion-driven aesthetic dentistry, restoring a functional occlusion and not just achieving beautiful aesthetics. These two focuses of
interest combined cover the full
span of modern rehabilitation. My
focus of interest isn’t a single area;
it’s managing full-mouth comprehensive restorative dentistry with a
focus on biology, immunology,
function and aesthetics, using the
entire array of tools that are available to the modern dentist with my
long-time team at the White Clinic.

sion and planning. Follow gold
standard evidence-based protocols in a sterile environment without cutting any corners and by
doing everything properly—and
even then, you might have some
complications. Following these
rules means that you cannot have a
large volume of work if you are
building a practice that does a lot
of 3D bone regeneration for implant placement. It’s not about the
number of cases you treat; it’s
about the number of failures you
have or, more importantly, you do
not have. It’s the very careful and
delicate balance between your desire to produce more work and
have less failure. Success is a mindset where you have to balance your
desire to be financially successful
and be a leader in your industry
and to actually protect your patient
from harm. We all understand the
Hippocratic oath of “do no harm”,
and at many points, being a successful dentist and creating a positive environment require you to
slow down and do the right thing.
That is the mindset of positive dentistry.

What can attendees expect
from your lecture, titled “Building a successful practice around
successful regeneration”, and
seminar on positive dentistry at
the October conference in Dubai?

I’ve continually learned in life
that it’s never good to expect anything; rather one should keep an
open mind and be curious. I think
that a dentist has to be a teacher
first and foremost. That means that
I will be teaching the audience
something, but I also hope to learn
because one of the privileges of
engaging with the public is that you
can also get feedback from the audience. In my lecture, I will cover
the topics of how modern biomaterials can help create more predictable results in comprehensive periodontal and oral surgery solutions,
meaning complex cosmetic 3D
hard- and soft-tissue regeneration
protocols, such as the combination
of hyaluronic acid with xenograft
material, as well as new materials
such as magnesium membranes
and fully resorbable screws, which
would allow all the benefits of a
metal mesh but without the need
for a second-stage surgery. I’ll be
talking about that and why it is very
important—because I won’t be
only showing cases, but also be
talking about practice management—to only work with the best
companies and the best materials if
you want to run a successful practice, not just because the products
are good, but also because there’s
a lot of business intelligence to be
gained from the companies that
develop leading biomaterials.
Regarding the seminar on positive dentistry, that’s going to involve a lot of storytelling about
how to navigate the current trends
in modern dentistry, how to work
out what’s marketing and what’s
actually science, and how you can
create and elevate your practice

Dr Miguel Stanley, Portugal. (Photos: Dr Miguel Stanley)

around a positive mindset that is
founded on science, clinical evidence and a comprehensive understanding of the modern dental
workflow that will combine clinical
practice and technology because
the modern dental consumer is
smarter and is looking for better
solutions to complex problems,
and I think that every dentist is
looking for exactly that. How can I
be more successful and have more
time for my family and have the
recognition among my peers that I
deserve? That’s what we’re going to
talk about.

How does the concept of successful regeneration tie into the
principles of positive dentistry?
How do these two concepts complement each other in the context
of building a successful dental
practice?

Very simply: nothing positive
comes from a failed bone regeneration. Your day will go poorly, your
reputation will be affected, and you
will lose money, time and energy. If
you miscalculate even the simplest
of therapies through poor treatment planning or the selection of
the wrong material, or if you skip
steps or cut corners to save time

and/or money, thinking that you
are outsmarting the clinical evidence and the leaders of industry,
then inevitably you will create a
toxic environment for yourself,
your patient and your team. It’s essential to make the right choices
when planning treatment and purchasing biomaterials. We all know
that the best biomaterials are very
expensive. There’re a lot of cheaper
solutions on the market that claim
to do the same thing, but don’t
have the patents or the appropriately qualified people developing
them. We need to be very careful to
separate marketing from science.
In order to build a positive work environment, you need to make a serious commitment to excellence,
especially when doing bone regeneration. Failure is very expensive, so
we’ve got to get it right.

Could you provide an overview of the key elements or strategies involved in building a successful practice around successful regeneration? How do these
strategies align with the principles of positive dentistry?
My rules of life are very simple.
Treat fewer patients a day with a
higher degree of excellence, preci-

When it comes to implementing successful regeneration techniques, what are some common
challenges that dentists may face,
and how can these be overcome?
How does the positive dentistry
mindset help address these challenges?

First, you have to define what
success is for you. For me personally, success is not about being the
guy who does the most cases or
has the most money in the bank.
For me, success is going home
every day knowing that my patients
are satisfied with the work that we
have done.
How do you reduce complications and failures? Talking about
success is very easy, but in order to
have a truly positive work environment and a successful, positive
mindset in what is one of the
toughest jobs in the world—
requiring a substantial number of
algorithms to be considered and
followed every single time you engage with a patient in order to plan
and predict a successful outcome,
not to mention the dexterity, training, technology, materials, resources and team you have to have
put in place—you have to do things
properly and plan properly and
that takes time. This is an orchestration of great complexity and
means that the best periodontists
and the best reconstructive dental
surgeons in the world will not see a
high volume of cases a day.
Through the Slow Dentistry
Global Network—of which I’m the

co-founder—we have learned that many
of the dentists who suffer from mental anguish, mental illness, stress, fatigue and
burn-out all have one thing in common:
they see too many patients a day and are
under great pressure. Perhaps seeing a lot
of patients a day is okay if you’re doing
hygiene or fillings, but then again, in my
personal experience, it’s never okay. If you
are performing 3D bone and soft-tissue
regeneration with implants and bone
grafts, you really need to be on board and
hands-on with your patients because failure is something that is very prevalent. It
happens to the best of us, and you need
to really have an amazing team around
you to help work through the complexities of case management.
With that said, here are a few things
that I do that not many people believe. The
first thing is that, because tobacco directly
affects healing and I want to be successful
in treating patients, I don’t operate on
smokers. The second one is that I do not
operate on patients with low vitamin D levels. Botiss just launched botissCARE, so
now we can test vitamin D chairside and
immediately supplement with vitamin D3
and K2 to improve the blood concentration of vitamin D3 and K2 and thereby im-

prove, based on science, the quantity of calcium and mineralisation
of the bone, which is very important for successful regeneration.
Sometimes—and that is a message
I am bringing to the CAPP meeting—it’s not just about technique
and materials but also about improving the patient’s baseline and
about making very careful patient
selection if you want to be continuously successful. Sometimes, the
most powerful word for success is
“no”.

Looking ahead, what do you
envision as the future of successful regeneration in dentistry, and
how do you see positive dentistry
shaping the evolution of dental
practices in this regard?

Stem cell therapy is showing incredible advances in how we can
build new bone and soft tissue. I
did a documentary with National
Geographic in 2019 in which we
spoke to companies that are doing
bioprinting, 3D printing of living
tissue such as cartilage and gingiva.
We are already seeing incredible
steps in the use of CRISPR technology in genetic science, and I think
that, if you look at the work of the
best in the world in periodontal
surgery, the techniques have really
improved dramatically, but I think
that the greatest leaps will come
from understanding immunology,
physiology and new advanced biomaterials, smart materials that can
be custom-made for patients’ immune response, affording more
predictable and successful outcomes. Some companies, like
Botiss, can already customise and
3D mill bone, and depending on
where you are in the world, stem
cell technology is already available.
I think there’s going to be a very exciting future for all of us in this
arena, but it is very important also
to understand that we must stop
thinking of the mouth and the bone
of the oral cavity as separate; they
are part of a human system, and
our patients’ overall systemic

health will have a direct impact on
the success of the regeneration.
Likewise, we are not just doctors of
the mouth; we are doctors per se,
requiring us to think holistically. For
example, we shouldn’t expect
amazing regeneration results if the
patient has an adjacent tooth with
a massive infection. It’s also important to consider the role of the
bone marrow as part of our immune system, giving us a unique
opportunity when dealing with patients’ bone to cultivate an incredible amount of healing through the
way that we engage our patients’
immune system.
Part of my topic as well is therefore to discuss how immunology, in
what I like to call immune dentistry,
is playing a huge role in the way
that we think and the way that we
understand success in the future. I
believe that immune dentistry will
be one of the new trends, combined with the understanding that
we shouldn’t think of the mechanics or the macro-biology only but
also think of a patient’s individual
genetic understanding and acceptance of the therapeutic process
that we are applying. Globally,
there’s a small percentage of cases
that fail, and we simply accept the
reasons for this to be self-evident.
However, if we all focus on, let’s say,
a zero-failure policy, what would
that look like?
With this all in mind, I’d like to
come to the conference and talk
with some of these leaders that you
have invited and learn from them
as well. I look forward to sitting in
the front row and watching this
amazing show that you have put on
of science and education, and I’m
very grateful and very honoured
that you would invite me to be part
of this amazing meeting.

Align Digital Platform: Delivering a
richer, integrated and seamless experience
through digital dentistry solutions
By Dental Tribune MEA
Align Technology is a global
medical device company that has
reinvented the way orthodontic
and restorative treatment is delivered through its Invisalign clear
aligner system and the iTero family
of intra-oral scanners and imaging
systems. The company has been introducing its technologies to the
Middle East, and Dental Tribune
Middle East & Africa spoke with
Angelo Maura, General Manager
for the Middle East region at Align,
about the new innovations Align is
launching in the region, how expanding the application of its technologies, including artificial intelligence (AI), is benefiting both dentists and patients, in oral healthcare.

Would you tell us about some
of the new innovations Align has
launched in the region?

Align has launched several new
innovations in the past years. Most

recently, we introduced the
Invisalign Moderate package to the
Invisalign portfolio of treatment
options, which offers a 20-stage
clear aligner treatment designed
for Invisalign-trained doctors to
treat mild- to- moderate cases. This
increases the flexibility of the portfolio, offering a solution for cases
that fall between the existing
Invisalign Lite and Invisalign Comprehensive offerings.
As part of the overall Align Digital Platform (ADP), an integrated
suite of unique, proprietary technologies and services delivered as
a seamless, end-to-end experience
and workflow that connects users,
we have also introduced other innovations, including that of iTero
Element 5D Plus imaging system,
which launched in February 2021.
We call it 5D because it can simultaneously record 3D images, intra-oral scans, intra-oral colour and
near-infrared images—for interproximal caries—all in the same
full-arch scan in as little as 60 seconds. iTero intraoral scanners are

known for their orthodontic use;
however, iTero solutions also enhance restorative treatment predictability.
Another innovation is the Invisalign Outcome Simulator Pro, a
next generation advanced patient
communication tool which enables
doctors to show patients their potential new smiles after Invisalign
treatment, using in-face visualisation and/or 3D dentition view, all
done chairside in minutes. The
Invisalign Outcome Simulator Pro
expands Align’s existing Invisalign
Outcome Simulator technology
and adds the benefits of Align’s
ClinCheck “In-Face” Visualization
tool that combines a photo of a patient’s face with their 3D treatment
simulation, creating a truly personalized view of how their new smile
could look.
We’re continuously evolving
our technology innovation to help
give doctors and their patients an
end-to-end visual experience, from
engaging the patient to monitoring
the treatment plan, and as part of
Angelo Maura – General Manager, Middle East at Align Technology.

that commitment Align has
invested over $1.6 billion (AED 5.8
billion) on research and development, over more than 25 years in
the industry.

As the Invisalign product
portfolio expands, what benefits
will both dentists and patients
choosing to use Invisalign clear
aligners have?

As Invisalign's product portfolio continues to expand, the benefits for both dentists and patients
using Invisalign clear aligners will
become clearer. The improved digitalised approach to oral healthcare
has already yielded better clinical
outcomes, enhanced patient experiences, greater convenience, improved visualisation, and higher
practice growth for dentists. The
recent addition of Invisalign
Moderate, iTero tools, and the evolution of the Align Digital Platform
has further optimised the end-toend workflow for practices, allowing for a more tailored treatment
plan to meet each patient's specific
needs. By adopting this digital approach, the information provided
to patients allows them to make
more informed decisions regarding
their treatment options and gives
them greater access to ongoing
treatment support and monitoring
from their dentist throughout the
process. As digital technology continues to play an increasingly sig-

nificant role in dentistry, more patients are choosing Invisalign as
their preferred treatment option,
and more dentists are adopting
digital platforms to treat a broader
range of cases.

We are seeing both dentists
and patients opting for clear
aligner treatment to treat a wider
patient age range as well as increasingly complex cases. Can
you tell us about some of the
trends you’ve seen in this regard?

The Invisalign system has been
used by doctors to treat more than
15 million patients1, of which more
than four million2 have been teenagers. Investing heavily in innovation and the applicability and predictability of the Invisalign System
has increased the age range and
complexity of cases that can be
treated with the Invisalign System.
For example, we introduced
Invisalign First, which is dedicated
to interceptive orthodontics in
younger patients, on average between the ages of 6 and 10. This is
a target group that has not historically been treated with clear aligners. We are also seeing increasingly
complex cases being treated with
Invisalign clear aligners as the clinical capabilities of the Invisalign
System broadens.

There is increasing awareness and adoption of clear
aligner treatment, particularly the Invisalign System. Align is
actively conducting market research in the region to better
understand patient behaviour and preferences, particularly in
the growing patient market. In this research, which will be released within the coming months, we’re looking at the most
influential factors when parents are considering this treatment type for their child, and how orthodontists can best
equip both parents and the children or teens, to make decisions around their orthodontic health.
With the rise of digital technology in everyday life, we also
see the value in providing patients a similar experience when
it comes to their dental care, by enabling patients of all ages
to interact digitally throughout their treatment journey. That’s
why the Align Digital Platform suite of digital tools is enabling
patients to engage with their provider and treatment device
digitally.
As we continue to learn and evolve our understanding of
patients’ needs and preferences, we remain committed to
providing the most innovative and patient-centric digital
solutions for orthodontic treatment.

We’re seeing technology play an increasingly vital role
in healthcare, particularly here in the region, where governments are working towards digital transformation
goals. Where do you see the development of AI contributing to oral healthcare?

AI has a significant role to play in advancing oral healthcare. In today's world, AI and machine learning are helping
support the analysis and decision-making processes, leading

to continuous technological improvements, and dentistry is
no exception.
Align’s fully integrated end-to-end digital workflows include digital patient data, analysis and diagnosis, treatment
planning, orthodontic appliances, and tools that support improved doctor-patient experiences.
We have developed an integrated digital consumer and
patient Invisalign journey, from awareness, when consumers
are thinking about teeth straightening, to the evaluation
phase, when consumers can access a chatbot in addition to
the Invisalign website and Smile Concierge, SmileView and
the MyInvisalign app. This continues throughout the patient’s
consultation phase with the physician, through the use of
in-office iTero scanning, to the onboarding and Invisalign use
phase, when Invisalign patients can share and recommend
their Invisalign journey on social media.
One of the key enablers of the Align Digital Platform is our
treatment planning capabilities. Over time, as we look to reimagine the treatment planning experience, we will continue
to introduce new technologies, features, and functionality,
that improve predictability, treatment planning, utilisation of
clinical preferences, and 2D/3D imaging; including digital
tools for faster and better case set ups with more accurate
final tooth positions.
The focus on digital tools is not only helping improve
treatments but also emphasising prevention. There is a paradigm shift in healthcare from simply treating disease, to
maintaining good health to prevent disease. This change of
approach is aligned with a comprehensive dentistry approach
involving interdisciplinary diagnosis and planning. Utilising

AI, dentists can provide patients with comprehensive digital
dentistry solutions, enabling better communication, diagnosis,
and treatment planning. AI-powered tools can help diagnose
misalignment and malocclusion earlier, resulting in better oral
health outcomes.
The Align Digital Platform provides digital dentistry solutions powered by AI, cloud computing, and machine learning,
resulting in a richer, more integrated, and seamless experience
for both doctors and their patients.

What other trends do you expect to see in the dental industry in the region in the next few years?

In the next few years, we expect to see a significant shift towards a more patient-centric approach in the dental industry.
This includes a strong focus on preventative care and a greater
emphasis on the overall patient experience. The digitalisation
of the practice workflow will play a key role in facilitating this
shift. By adopting a digital ecosystem, dentists can provide
more efficient, precise, and comfortable treatments to their patients. This will involve monitoring patients remotely and empowering them to take an active role in their oral health. In addition to enhancing the patient experience, digitalisation will
also enable dentists to improve how they diagnose and communicate with patients, plan and perform more consistent
treatments, and care for more patients in shorter periods of
time. These trends are reflective of a broader movement towards improving user experiences through the implementation
of digital solutions, which are seen in various fields beyond
healthcare.

What are Align’s plans for 2023 across the region?

Align’s plans for 2023 include continuous investment in
order to increase access to innovation, and focusing on expanding geographically in the Middle East. We’re solidifying our
presence in Oman by building on last year's introduction into
the country through additional resources to support Omani
dentists, and we recently hosted the official launch event in
Muscat where we were able to showcase our commitment to
the region, welcoming over 130 orthodontists, clinic owners,
and general dentists from across the Sultanate.
We recently participated in the annual conference of the
Saudi Orthodontic Society, and we expect to continue our collaboration with doctors and the organisations that support
them whilst looking at future collaborations with other local
dental organisations. These efforts will be focused on the main
pillars of increasing access to innovation, increasing confidence
in digital dentistry and, in general, improving the patient and
dentist experience by digitising the workflow.
Align is dedicated to investing in knowledge-sharing and
the promotion of the benefits of doctor-led therapy. We are
committed to raising awareness around the importance of doctor-led care for optimum patient results and clinical outcomes
and will continue to work in collaboration with the orthodontic
and dental professional associations in the region. This commitment is achieved through collaborating with several organisations, such as the Centre for Advanced Professional Practices.

reinforce our commitment to sharing the benefits of the Invisalign System with more doctors in the market. In addition, we
have continued plans to provide peer-to-peer coaching programs, particularly for dentists new to the Invisalign System, a
tool which proves ever-valuable as we saw great success with a
recent workshop delivered in May focussing on growing dental
practices with teens and kids.
These local efforts complement our global initiatives and
are all aimed at demonstrating our dedication to supporting
dentists and orthodontists.
We continue to expand our team of talented individuals
who support dental practices and aid in better clinical outcomes, patient experiences, and practice growth. As the region
experiences change and development, we strive to remain at
the forefront and be a valued support for dental practices, helping orthodontists and dentists build their practices for the future.

“Nowadays, most of our patients
ask for laser treatment”
By Dental Tribune International
Dr Alexandra Marques is a doctoral student at the Universitat Internacional de Catalunya in Barcelona in Spain and holds a master’s
degree in endodontics and surgery/apical surgery from the same
university. She also holds a master’s degree in implantology, as
well as postgraduate diplomas in
advanced surgery and maxillofacial
osteogenic distraction and in dental aesthetics and aesthetic injectables. In this interview, Dr Marques
talks about her experience with lasers in dentistry.

For how long have you been
using a laser system in your dental practice, and how has this
changed your everyday work routine?

I’ve been using a Fotona laser in
my clinic for the past six years, and
sometimes I jokingly refer to the
time before laser and after laser. So
many procedures have been significantly improved with the introduction of a laser. Nowadays, most
of our patients ask for laser treatment and don’t want to undergo
traditional therapy when it comes
to frenectomy, gingivectomy, facial
aesthetic treatments and other
treatments.

Why did you decide to extend
your treatment portfolio to include aesthetic procedures? Are
these possible with the same
laser device that you use for dental applications?

I’ve been doing facial aesthetic
treatments for the past 15 years—
mostly with toxins and fillers, but
for some patients who wanted their
skin quality improved and for treating vascular lesions, I had no treatments to offer. That is why I decided to extend my clinic portfolio
to offer laser treatments too. I use
the same laser device for dental
treatments and facial ones. The
best thing about the Fotona laser is
that you can use only one device
for so many different applications,
and dental treatments and facial
treatments go hand in hand nowadays.

Has your dental practice
grown since introducing laser
aesthetic treatments to your
practice?
Yes, the practice has grown by
20%, mostly in oral surgery, facial
aesthetics, paediatric dentistry and
snoring treatment.

What laser aesthetic treatments do you provide for your patients, and which is your personal
favourite?

In my clinic, we offer many laser
aesthetic treatments, including for
eyelids (SmoothEye protocol), dark

spots, resurfacing, tightening, lips,
scars and facelifts (Fotona3D). My
favourite is the LipLase treatment,
which gives natural volume to the
lips without injecting anything artificial into the body but by stimulating new collagen formation instead.

Could you share an example
of one treatment you recently undertook?

I do a lot of combined treatments. Recently, I’ve been doing a
lot of SmoothEye treatments for
wrinkle reduction in the periocular
region, combined with microinjections of a multivitamin complex.
The results are beautiful and very
attractive.

What about the end results?
Is laser technology effective, patient-friendly and safe? How long

do the results last for aesthetic
procedures?

The end results with the laser
are very natural and very effective.
These are patient-friendly, non-invasive, walk-in, walk-out procedures that require no recovery
time. This makes laser aesthetic
treatments more profitable for the
dentist, as the patient turnover is
higher. The only thing is that some
laser aesthetic treatments have to
be repeated more frequently than
do treatments with fillllers or botulinum toxin, making it more expensive for the patient.

What are the benefits of laser-assisted treatments compared with using a traditional approach?

When we use a laser, we’re not
putting something artificial inside
the body like with fillers. Instead,
we work with the patient’s own col-

lagen, stimulating natural new collagen production. Another benefit
is that no recovery time is required
after laser treatment. Patients can
leave the clinic and immediately go
to dinner with friends without concern.

Was it difficult for you personally to learn how to use your
laser system, such as selecting
the appropriate parameters?
Would you say that laser technology is suitable for beginners, experienced users or both?

I went to university to obtain a
degree in laser dentistry, but that’s
not necessary. If you want to work
with a laser, you have to learn about
laser principles so that you can play
with the parameters and understand all the processes: provide the
right diagnosis, plan the treatment,
find the optimal parameters and
that is it. For each clinical case, you

have to know which laser to choose:
the Nd:YAG or the Er:YAG. This
choice is determined by the absorption characteristics of the tissue being treated. Therefore, I
think that lasers are suitable for
both beginners and experienced
users.

Let’s look into the future.
Where do you see laser aesthetic
treatment in the dentist’s office in
ten years’ time?

Every clinic that wants to be at
the top will have to use laser technology. Our patients want painfree care, non-invasive treatments,
no sutures and no recovery time.
There is only one thing that will
give you all this: the laser.

“Proper resources and evidencebased practices are crucial for providing
high-quality dental care”
By Iveta Ramonaite, Dental
Tribune International
Over the last five years, FDI has
been developing resources to assist
dental professionals in the planning
and delivery of care for partially dentate patients. At the 2023 FDI World
Dental Congress, Dr Shiamaa Shihab
Ahmed al-Mashhadani, who has
been closely involved in the development and implementation of several
projects related to improving the oral
health of partially dentate patients,
will hold a session on the topic.
During the session, she will present a
valuable assessment tool developed
by FDI that will help patients understand their treatment options and will
assist dentists in better understanding their patients’ health status and
needs.

Dr al-Mashhadani, the number of partially dentate patients is
on the rise. How does this trend
affect dentistry?
The increase in partially dentate
patients is having a significant impact
on dental care, and different facets of
oral healthcare, treatment options
and dental practice are being affected. Dental practitioners must
offer a variety of treatment choices
for replacing missing teeth. These
choices include implant-supported
dentures, fixed bridges and removable dentures. To propose the best
option for treatment, dentists must
evaluate each patient’s unique needs,
preferences and oral health status.

(Image: Freepik)

Dental practitioners also need to
educate partially dentate patients on
proper oral hygiene practices, focusing on preventing further tooth loss,
maintaining the remaining teeth and
addressing the risk factors that may
contribute to their dental problems.
There will also be a demand for specialised training for prosthodontic
and restorative treatment options.
This will trigger interest in research
and innovation in dental materials,
implant technologies and treatment
modalities.
There will be a long-term relationship between dentists and patients for follow-up care and maintenance, and there could be a rise in
public health dentistry concerned
with the care and health of partially
dentate patients.

In your session at the FDI
congress, you have presented
some resources to assist dental
professionals in the planning and
delivery of care for partially dentate patients. Could you elaborate
on that and explain how important it is that dentists use the right
resources?
Dentists need to use the best resources available to them when treating partially dentate patients. Proper
resources and evidence-based practices are crucial for providing
high-quality dental care and achieving optimal outcomes for these patients. The main goals for the planning and delivery of care for partially
dentate patients are to empower

health status.”

dental practitioners to comprehend
and support the unique needs of
these patients.
To this end, our team of experts
collaborated to shift the paradigm
from a biomedical to a patient-centred care model. To deliver quality
dental care, we have developed a
toolkit that has chairside guidelines
and offers detailed instructions on
all treatment options as well as a
three-phase treatment planning approach that covers all aspects of the
patient’s journey. The resource also
aims to increase global awareness
about the specific needs of partially
dentate patients and the treatment
options available for them. Those interested can find it freely available
on the FDI website.

assessment tool. It was developed to
aid with the evaluation and treatment of people with partial dentition, ensuring comprehensive and
patient-centred care. Specifically,
patients will benefit from a comprehensive evaluation, an individualised treatment plan, informed decision-making and improved communication with their dental health provider. Dentists will have in hand a
standardised evaluation tool that is
evidence-based and increases efficiency and accuracy in decision-making. The tool will also help
to facilitate interdisciplinary collaboration and will keep dental professionals updated on the latest developments in treating partially dentate patients.

How exactly will FDI’s assessment tool benefit partially dentate patients and dental professionals?

Editorial note: This interview was published in World Dental Daily at the 2023
FDI World Dental Congress in Sydney.

Both patients and the dental
professionals involved in their treatment will greatly benefit from this

Digitally plannedhighly aesthetic
restorations
By Atsushi Hasegawa &
Dr Yusuke Takayama, Japan
Aesthetic intuition, creativity
and technical expertise—these are
just a few relevant factors for implementing high-quality restorations.
Choosing the right material is also of
key importance for accurate fitting.
This patient case illustrates how restorations can be functionally effective and aesthetically convincing if
essential components of modern
dental technology are combined
with the expertise and experience of
both the dentist and dental technician.
Case presentation
A 37-year-old female patient
presented to Dr Takayama’s dental
practice requesting correction of
the colour and shape of her anterior
teeth and treatment of her gingivitis.
Her tooth #23 had been extracted
when she was at school, and gingival
recession was visible in this region.
When she first came to us, she had
already received a bridge from tooth
#21 to tooth #24 and crowns in regions #11 and 12. Especially the
shape of the bridge did not meet her
aesthetic demands (Figs. 1 & 2).
Planning and pretreatment
The given circumstances meant
that no labial space was available for
layering porcelain. It was thus decided to treat the patient with
monolithic restorations made of the
highly translucent and polychromatic zirconia Zolid Gen-X Multilayer (Amann Girrbach) and to undertake partial orthodontic treatment of the labially inclined and
crowded mandibular anterior teeth.

01

In addition, correction of the
gingival margin was to be carried
out before starting the restorative
treatment. Owing to the gingivitis
and missing tooth #23, the gingiva
in the region had receded considerably. This was corrected by surgery

02

in order to later achieve a natural
gingival margin (Figs. 3 & 4).
After
these
pretreatments
(Figs. 5 & 6), the teeth were prepared for restoration by the dentist
(Fig. 7).

03

The perfect fit right from the
start
The use of a facebow (Fig. 8) is
considered essential to show the individual relationship of the patient’s
condyles and occlusal plane. This
ensures subsequent realisation of
patient-friendly occlusal concepts
on the Artex articulator (both virtual
and physical, Amann Girrbach).
The classic model work used the
Model Management System workflow (Amann Girrbach). Afterwards,
the models were mounted on the
Artex CR articulator using the Artex
facebow record (Fig. 9) and digitalised using the Ceramill Map 600
scanner (Amann Girrbach). The CAD
was done using the Ceramill Mind
CAD software. One of the major advantages of the Ceramill system is
that both analogue and digital
workflows can be employed using
the same concept (Figs. 10 & 11).
The setting of the Artex articulator (condylar inclination and Bennett
angle) was average (sagittal condylar inclination of 35° and transversal
condylar inclination of 10°). For incisal inclination, a custom incisal
guide table was chosen to set at 35°,
the left wing at 0° and right wing at
30°, owing to the shallow anterior
overbite and the right canine being
natural dentition (Fig. 12).
When designing anterior restorations, it is necessary to consider
not only aesthetics but also occlusion. The goal is to achieve an occlusal scheme in which the posterior
teeth prevent excessive contact of
the anterior teeth in the maximal intercuspal position and the anterior
teeth disengage the posterior teeth
in all mandibular excursive movements. For this reason, when the an-

terior region is in occlusion, it is an
important area that guides the mandible and controls both protrusive
and lateral movements. In this case,
since one canine was a pontic, tooth
#24 was also involved to reduce the
load on the pontic and to control the
lateral movement (Figs. 13 & 14).
Virtual reality—Ceramill
M-Smile brings restorations to
life
Designing a restoration with the
Ceramill M-Smile module creates
possibilities not available to technicians in the analogue workflow. References, such as the pupillary line
and midline, previously difficult to
make out with an analogue articulator and models are now easy to see,
and it is even possible to use the patient’s smile for the design (Fig. 15).
It has to be considered that
Ceramill M-Smile uses 2D images.
Therefore, it is not advisable to use
it for determining the crown labial
contour or for the lip support design. In order to sufficiently design
these aspects, designs from provisional restorations (preoperative
scan data) are useful, as the position
of the incisal edges and the contour
of the labial aspect can be adjusted
with reference to the provisional
restorations (Figs. 16 & 17).
When considering the centreline
based on the models, the axial direction of teeth #11 and 12 suggests
setting the midline along the red
dotted line (Fig. 18). With digital design, the feeling for shape and tooth
size is lost because, unlike when
looking at a physical model, it is easy

important consideration in digital

and longevity for the restoration.

process was then started (Fig. 28).
After sintering, slight adjustment of the morphology was undertaken using the Zolid Sinter-State
Polishing Kit, including medium-grade polishing at the end
(Fig. 29).
Zolid Gen-X has a beautiful multilayer graduation and a very natural
look, so it is possible to produce restorations that require minimal staining. In this case, Vintage Art Universal stains and glazes (SHOFU) were
used. The incisal area was coloured
blue-grey for transparency. During
application, care must be taken not
to make the incisal area too blue. A
very thin layer of Shade A was added
over the body area (for colour supplementation), and the cervical area
was coloured darker than the body
area (Fig. 30). At the end, a thin layer
of glaze paste was applied over the
surface. As a second layer, glaze
paste was mixed with a small amount

of white and layered on the mesial
and distal marginal ridges (Fig. 31).
By using Zolid Gen-X, aesthetic and
functional restorations were produced (Fig. 32).
The typical cementation protocol was followed, but retraction cord
was used and extra care regarding
moisture and contamination was
taken. Especially the tting surfaces
of restorations can be contaminated
by salivary protein during the try-in
stage and have to be cleaned, for
example with KATANA Cleaner
(Kuraray Noritake Dental; Fig. 33).
PANAVIA V5 (Kuraray Noritake
Dental) resin cement was used. To
control the value, bleach shade cement was chosen. Cementation was
performed under the dental microscope (Fig. 34).

Result
In this case, the position of the
teeth limited the thickness of the
crowns and there was no space for
ceramic layering. Anterior bridges
involving canines must be strong
and aesthetic. In such cases, Zolid
Gen-X is the best material because it
offers strength and aesthetics. In addition, the use of Zolid Gen-X in
combination with Ceramill M-Smile
in the fabrication of the anterior
monolithic crowns produced a
highly aesthetic result (Fig. 35). The
patient had a beautiful smile after
cementation of the anterior monolithic zirconia restorations in her
mouth (Fig. 36).

Study evaluates whether AI can craft
a universally aesthetically pleasing
restoration
By Anisha Hall Hoppe, Dental
Tribune International
ISTANBUL, Turkey: Digital
smile design (DSD) programs have
become instrumental in dental
treatment, enabling dentists and
patients to plan treatments in harmony with the entire face. A recent
innovation in DSD is the integration
of artificial intelligence (AI). A study
in Turkey has compared aesthetic
preferences among dental professionals, dentistry students and laypeople regarding smile designs created manually and using AI, considering factors such as sex, professional experience and specialty. The
findings offer insights into the potential of AI in DSD.
For the study, four cases representing major smile design groupings were selected. They were chosen based on relationships between
the trichion, glabella, subnasale and
menton, these being crucial for
smile designs, employing the concept of “facial flow”, which refers to
the direction of facial structures.
Case 1 showed a facial flow towards
the right side, Case 2 showed a facial
flow towards the left side, the nose
and chin pointed in different directions in Case 3, and Case 4 was a
symmetrical face.
Two smile designs were created
for each case using Smile Designer

App: one via AI mode and another
manually. The app utilises the Microsoft Face API, a robust AI tool with
facial recognition capabilities. The
API identifies 68 facial landmarks essential for determining the patient’s
facial type and appropriate tooth
sizes, ensuring a precise and personalised treatment plan.
To gather perceptions on these
designs, an online survey was conducted. The 807 participants were
classified into three occupational
groups—dentists, dentistry students and other professionals (laypeople)—and were asked about
their professional knowledge, expertise in smile design and usage of
a smile design program, in the case
of dentists. They were then asked to
choose whether they found the
AI-created or the manually created
design more attractive for each
case.
The socio-demographic breakdown showed that the majority of
the dentists had 0–4 years of experience and were general practitioners. Almost half the laypeople
and over half the dentistry students
were familiar with aesthetic smile
design. Age, education and clinical
experience did not influence aesthetic preferences.
For Cases 1–3, both dentists who
used smile design programs and
those who did not favoured the

manually created designs. However,
for Case 4, dentists who used a smile
design program preferred the manually created design, whereas those
who did not preferred the AI-generated design. For Case 3, orthodontists notably favoured the AI-generated design. The authors suggested
that this might have been due to
their familiarity with AI values or experience with treatments based on
the landmarks used.
Aesthetic preferences varied
significantly between all three occupational groups for Cases 1–3, but
notably not for Case 4. The authors
suggested that dentists’ aesthetic
perception may be different from
that of laypeople in complex cases.
There were significant differences
between dentists and both dental
students and laypeople for certain
cases. The survey revealed an overall
perceptual gap between dentists
and laypeople, however, for symmetrical faces, AI-generated designs were acceptable to both dentists and laypeople, suggesting a
potential time-saving tool for clinicians in such cases.
The challenge of incorporating
AI lies in recognising that faces and
smiles are not always symmetrical.
Studies have found a link between
facial symmetry and perceived
beauty, but crucial landmarks used
in smile designs on asymmetrical

A study has found that, while the use of artificial intelligence in smile design requires further understanding of the relationship between facial landmarks and of
human aesthetic perception, it can already be used satisfactorily for symmetrical
faces. (Image: Pikisuperstar/freepik)

faces still need research for using AI.
AI relies on mathematical models to
create symmetrical smiles. However,
because facial flow takes into account human perception, it allows
for more natural smile designs for
both symmetrical and asymmetrical
faces. While AI-driven DSD works
well for symmetrical faces, manual
techniques may be better for asymmetrical cases. Given AI’s infancy in
healthcare, there is potential for

misinterpretations owing to algorithmic constraints. The authors thus
recommended ongoing research to
optimise AI in DSD and understand
aesthetic perception.
The study, titled “Evaluating the
facial esthetic outcomes of digital
smile designs generated by artificial
intelligence and dental professionals”, was published online on 6 August 2023 in Applied Sciences.

Intra-oral scans may present more humane
option for evaluating clefts in infants
By Anisha Hall Hoppe, Dental
Tribune International
ALEXANDRIA, Egypt: Clefts of
the lip and/or palate and alveolar
bone are the most common congenital anomalies of the head and
neck and result in feeding, psychological, craniofacial and speech
challenges. In infants, care may involve preoperative appliances, for
which impressions of their clefts are
required. Conventional impression
taking techniques pose risks like ingestion and suffocation. A study at
Alexandria University has assessed
the reliability of digital versus conventional impressions in reproduc-

ing unilateral cleft lip and palate in
newborns and found digital impressions to be as accurate but more acceptable for guardians.
The study involved seven infants
aged 0–28 days diagnosed with
complete unilateral cleft lip and palate. Impressions of their clefts were
taken with the conventional method
using an irreversible hydrocolloid
impression material and with an intra-oral scanner. Stone models of
the conventional impressions were
scanned, creating virtual 3D models,
and the intra-oral scans were saved
as virtual 3D models and 3D-printed.
The virtual models from both
methods were superimposed to

compare the alveolar arch width and
alveolar cleft defect. The maximum
alveolar arch width and maximum
distance between the premaxillary
segments were measured on the
physical models from both techniques using vernier callipers. The
superimposed 3D scans of the conventional and digital impressions
showed significant differences in
three of the cases. However, the calliper measurements showed no significant variation between the conventional and digital impressions.
Additionally, the infants’ guardians completed a questionnaire on
their acceptance of both impression
techniques, and their answers re-

vealed a distinct preference for the
digital method. Two significant findings were that the guardians felt that
the conventional method was more
invasive and that they believed their
infant had suffered during its application.
The study indicates a shift away
from traditional impressions owing
to associated risks and the stress it
places on both patients and guardians. Digital impressions emerged as
safer and preferred because they
minimised risks to infants as well as
eased guardians’ concerns. The
study also showed that digital impressions are accurate and efficient.
Digital impression taking also offers

the advantage of creating reliable
models for future treatment planning and provides visual aids to parents that showcase the potential improvements in their infant’s condition.
The study, titled “Diagnostic
evaluation and guardian assessment
of using digital impression in neonates versus the conventional techniques”, was published online on 30
August 2023 in the Alexandria Dental Journal, ahead of inclusion in an
issue.

New study could help reduce pain
in oral cancer patients
By Iveta Ramonaite, Dental
Tribune International
NEW YORK, US: Oral tumours
can cause agonising pain and thus
greatly impair the quality of life of patients. Researchers from the New
York University College of Dentistry
(NYU Dentistry) have recently reported that the ORAI1 calcium channel, which is a vital protein that controls calcium entry into cells, plays a
key role in oral cancer progression
and pain generation. The researchers
believe that the findings could help
develop a new approach to treating
oral cancer and reducing associated
pain in patients.
According to the researchers,
calcium channels have been shown
to contribute to cancer progression,
but very few studies to date have examined the role of ORAI1 in not only
cancer progression but also pain
causation. “Our results show that the
ORAI1 channel fuels the growth of
oral cancer tumours and produces an
abundance of molecules that, once
secreted, interact with neurons resulting in an increased sensitivity to
pain,” lead author Dr Ga-Yeon Son, a
postdoctoral fellow in the Department of Molecular Pathobiology at
NYU Dentistry, said in a press release.
The name “ORAI” pays homage
to the Horae sisters who, according
to Greek mythology, guarded the
gates of heaven at Mount Olympus.
Similarly, the researchers view the
ORAI1 channel as the guardian of calcium entering the cells.
“These calcium channels can be a
source of good or bad for cells,” said
senior author Dr Rodrigo S. Lacruz,
professor of molecular pathobiology
at the university. “Calcium entering
cells is necessary for many good
things, but too much calcium for a
long time has the opposite effect,” he
explained.

mours progressed more slowly and
were less painful, which caused them
to wonder whether the MMP-1 was
the messenger relaying pain. They
later discovered that the MMP-1-rich

fluid surrounding oral cancer cells
with ORAI1 induced an increase in action potentials, which are essential
for transmitting pain signals.

“This gives us evidence that an
abundance of MMP-1 may generate
increased sensitivity to pain,” Prof.
Lacruz explained.

Further experiments also showed
that ORAI1 gene overexpression in
non-invasive cells transformed the
cells into invasive ones. Based on the
findings, the researchers will now assess a novel drug delivery method
that seeks to block the ORAI1 gene in
order to test whether it can help prevent oral cancer progression and
pain in patients.
The study, titled “The Ca2+ channel ORAI1 is a regulator of oral cancer
growth and nociceptive pain”, was
published on 5 September 2023 in
Science Signaling.

In the study, the researchers analysed tissue samples obtained from
both human oral cancer tumours and
healthy tongues. They observed a
significant upregulation of the ORAI1
gene, which is responsible for encoding the ORAI1 calcium channel, in the
tumour samples. However, ORAI1
gene overexpression was not observed in the healthy tissue.
The researchers then examined
human oral cancer cells and discovered that the activation of the ORAI1
channel, as opposed to other calcium
channels, triggered a substantial influx of calcium into the cancer cells.
This influx, in turn, led to an elevation
in the levels of a calcium-dependent
enzyme known as matrix metalloproteinase-1 (MMP-1), which is secreted from the cancer cells and is
critically involved in carcinogenesis.
After removing the ORAI1 gene
from oral cancer cells in animal studies, the researchers noticed that tu-

“We are thinking beyond dentistry”:
Study helps clinicians identify Sjögren’s
syndrome
By Iveta Ramonaite, Dental
Tribune International
Indianapolis, US: Sjögren’s
syndrome is a chronic autoimmune
disease that can affect the entire
body including the teeth. Since diagnosing the disease can be challenging owing to its numerous
symptoms which often overlap with
those of other conditions, a recent
study has linked the electronic
health and dental records of patients with Sjögren’s syndrome. The
study may help provide better care

and outcomes for individuals with
the condition and may have implications for other systemic autoimmune diseases.
According to information in the
National Library of Medicine, between 400,000 and 3.1 million adults
have Sjögren’s syndrome. The condition is frequently associated with
other autoimmune disorders, including rheumatoid arthritis and
systemic lupus erythematosus.
Since there is no single test for
Sjögren’s syndrome, doctors typically ask about symptoms such as

dry mouth, dry eyes, dental caries
and fatigue and ask the patients to
undergo a series of tests. According
to the Sjögren’s Foundation, it takes
approximately three years to diagnose the disease from the time the
symptoms first appear, which is a regrettably long time to allow the disease to progress without treatment.
Since dental and medical care
records are typically kept separate
or medical records are not readily
made available to dental providers,
dental professionals often have no
information on a patient’s medical
Through a new study, researchers in the US have sought to improve the treatment
and outcomes of patients with Sjögren’s syndrome by linking their electronic dental and health records. (Image: charnsitr/Shutterstock)

background, including the presence
of any diseases, and depend on their
patients to communicate their diagnosis. Consequently, dentists usually
identify the patient’s condition at a
late stage in the progression of the
disease, often coinciding with the
appearance of caries.
Using data from the Indiana
Network for Patient Care, researchers at the Regenstrief Institute and
the Indiana University School of
Dentistry have recently linked electronic health and dental records of
individuals with Sjögren’s syndrome.
They found that fewer than onethird of the patients who were diagnosed by their physicians had informed their dental providers about
the diagnosis, thus depriving their
dental care providers of the chance
to provide early treatment that
could help preserve teeth.
Senior author Dr Thankam P.
Thyvalikakath, the founding director
of dental informatics at the Indiana
University School of Dentistry and
the Regenstrief Institute, explained
that the motivation for undertaking
the research stems from the significant oral health challenges faced by
individuals with Sjögren’s syndrome,
which often lead to premature tooth
loss during the middle-age years.
“In spite of the fact that patients
with Sjögren’s syndrome typically
have a high level of awareness regarding their health, because they
don’t inform their dentist, they end
up losing their teeth, which can have
a huge impact on their quality of life.
They may be unable to hold fulltime jobs because they have a lot of
dental decay and have missing or
damaged teeth. Without the information from the patient’s electronic
medical record, dentists don’t have
a complete picture and don’t know
[how] to evaluate treatment which
could preserve teeth,” she explained.
“We believe the methods we developed in this study and our results
can lead to more large-scale studies
to understand the disease over time,

which may have the potential to topple silos and for earlier diagnosis
than the current three-year lag and
for earlier treatment,” she added.
“We are thinking beyond dentistry.
We are thinking about an inclusive
healthcare system, not one in which
dentistry, like mental health, is considered a stepchild.”
“Dentists see many patients, especially women, with non-specific
symptoms, like joint pain and fatigue, but it’s very difficult for a dental professional to know about their
patient’s complete list of medical
conditions,” said lead author Dr
Grace Gomez Felix Gomez, assistant
research professor at the Indiana
University School of Dentistry. She
suggested that, if the patient who
has not been diagnosed with
Sjögren’s mentioned symptoms
such as dry mouth, the dental professional could do some preliminary
salivary flow tests to help identify
the cause. If the patient has Sjögren’s
syndrome, the dental professional
could then refer the patient to a
rheumatologist or other medical
specialist to rule out underlying conditions, she suggested, and refer the
patient to oral medicine or oral pathology specialists to further confirm the diagnosis.
“Unfortunately, Medicare and
many health insurances do not pay
for tests and treatments performed
in dental offices, which makes working together with physicians more
difficult for oral health providers.
Screening and awareness of
Sjögren’s and its symptoms should
be spread through educational programmes targeting women to encourage them to talk to both medical and dental clinicians,” Dr Gomez
commented.
The study, titled “Characterizing
clinical findings of Sjögren’s disease
patients in community practices
using matched electronic dental-health record data”, was published on 31 July 2023 in PLOS One.

Academic review of CAD/CAM applications
within dentistry
By Dental Tribune International
One thing is certain: technology
is always changing. Whether a dental clinician is experienced in CAD/
CAM or is a novice in digital dentistry, it is important to stay abreast
of new developments. To help dental practitioners navigate the myriad
of ever-evolving terms developed to
describe CAD/CAM, including 3D
printing, within dentistry, researchers at the Department of Oral Medicine and Radiology at the Datta
Meghe Institute of Higher Education
and Research in the Indian state of
Maharashtra have released a helpful
reference guide.
Understanding innovation within
CAD/CAM is only possible with an
overview of the fundamentals. This
is particularly relevant because the
majority of currently practicing clinicians did not receive any formal education on CAD/CAM, 3D printing,
materials science or engineering, all
of which are now fundamental areas
of knowledge in the practice of dentistry.
The authors ease clinicians into
the technologies with an overview
of the three broader approaches to
CAD/CAM. Direct CAD/CAM enables restoration in a single visit, and

all processing stages occur on-site.
For semi-direct CAD/CAM, at least
one step has to be performed externally. The practitioner obtains the
data for the CAD, CAD is performed
either in-house or by an outside laboratory, and the requisite prosthesis
is manufactured externally. In indirect CAD/CAM, the clinician creates
a physical impression that is sent to
an external production facility, leaving no stage of design or production
in-house.
There are possibilities for application in all dental specialties, including restorative dentistry, prosthodontics, oral surgery, and orthodontics. For example, CAD/CAM can
be used for the design and fabrication of inlays; onlays; prostheses;
prosthetic bases; frameworks and
bars; veneers; surgical guides; indirect bonding trays; and models for
various uses, including for fabrication of clear aligners. In fact, there
are increasingly fewer procedures
that cannot be carried out with the
aid of some form of digital technology.
The benefits of choosing to learn
more about and invest in CAD/CAM
processes are significant, according
to the researchers. Aside from increased profitability and productiv-

Though daunting from the outside, it is not dif cult to learn the basics of CAD/CAM applications for dentistry, and with these
technologies, clinicians can advance the care they provide and grow their practices. (Image: olguit/freepik)

ity, a clinician has access to a much
broader range of treatment options,
improving the patient experience
and saving chair time. Additionally,
for patients, involving them in their
treatment through computer-gen-

erated visualizations of their treatment plan and envisaged outcome—as opposed to being passive
recipients of treatment—boosts
treatment acceptance.

The reference guide, titled “Application of CAD/CAM technology in
dentistry,” was published in the October 2022 issue of the Journal of Research in Medical and Dental Science.

Tracking periodontal disease with
electronic dental records may enhance
diagnosis and treatment
By Franziska Beier, Dental
Tribune International
INDIANAPOLIS, US: Despite
advances in periodontal disease research and treatments, it remains a
growing health issue in the US. To
address this topic, researchers from
Regenstrief Institute and the Indiana
University School of Dentistry in Indianapolis have developed algorithms to track periodontal disease
changes through electronic dental
records. This method could help
dental professionals follow disease
progression and diagnose the disease early—when it is potentially
still reversable—and thereby reduce
the risk of other systemic diseases
associated with periodontal disease.
For their study, the researchers
used data on 28,908 patients who
had received a comprehensive oral
evaluation at the dental school’s

clinics between 2009 and 2014. They
developed two algorithms to extract
periodontal disease-related information from the patients’ electronic
dental records and to classify them
into three groups—patients with
disease progression, patients with
disease improvement and patients
with no disease change. The algorithms were applied to the 15 years
of electronic dental record data to
generate the final patient cohorts.
Both algorithms showed a high accuracy of 98%, and they have been
made publicly available for use by
other researchers.
“Gum disease, which is typically
underdiagnosed, is reversible if
caught at an early stage before it has
affected underlying structures and
adversely impacted tooth support.
Enabling dentists to track the disease using both the information in
clinical notes and the periodontal

charting data contained in a patient’s electronic dental record can
enable diagnosis and hope,” said
co-author Dr Thankam Thyvalikakath, head of the institutions’ joint
Dental Informatics programme, in a
press release.
She added: “We are here to develop and establish a culture of documenting and diagnosing cases in a
structured manner as is done in
medicine.”
The high usage of electronic
dental record systems to document
patient care information provides a
significant opportunity to study the
clinical course of periodontal disease and the influence of risk factors. “I think the advantage of our
approaches is that, using routinely
collected data, we can automate
and monitor gum disease treatments and changes that are visible
only clinically, so we can catch gum

disease at an early, potentially reversible, stage. This contrasts with
other approaches that leverage only
radiographs, which only show advanced gum disease,” said Dr Thyvalikakath.
The authors concluded that
their study demonstrated the viability of using longitudinal electronic
dental record data to track periodontal disease changes and that
their algorithms were successful in
classifying the three different patient
cohorts using the data. This approach can be used to study the
clinical course of periodontal disease using artificial intelligence, including machine learning methods.
In addition, Dr Thyvalikakath
commented on the importance of
tracking periodontal disease for an
interdisciplinary treatment approach: “There is a bidirectional relationship between certain risk fac-

tors and gum disease. For example,
having diabetes increases risk of
periodontal disease and having
periodontal disease negatively affects the course of diabetes. A similar relationship exists between cardiovascular disease and periodontal
disease. Recognising, monitoring,
and treating gum disease is an important part of overall patient
health.”
The study, titled “Developing
automated computer algorithms to
track periodontal disease change
from longitudinal electronic dental
records”, was published on 8 March
2023 in the special issue Advances in
Biomedical and Dental Diagnostics
Using Artificial Intelligence of Diagnostics.

CAPP and HuFriedyGroup collaborate
to help dentists deliver the best
care to patients
By CAPP Events & Training
An American Dental Association
Continuing Education Recognition
Program recognised provider, the
Centre for Advanced Professional
Practices (CAPP) delivers excellence
in continuing dental education at its
training institute in Dubai, as well as
online. It is one of the largest continuing medical education providers
in the Middle East.
The CAPP Training Institute can
accommodate up to 24 delegates
and has world-class facilities
equipped with phantom head stations, microscopes, high- and lowspeed handpieces supported by
water and air compressors, and the
latest TV and audio-visual technologies.
Since 2019, it has been partnering with HuFriedyGroup to provide
the optimal instrument solutions for
its hands-on training courses and
post-graduate programmes, establishing a long-lasting and satisfying
relationship for all parties involved
and, most important, for trainees,
who can improve their skills and
learn new techniques with top-class
instrumentation. In this manner, in
3 years, 21,600 dental professionals

Delegate using the implant dentistry kit from HuFriedyGroup (Photo: CAPP Events & Training)

have gained experience by using HuFriedyGroup
instrument kits, giving them confidence in treatment to achieve a predictable aesthetic and functional result in endodontic, restorative and implant
dentistry.
Each kit includes all the necessary instruments
by procedure, allowing practitioners to realize the
entire clinical technique from beginning to end,
thus facilitating its organization before, during and
after the dental treatment. This concept gets its
best results when applied to dental clinics and offices: each procedure has its specific kit, with everything the dentist needs in one single place and arriving altogether to the chairside.
For example, the endodontic kit used in CAPP
Training Institute consists of all instruments needed
for endodontic surgical and non-surgical retreatments: an excavator, a general multi-purpose apical
explorer, a retro-filling instrument , a periosteal elevator and a tissue pliers. This kit is broadly used
during the CAPP’s “Clinical Endodontics PG Diploma” programme. In May 2023, participants will
be using the kit in Prof James Prichard’s two-day
hands-on course “Enhance your expertise in endo—
the 3Dimensional revolution” as well as the “Primary
treatment and retreatment in endodontics: the
smooth access to the apex” course with Dr Marc Kaloustian.
The restorative dentistry kit allows trainees to
practice on every kind of composite restorations,
from anteriors to posteriors, and is used in all
courses and post-graduate programmes related to
prosthodontics, restorative dentistry, and aesthetic
dentistry. The programme “Restorative & Aesthetic
PG Diploma” is where the kit is used throughout the
entire course.
Finally, the extensive implant dentistry kit features in CAPP’s implant and surgical courses, including those covering periodontics, extraction and

management of impacted teeth. It consists of a series of surgical and diagnostic instruments that allows trainees to improve their abilities on suturing,
socket debridement, extractions, and soft-tissue
management.
In June, participants will be using the kit in Dr
Maurizio Martini’s one-day hands-on courses “Extractions and suturing techniques. Tips and tricks in
the daily dental practice” and “Impacted wisdom
teeth: A–Z management and easy extraction technique”.
To experience the HuFriedyGroup instruments
for themselves, readers can find a list of courses in
which to enrol at www.cappmea.com/events.
