On 27 April, four hands-on

Dr. Rajiv Rajipal, a New Zealand-based specialist oral surgeon,
will delve into 'GBR: Hard and Soft
Tissue Management—Immediate
Implants.'
Dr. Brijesh Patel from the United
States will present the opportunity
to explore the latest techniques
and materials for the minimally invasive treatment for atrophic maxillary sinus. Notably, he will offer
advice on how to use CBCT to help
plan and predict treatment for
sinus membrane lifting.
Dr. Nicolas Widmer from Switzerland will touch upon an intriguing topic, 'ONE CAS—ONE LOVE:
Why every implantologist falls in

I need that shade of composite it’s
been a year and a half since I’ve
used it and it’s gone bad, it’s stiff,
it’s hard. Now I’ve cost myself
money doing that filling. So, one
shade for everything simplifies
your life, streamlines things, reduces inventory… it just makes life
easier.”
“And I know some of these
other one shade systems advertise
‘one shade for all colors’ or whatever they say. Well, that’s not exactly true if I need a blocker, because then it’s the blocker plus one
shade. I’m not a math expert, but
that’s two. When Transcend says
it’s only one shade, it really is, it’s
not a blocker and a shade, it’s just
one."

nicians can complete most restorations with just one shade of composite without a blocker shade. “For
me personally, 85, 90, maybe even
95 percent of what I do, I can get by
using [Transcend’s] Universal Body
shade,” Findley says. “I’m not doing
these super complex restorative
cases, I’m just filling holes,” he adds

with a chuckle. “I’m just doing basic
restorative dentistry, that’s how I
earn my living. Many dentists are
out there using A2 as their universal
shade. For those people, just use
Transcend UB (universal body
shade) and it will be like using A2
but better. It will make your work
look better, and good luck distinguishing it from the natural tooth
color.”

Naturally, Dr. Findley does encounter more complex cases where
he isn’t “just filling holes” and feels
a layering technique is appropriate;
the complete Transcend kit allows
him to do so. “When ordering the
complete kit, I also get some dentin
shades and some enamel shades,
so if I feel compelled to use the layering technique, I have that option.”
Handling is an important factor
to consider when selecting a composite and the viscosity of Transcend composite has been finely
tuned to be malleable so that it
doesn’t crack during manipulation.
The result is a composite that
moves and sculpts the way you
want it to and stays in place without
drooping or slumping. “Transcend
is a softer composite, and I like
that,” Findley says. “It flows, or
adapts, really well to irregular
preparations.”
Transcend composite is created
using a blend of resin monomers
and filler particle sizes for sculptability while maintaining a strong
and durable nature. “If you’re looking at the chewing surfaces of a
tooth, you have bumps, valleys, and
little lines going from one to the
other. So as a dentist, we try to tap
[the composite] in there and mimic
some of that natural structure because we want a finished tooth to
still look like a tooth—we try to

re-create the grooves and the
humps that teeth naturally have,”
Findley says. “Some composites
don’t hold their shape well, so you’ll
tap the composite into the tooth
and before you can reach for the
curing light to cure it, [the composite] is just kind of ‘slumped’ in there,
it doesn’t hold its shape [...] It’s kind
of like just trying to sculpt pudding,
and you lose all your work before
you can cure it. Transcend is softer,
but it still maintains its shape, its
sculptability.”

ChatGPT 4.0 passes dental licensing
examinations

The omnipresence of artificial intelligence (AI) in today’s society has brought
with it numerous transformations in work
dynamics. Further exploring its potential
in healthcare settings, a recent study has
examined the performance of generative
AI in dental licensing examinations. It
found that the newer version of ChatGPT
could answer questions from dental licensing examinations proficiently, thus
demonstrating a great potential to change
the way dental professionals are trained in
the future and even to improve the communication between dentists and patients.
“Generative AI, such as ChatGPT and
other chatbots, has recently become a hot
topic. Despite being marketed as an AI
that can write text and create stories, messages and programming code, for example, users may also consult it about their
own medical and dental conditions. This is
similar to the 1990s when the internet became available to the public and people
started obtaining information from search
engines,” senior author Dr Walter Yu-Hang
Lam, a clinical assistant professor in prosthodontics at the University of Hong Kong,
told Dental Tribune International.
“When using chatbots, there is no longer a need to know the correct keyword
that we usually have to input into search
engines. Instead, we can simply type our

questions directly and the chatbots will
help us interpret the question. Furthermore, the chatbots will summarise the information available in their web database
to provide concise answers, and users are
not required to interpret a variety of information from the web,” he continued.
ChatGPT versus dentists
According to Dr Lam, the use of generative AI is expected to increase in the future. However, since the accuracy of data
provided by chatbots remains unknown,
he and his fellow researchers sought to fill
this gap in the literature by investigating
the reliability of the information provided
by ChatGPT. To assess how proficient language learning models are in interpreting
written input and providing accurate answers in dentistry, the researchers posed
1,461 multiple-choice questions from the
US and the UK dental licensing examinations to two versions of ChatGPT—
ChatGPT 3.5 and 4.0.
The earlier version correctly answered
68.3% of the questions from the US dental
licensing examination and 43.3% from the
UK counterpart, failing to meet the pass
criteria for both. In contrast, ChatGPT 4.0
showed a marked improvement, correctly
answering 80.7% of the US and 62.7% of
the UK dental licensing examination questions, successfully passing both tests.
“It is truly remarkable that a general-purpose chatbot can excel in a specialised examination. With further advance-

ments in AI algorithms and the expansion
of its database, it is anticipated that the
accuracy will continue to improve,” Dr Lam
noted.
AI as an educational resource in
dentistry
A direct comparison between the two
versions found that ChatGPT 4.0 answered
327 more questions correctly and had 102
more incorrect answers than its predecessor. These results underscore the advancements in AI capabilities in the newest version, highlighting its potential as an educational tool and resource in the field of
dentistry.
Dr Lam believes that, since chatbots
can be utilised by both the general public
and dental professionals to provide dental
information, this could potentially shift the
landscape of dental education and alter
the skill set and competencies that dentists need to develop. It could also affect
the dentist–patient relationship, since it
would eventually lead to a reduction in
time spent with patients.
However, he thinks that the integration
of AI in various aspects of dental services
could enhance the quality of care provided
by dental professionals. “Dentists can delegate certain aspects of patient education
and management to chatbots, allowing
them to focus more on delivering personalised preventive treatments to individual
patients,” he commented. To make use of
these to support the provision of

high-quality care, dental professionals will need to
allocate a portion of their time and resources to
adapting to the current technological advancements.
The study, titled “Performance of generative artificial intelligence in dental licensing examinations”, was published
online on 19 January 2024 in the International Dental Journal, ahead of inclusion in an issue.

FDI sends unifying
video message in
celebration of World
Oral Health Day

“A happy mouth is a happy body,”
FDI World Dental Federation said on
World Oral Health Day (WOHD). Celebrated annually on 20 March, WOHD
aims to advance oral health globally
through the sharing of knowledge as a
tool of empowerment. FDI’s message
for this year is shared in a video that
brings together voices from governments, the private sector and advocacy
organisations for chronic health conditions. The initiative underscores the urgent need for collective action to ensure universal access to oral healthcare
and to support systemic health worldwide.
“We say that a happy mouth is essential for a happy body because the
two are intricately connected and mutually influence each other. Understanding this connection is vital to improve the quality of life for countless
people around the world,” explained
FDI President Dr Greg Chadwick.
Moreover, maintaining good oral
health may contribute to preventing
other chronic health conditions, such
as heart and respiratory disease, cancer
and diabetes. Regrettably, oral diseases rank as the most prevalent health
conditions globally, affecting nearly 3.5
billion people, according to World
Health Organization estimates. Given
this unacceptably heavy burden, it is
crucial to raise awareness through
WOHD about the importance of brushing with a fluoride toothpaste, having
regular dental check-ups and avoiding
risk factors such as smoking and consuming foods high in sugars. However,
“no single entity can effectively tackle
oral health issues alone,” emphasised
Dr Anna Lella, WOHD Task Team chair.
“This is why FDI has made it its mission
to catalyse partnerships, networks,

policies and programmes to advance
oral health as an essential component
of overall health and well-being,” she
explained.
By working with organisations like
the NCD Alliance, World Heart Federation, International Diabetes Federation
and International Pharmaceutical Federation, FDI advocates for collaboration across health conditions and professions to safeguard people’s health.
By mobilising its members and partnering with industry leaders like WOHD

strives to shape the future of oral health

Conservative aesthetic management
of white spot fluorosis lesions

Introduction
Dental fluorosis is an enamel
anomaly that adversely affects inorganic phase deposition and organisation, causing enamel hypomineralisation.1 Despite the essential effect of fluoride in remineralisation of dental hard layers when
used topically, a direct relationship
has been established between the
frequency and quantity of fluoride
ingestion during tooth development and the severity of fluorosis. 2
The severity of enamel fluorosis
varies and may affect deeper layers
of enamel, causing excessive porosity that renders the enamel
fragile and prone to fracture once
the tooth emerges into the oral
cavity. 3 Clinically, this disease is
manifested by white, yellow or dark
brown stains within enamel layers
that may be pitted.4
Histologically, enamel scaffold
proteins, such as amelogenin, are
routinely secreted by ameloblasts
and deposited within immature
enamel during the secretion stage.
As the maturation stage begins,
these proteins are degraded by
protease activity and spatially replaced by hydroxyapatites. In the
case of fluorosis, an increased level
of fluoride ions (instead of calcium
ions) is detrimental to protease activity, leading to protein and water
molecule retention within the
enamel layers; as a result, the affected enamel is low in mineral
content and is porous.4
Several treatment modalities
have been suggested for treating
fluorosis enamel discoloration,
ranging from ultra-conservative
tooth bleaching to invasive complete crowning of the affected
teeth.4 Resin infiltration, enamel
macro- and micro-abrasion, and
facial veneers are other options for
correcting shade discrepancy.5
Choosing the appropriate treatment depends greatly on the size
and depth of the lesion, the clinician’s experience and the patient’s
choice. In mild cases, bleaching
alone may provide satisfactory
aesthetic results, as the discoloured spots will blend better with
the surrounding healthy enamel.5
Alternatively, mild to moderate
cases may be approached by
bleaching treatment followed by
resin infiltration, enamel macro- or
micro-abrasion, resin or porcelain
veneers, or a combination treatment. Nevertheless, deeper lesions
are more challenging and often require more invasive procedures,
such as complete crown coverage.4
Dental bleaching is routinely
done in office or at home, both
achieving similar long-term suc-

cess.6 Resin infiltration is a technique that utilises a low-viscosity,
polymerisable resin after the external enamel surface has been
treated with 15% hydrochloric acid.
A single application of 15% hydrochloric acid to enamel surfaces can
dissolve and remove 37–58 µm of
superficial enamel, rendering the
deeper fluorosis lesion more accessible by the infiltrate. This novel
technique was initially introduced
as a conservative approach to arresting incipient carious lesions.5
Over time, clinical evidence has
emerged that suggests its ability to
mask opacities associated with hypomineralised enamel lesions.7 In
theory, treating white spot lesions
with resin infiltration obliterates
enamel pores with a material (i.e.
resin) that has a refractive index
closer to that of healthy enamel
(RI = 1.62; resin RI = 1.42) than air
(RI = 1.00) and water (RI = 1.33). In
contrast to resin infiltration, microabrasion utilises an acidic slurry of

hydrochloric acid and pumice that
is actively brought into contact
with affected enamel using a rubber cup rotating at a slow speed. A
recent systematic review concluded that resin infiltration has the
highest effectiveness in treating
mild to moderate lesions, followed
by bleaching and then microabrasion. Macro-abrasion simply
utilises a dental bur to eliminate the
affected hypomineralised area,
which is subsequently restored
with a restorative material. This
case study aimed to visually evaluate the effectiveness of home
bleaching followed by resin infiltration in resolving the aesthetic concerns of a patient with fluorosis-affected central incisors.
Case presentation
A 27-year-old female patient
attended the Operative Dentistry
Clinic at the University of Iowa
College of Dentistry and Dental
Clinics complaining of white spot

enamel opacities on her maxillary
anterior teeth, mainly the central
incisors (Fig. 1a). There was no discomfort or history of sensitivity.
Other family members had similar
white spot complaints, according
to the patient. Her local dentist
suggested that her condition was
most likely due to the high fluoride
content of the drinking water. Periapical radiographs were within
normal limits, and vitality testing
(liquid carbon dioxide) revealed a
normal response. Conservative
treatment options to correct shade
discrepancy included resin infiltration, enamel macro- and micro-abrasion, and/or porcelain veneers.
Tooth bleaching was suggested
to reduce colour discrepancy and
enhance the blending effect between the lesion body and the surrounding enamel. The patient
opted for home bleaching using
10% carbamide peroxide with potassium nitrate and fluoride (Opal-

escence PF 10%, Ultradent).
Custom-made trays and home instructions were also provided at
the same visit. The patient was
asked to discontinue bleaching two
weeks prior to her upcoming infiltration treatment appointment. According to a review, a waiting period of one to three weeks is recommended prior to resin composite bonding to reverse the
detrimental effect of any remaining
oxygen on resin polymerisation
and bonding.8
Six weeks after the initial appointment, the patient returned
with visually whiter teeth, but the
lesions were still noticeable
(Figs. 1b & 2). To determine the
most conservative and effective
technique to address her aesthetic
concerns, coloured filters were attached to a curing light tip, the light
was projected from the lingual aspect and the colour changes on the
facial surface were observed
(Fig. 3). This procedure can assist

the clinician in estimating the depth
of the lesion; the deeper the lesion,
the greater the light blockage. For
deeper lesions, a combination of
various treatment means may be
more appropriate. In this patient’s
case, the right central incisor lesion
was less extensive than the left
central incisor lesion, and thus, a
better outcome was anticipated.
Resin infiltration was selected to
treat both lesions.
A dental dam with tight floss ligation was placed (Fig. 4a) to assist
in isolation, prevent saliva contamination and protect the soft tissue
from the hydrochloric acid. Both
central incisors were polished with
a non-fluoride polishing paste and
a rubber cup. The resin infiltration
treatment was performed according to the manufacturer’s instructions. Icon-Etch (15% hydrochloric
acid; DMG) was applied passively to
the affected areas and periodically
massaged for 2 minutes. The surfaces were then wiped with a cotton pellet to remove the acid gel,
and the residue was rinsed off with
copious water irrigation for 30 seconds. The surfaces were then thoroughly dried with a continuous
stream of oil-free and water-free
air.
Icon-Dry (99% ethanol; DMG)
was applied to and left on the surface to slowly evaporate, facilitating the elimination of water molecules located within the enamel
porosities. Shade improvement
was observed, but the process was
repeated to further enhance the result. After the second Icon-Dry application, a substantial shade enhancement was observed clinically,
and the TEGDMA-based IconInfiltrant (DMG) was applied to the
etched and dried surfaces and periodically massaged to enhance
penetration for 3 minutes. Excess
material was removed with a cotton pellet and dental floss before
light polymerising at a minimum
intensity of 800mW/cm2 for 40 seconds. The infiltration process was

then repeated with only 1 minute
of application time. Finishing cups
and polishing discs were utilised to
smooth the surfaces (Fig. 4b). The
dental dam was removed, and the
patient asked to assess her outcome. She was happy with the result and the shade modification
(Fig. 5a).
At the 14-month follow-up appointment, the patient expressed
her great satisfaction with the result and agreed that no further
treatment was needed (Fig. 5b).
Conclusion
Dental fluorosis is an increasingly prevalent phenomenon. According to a Centers for Disease
Control and Prevention website review paper,9 the prevalence of fluorosis in 2011/2012 has doubled
from 30% in 2001/2002.10 This rapid
increase in fluorosis cases, accom-

panied by growing public awareness and aesthetic demands, requires dental care providers to be
creative in offering the most conservative treatment modalities,
such as tooth bleaching and resin
infiltration. A combination of home
bleaching and resin infiltration was
used in this case to successfully resolve anterior white spot fluorosis.
The 14-month follow-up indicated
both colour stability and patient
satisfaction.

Diastema closure using a direct
two-matrix technique

The patient presented with
spaces between teeth #11 and 21
and between teeth #12 and 11.
During the diagnostic phase, a digital screening of the smile line was
carried out. The treatment involved
the closure of the anterior spaces
with a direct technique using a nano-hybrid composite. The operative steps to close the gap between
teeth #11 and 21 provide photographic guidelines on a direct technique presented here with the
name: “two-matrix technique” for
diastema closure.
Case presentation
Clinical history
The patient came to the initial
dental consultation for an aesthetic
assessment. The patient was in
good health without any systemic
conditions and had never undergone any orthodontic treatment.
The treatment steps necessary to
improve the aesthetics were explained to the patient after a careful diagnosis. The patient desired
complete closure of the anterior
spaces for aesthetic purposes.
Record taking and diagnosis
On clinical examination, there
were no signs of structural impairment related to previous treatments. On radiographic examination of the crowns, pulps, roots and
periodontium, there were no findings. The diagnosis was a diastema
between teeth #11 and 21 and minimal gap between teeth #12 and 11.
Treatment plan
The adhesive technique consisted of the two-matrix technique
for diastema closure between teeth
#11 and 21 and the transparentmatrix technique for closure of the
space between teeth #12 and 11. A
nano-hybrid composite, in combination with a universal adhesive,
was used as the material for the
gap closures. For spaces <1.5mm, it
is recommended to restore freehand (like in this case), whereas for
gaps >1.5mm, it is advisable to use
a silicone key to achieve proportional tooth widths and an aesthetic result (Fig. 1).
The Digital Smile Design (DSD)
visual technology (DSD Planning
Center) for a thorough analysis
based on the proportions between
the teeth was used. The patient was
shown the new geometric lines of
the planned restorations.
We followed a thorough analysis of the chromatic spectrum, as
described by Naorungroj, to accurately and precisely establish the
shades to be used in the operative
steps (Fig. 2).1 The photographic
material was carefully analysed to

determine the ideal work planes to
gain the information required to
achieve the ultimate aesthetic aim
of the treatment.
In order to avoid a negative
gingival influence and an unaesthetic open gingival embrasure, the
complete papillary filling of the interdental area was set as a goal.
Among other concerns, black triangles are a plaque and food trap. 2,3

<5mm (Fig. 3).4
Timeline of treatment steps
Having completed the assessment steps, it was possible to proceed with the operative steps for
the aesthetic and functional management of the factors affecting
the smile line.5,6 As a first step, a
detal dam was positioned (Fig. 4).7
Conditioning of the enamel and
bonding then followed. Acid etching (Vococid, VOCO) of the interproximal surfaces of teeth #11 and
21 as well as of the mesial surface
of tooth #12 was performed for
20 seconds (Fig. 5). The etching
agent was then aspirated and
rinsed off with water for 15–30
seconds and the surfaces dried
with a gentle air stream to obtain a
chalky white opaque appearance
(Fig. 6). The universal bonding
agent (Futurabond U, VOCO) was
applied to and rubbed on the conditioned enamel surfaces (Fig. 7),
and the solvent was carefully evaporated using compressed dry air
for at least 5 seconds so that the
layer of adhesive became thin, immobile and shiny. The bonding
agent was then light polymerised
for 10 seconds with a high-power
LED lamp (Celalux 3, VOCO).
Reconstruction of the contact
point mesial to tooth #12 and distal to tooth #11
A wedge and matrix system
(Ena Matrix, Micerium) was inserted. To ensure good visual control and proper polymerisation on
tooth #12, a transparent matrix was

chosen (Fig. 8). After the application of an initial layer of packable
composite, a nano-hybrid very
low-viscosity flowable composite
(GrandioSO Light Flow, VOCO) was
used to ensure proper sealing of
the conditioned enamel (Fig. 9).
Closure of the diastema between teeth #11 and 21
The operative steps chosen to
close the space betweencteeth
#11 and 21 were carried out using a
technique we adapted involving
the simultaneous use of two
appropriate sectional matrices
(Fig. 10) applied sequentially as described next. This was done drawing inspiration from scientific work
aiming to restore, in a single clinical
session, the natural smile line of patients with periodontal disease8 or
to manage the matrices in a versatile manner.9
First of all, the first metal sectional matrix (thickness of 60 μm)
was positioned vertically and fixed
with a wedge (Ena Matrix). To obtain a clinical preview of the contact
point of the central incisors,10 we
then added a second matrix, again
in a vertical position, with the intention of defining the emergence
profile when performing the cervical restoration. It acts passively on
the wedge and pushes it against
the interdental papilla. Having stabilised the wedge and matrix system, the first layer of sculptable
composite (Admira Fusion, Shade
A2; VOCO) was applied to the mesiovestibular wall of tooth #11
using the median landmark created
by the physical contact generated
between the two matrices. Not too
much material should be applied in
the vestibular-palatal direction.
This system offers the advantage
that, once a matrix has been removed, the special wedge can be
left in place for haemostatic purposes ready for the insertion of another matrix of the same system.

from the wall of tooth #11 while

maintaining
the
previously
achieved anatomical limits (Fig. 11).
We then applied a composite increment to the mesial wall of tooth
#21, as here the matrix was still in
place.11

Transition lines were drawn on
the vestibular surfaces of teeth
#12–21 using a pencil (Fig. 12) to
provide guidance for the contouring.12 Lastly, the texture steps were
performed (Fig. 13), creating vertical texture with a rough appearance for the demarcation of vestibular features and giving an appropriate curvature to the area of the
contact point;13 as well as creating
microtexture: after proper mirror
polishing, restorations can be
made brighter depending on how
the incident light is dispersed, absorbed and reflected on the irregular vestibular surface geometry
(Figs. 14 & 15).14
Once the dental dam had been
removed, abrasive discs were used
(Sof-Lex, 3M) to optimise the mesial margins by reducing hinderances to the passage of dental
floss.15 The shine and three-dimensionality of the treated surface
were enhanced by polishing with a
rubber cup and a diamond paste or
zirconium powder16 (Fig. 16), all the
while paying attention to the interdental spaces.17 To complete the
polishing, a felt disc (Super-Snap
Buff Disk, SHOFU) and a diamondimpregnated
polishing
paste
(DirectDia Paste, SHOFU) were applied to the enamel, and the final
polishing step was performed
using silicone polishers (Dimanto,
VOCO). Conventional radiographic
checks were conducted to confirm
that the restoration had been optimally carried out and was free of
potential infiltration sites (Fig. 17).
Radiographs would serve as confirmation at the DSD check.
At the six-month follow-up appointment, the patient’s situation
was very different to that at the initial consultation. There was proper
closure of the interdental gaps and
consequent formation of papillary
tissue that harmonised the shape
of the attached gingiva and of the
reconstructed enamel perimeter
(Figs. 18–21). In addition, the examination showed no fractures of the
restoration or wear, no marginal
discoloration or other staining, adequate marginal seal, adequate colour stability and translucency, very
good surface structure and anatomical shape, excellent contact
points, very good integrity of the
teeth and no inflammation.
Special advantages of the
VOCO products used
The materials used proved to
be particularly useful on account of
their aesthetic and functional characteristics. Futurabond U was chosen because it can be applied as a
single layer with a total working
time of just 35 seconds, has exceptional adhesion values, expanding
its use to materials such as metals
and ceramics, and has high moisture tolerance. Admira Fusion was
used because it is universally appli► Page 18

establish the correctness of the interdental contours.
The clinical and therapeutic
factors that led us to choose a direct approach with a chairside
technique were concerned with the
aesthetics.18,19 The use of novel chameleon effect nano-hybrid composites ensures the optimisation of
the operating time in the chair,
making it possible to obtain restorations with mechanical and aesthetic properties identical to those
of restorations obtained using a
build-up technique. 20 A further, advantageous aspect was the economic one, since the reduced use
of prosthetic procedures afforded
considerable cost-savings. 21 The increasing economic pressure on the
dentistry system and, in many
cases, the shortage of economic
resources on the patient’s part for
covering the costs of treatment result in the need to carry out anterior restoration treatments that are
reliable, easy and quick and, therefore, more affordable, instead of
the more demanding indirect restorations, such as veneers or
crowns. 22
Conclusion
The patient was very satisfied
with the aesthetic result and the
enormous advantages it would
bring in terms of hygiene and
speaking. Diastema closure, which
was previously mostly performed
using prosthetic or orthodontic
procedures, can be solved with direct restorations.

cable, has perfectly harmonious
translucency and opacity, enabling
high-quality results that respect
anatomical characteristics, and affords the flexibility to work with a
single-colour or multicolour system. Grandio SO Light Flow offers
low viscosity with exceptional fluidity, pinpoint application and outstanding aesthetic results.

Results
The patient was satisfied with
the aesthetic accuracy in terms of
restored anatomy, the gaps in the
interdental space have been resolved. This result gave an implicit
added value to the direct restoration technique: micro-invasiveness. The patient did not feel any
discomfort during the operative

steps, as no significant preparation
of the teeth was needed.
Discussion
For the diagnostic steps, we
chose a digital approach that respected dentofacial symmetry. It
was necessary to carry out a smile
line study with a corresponding
aesthetic diagnosis and treatment

plan to provide us with a correct interpretation of the margins and
emergence profile. This was made
possible using DSD technology. Radiographic monitoring was required to record the pre- and
post-restoration anatomical characteristics. The intra-operative
photographs made it possible to

Veneers, bonding and
artificial intelligence
An interview with Dr Maciej Żarow

In aesthetic dentistry, composite veneers are a topic of interest
for dental professionals seeking to
enhance patients’ smiles with minimally invasive and cost-effective
solutions. Speaking with Dental Tribune International, Dr Maciej
Żarow, a specialist in aesthetic dentistry, discusses the differences and
similarities between composite and
porcelain veneers, providing insights into clinical considerations
and patient suitability. He also
gives advice to dentists starting in
the field and addresses the evolving trends and technological advancements shaping aesthetic
dentistry.

What are the similarities and
differences between porcelain
veneer and composite veneer,
commonly known as bonding?

In my opinion, both solutions
are good, but we should remember
that the indications are completely
different. A composite veneer is
relatively easier to perform, as it is
a truly non-invasive restoration
that does not require preparation
of the tooth. Indications for this
type of restoration include young
patients and patients in whom por-

celain veneers should not be performed. Porcelain veneers should
not be performed on patients
under 25 years or in patients who,
for various reasons, are afraid of
preparation of the tooth, as well as
patients who, for any reason, would
not like to have the tooth prepared
at all.
When deciding on the type of
restoration, one should remember
that composite veneers have their
limitations. They are difficult to
place when teeth are significantly
discoloured. Also, they are not easy
to do when the work involves numerous teeth—six, eight or even
ten teeth—in one arch. It is also not
easy to perform composite veneers
in the gingival area because, on the
one hand, the composite should
cover the labial surface of the
tooth, and on the other hand, we
have to avoid a large emergence
profile and any overhang—that’s
not easy. In the case of porcelain
veneers, the dental technician can
maintain the tooth contour and the
appropriate emergence profile
much more easily working extraorally in the laboratory.
Bonding is often a cheaper,
simpler and faster solution for the
patient. Porcelain veneers are more
expensive, but more durable. Veneers I placed 20 years ago are still

functioning in the patient’s mouth.
As long as the patient has a
well-supported occlusion in the lateral region and no occlusal overload and takes good care of the
gingivae, and there is no recession
around the veneers, then veneers
made several years ago will still be
good.
As I already mentioned, both
solutions have their advantages
and disadvantages. The disadvantage of porcelain veneers, besides
their high price, is the dental clinician’s dependence on the dental
technician for their production. The
technician needs to have the requisite experience in order to perform
veneers in the right way, as they
should be as thin as possible and
aesthetic, having perfectly matched
colour and shape and a preserved
emergence profile. This requires
experience, knowledge and time.
When performing porcelain veneers, the dentist should meet with
the patient at least twice. At the
first visit, he or she must prepare
the tooth, at the second visit, place
the finished work, and at the next
visit, check function and aesthetics.
Sometimes, the work requires fittings and corrections, which entails
more visits and has consequent additional costs for both the dentist
and the patient. These are the disadvantages of porcelain veneers,
but their obvious advantages, in
addition to durability, are compatibility, excellent gingival integration, and ability to mask discolorations and cover many teeth. All
this is possible when working with
a facebow and a good laboratory.
In the case of composite veneers, the more teeth we want to
cover, the more difficult it is to do.
If bonding is performed on one or
two teeth, the work is relatively
simple. If it is eight or more, it re-

quires a lot of skill on the part of the
dentist, and that involves not only
educational courses but above all
practice, practice and more practice. To work well with composites,
one needs to have a well-practised
hand and to learn about the properties of the composite and how to
work with it.

Can you explain what exactly
bonding is?

Bonding is a direct composite
veneer that covers the facial surface of the tooth. “Bonding” is a
simplified name derived from the
word “bond”, which means connection, more precisely a connection
between two surfaces or objects
that have been joined together.
This is a great clinical solution in the
hands of a good, experienced dentist who has the appropriate knowledge and for cases for which its use
is indicated, that is, where there is
no significant damage to the hard
tooth tissue, when we do not want
to prepare the tooth and when we
do not wish to change the shape or
colour of the tooth too much. This
is a very useful method of treatment of young patients with good
occlusal conditions or when these
conditions need to be changed and
can be changed by rebuilding the
occlusion. The patient’s smile can
be enhanced with bonding, but the
bonding is often replaced with porcelain veneers after many years,
because bonding requires good
oral hygiene, including proper
flossing, and avoiding foods that
stain. It is also important to under-

stand that over time, just like natural teeth, it simply wears down.

In your experience, are patients requesting composite veneers more often now than a few
years ago? Have composite veneers become more popular
among patients?

Yes, and I think it’s mostly because of the growing popularity of
medical social media accounts and
case photographs presented there.
Very often, patients see the word
“bonding”, but they actually do not
know what it means. I’ve had patients with porcelain crowns request replacement of their crowns
with bonding, implying that many
patients understand “bonding” to
mean a white, beautiful smile.
Additionally, there are companies selling orthodontic aligner
systems which promote their products with the phrase “align, bleach
and bond”, meaning tooth alignment followed by tooth whitening
and then application of bonding.
This is even referred to as the
“golden trio” in dentistry. But this
golden trio is not always suitable,
because not every patient after orthodontic treatment requires composite veneers, and sometimes
bonding can do more harm than
good. Bonding can be a very good
solution but not always and not for
everyone.

What do you think about
CAD/CAM veneers, that is, milled
or 3D-printed? Are they comparable?

I have experience with both veneers made with feldspathic porcelain technology as well as those
made in the laboratory using the
milling technique. Milling techniques use lithium disilicate, the
most popular material, and it undergoes two distinct phase changes
to achieve its final form. Veneers
made in a dental office using milling machines or 3D printers can be
seen as a competitive alternative
for dental technicians. I know dentists who use chairside CAD/CAM
systems to perform veneers onsite, but they usually employ a dental technician in their office anyway
because they need someone to adjust the colour and shape and make
any necessary corrections immediately. Many dentists do not want to
spend time on these adjustments.
It is easier to do restorations in the
lateral zone but more difficult in the
anterior part owing to the high aesthetic requirements. I am convinced that technologies will continue developing very rapidly, and
soon dentists will be able to perform such work without the involvement of a technician, using
scanners and complete CAD and
printing systems, but at the moment, it is not very common—at
least not in my country.

The precision of these techniques is certainly an advantage,
but do you think that they also
work for highly aesthetic restorations in the anterior zone?

I know dentists who design and
print and/or mill restorations from
various ceramic materials and are
able to do it in a similar way to a
dental technician, but these are
hobbyists and people who have the
time, conditions and passion in this
direction. They are exceptions. In
many cases, the dentist will not
have the time, patience or skills to

do the work of the dental technician.

As the co-organiser of the annual meeting of the European
Society of Cosmetic Dentistry
(ESCD), which was held in
Wrocław in Poland last year, can
you tell us about the significance
of this meeting? Was it in line
with trends in global aesthetic
dentistry?

First of all, I’d like to say that it
was an outstanding meeting of a
very high international level. The
quality of the presentations was excellent. Half of the congress participants were dental professionals
from Poland and half were foreign
guests. All participants I spoke to
during and shortly after the event
agreed that it was an up-to-date
meeting that touched on all issues
related to modern dentistry. We
managed to include in the programme topics covering all areas of
dentistry, including aesthetic dentistry, implantology, endodontics,
restorative dentistry and the very
popular digital orthodontics.
The meeting was outstanding!
ESCD is a unique organisation in
that every year it not only attracts
more and more participants to its
annual conference, but also holds
better and better congresses—and
they are usually held in very interesting European cities. A year ago,
the meeting was held in Rome, and
before that in Athens and Lisbon.
The congress was devoted to
digitalisation, one of the leading
trends in dentistry. There were
many lectures focused on digital
orthodontics, scanning and the
flow of information between the
laboratory and the dental office.
Many speakers presented on digital treatment planning, from simple
restorative dentistry to complex
prosthetic work, implantology and
extremely complex digital planning
of bone grafting. For example, we
had a very interesting lecture by
Prof. Marzena Dominiak from

Wrocław, who presented innovative techniques for the printing of
bone grafting materials.

Among the conference participants, there were many young
dentists just starting their professional careers. What initial advice would you give them? In
what direction should they develop their careers and skills?

The most important thing is not
to move forwards alone but together as a team. Teamwork, especially working with mentors, gives
great opportunities, above all, to
correct your own mistakes. Currently, dentistry is based on excellent photographic documentation
and digital imaging, and consultations are much simpler, so even if
your mentor lives in a different
country it is easy to communicate
with him or her.
ESCD promotes development
based on mentors. It is possible
and indeed recommended for a
new member to find a mentor
among ESCD experts, who will
monitor the young dentist’s work
for a year and teach him or her how
to operate according to the highest
international standards. After a
year of working under the supervision of a mentor, a young dentist
can become a certified member of
ESCD. More information about
mentorship and certification procedures is available on ESCD’s website. We invite all dentists who
would like to participate in the scientific life of this organisation to
join the society. Also during the
Dental Spaghetti congress in April
this year, there will be an opportunity to meet European ESCD mentors.
Holding the ESCD congress in
Wrocław last year allowed the promotion of Polish lecturers. Among
the invited lecturers, in addition to
Prof. Dominiak, were Drs Krzysztof
Chmielewski and Bartłomiej Karaś,
and I too presented. I had the pleasure of lecturing with my mentor,

Dr Marco Castro. It is thanks to him
that I have achieved so much, because it would have been very difficult for me to learn and grow
alone. The next ESCD annual meeting will be held in September in
Belgrade in Serbia, and today I cordially invite dental professionals to
attend this worthy event.

What in your opinion has
changed dentistry in the last ten
to 20 years the most, and in what
direction do you think dentistry
is moving?

The dynamics of change are
currently enormous. Twenty years
ago, we awaited changes, for new
technologies. Now, these changes
are widespread; for example, working with a microscope has become
standard, and many dental offices
already have scanners. This, of
course, requires learning new competences and skills. If today a dentist does not learn how to scan, in a
few years, he or she will not be able
to use new technologies. It is mandatory to understand the principles
of milling and printing because
these technologies are the future
of our profession. In my opinion,
changes in dentistry in recent years
have been driven mostly by new

technologies and new equipment
but, above all, by postgraduate education and professional development of dental practitioners.
New technologies have made it
possible to make veneers with a
thickness of 0.3 mm, thus reducing
preparation to a minimum and
even making it possible to provide
restorations without preparation.
Reconstructions are possible without disturbing the tooth structure.
Implantology has also changed in
that it is becoming more predictable. Implants can be planned digitally and placed precisely with help
of 3D-printed guides.
And what do I expect in the
coming years? I expect that artificial intelligence algorithms will
enter dentistry in a more visible
way and be able to help us a lot and
improve our work. However, I certainly would not want new technologies and artificial intelligence to
disturb our professional thinking
and the basics of dentistry. I would
like us to plan our treatment and
care based on all our knowledge
that we have acquired during our
professional education and years
of work.

In your current role at Barazi
Dental Centre in Al Ain, could
you provide our readers with
more information about the clinic
and the range of procedures you
perform there?

Barazi Medical Center has been
a cornerstone of healthcare in Al
Ain since its establishment in 1987,
making it one of the oldest and
most renowned medical centers in
the city. Within the dental department, my focus primarily lies in oral
surgery and periodontics. I take
great pride in my specialization,
which involves addressing complex
surgical cases and the treatment of
periodontal conditions. Additionally, I find joy in performing restorative cases, contributing to the
overall oral health and aesthetics of
our patients.

Dr. Tarek, it is a pleasure to

such as yourself in Dental

To begin, could you provide our
readers with some insights into
your background? Tell us about
your educational journey and
your extensive work experience,
particularly in relation to your
practice in the UAE.

Certainly. I completed my dental education at Ajman University
for science and technology (AUST)
in 2010, and over the past decade, I
have been dedicatedly practicing
dentistry at Barazi Dental Centre in
Al Ain. My professional journey has
been rooted in the UAE, allowing
me to gain valuable experience and
contribute to the dental healthcare
landscape in this dynamic region. I
have had the privilege of witnessing and being part of the evolving
dental industry, and it has been a
fulfilling experience serving the
community here for the past ten
years.

We understand that you have
successfully completed the Clinical Implantology Dentistry Certificate and Diploma programme
from the CAPP-Tipton Dental
Academy, the British Academy of
Dental Implantology, and the
British Academy of Restorative
Dentistry. Could you share your
experience with our readers and
elaborate on the programme's
components and what it entailed?

to provide enhanced and more sophisticated care to my patients. The

and technologies into my practice,
all gained through ongoing education, has been pivotal in elevating

Charting the future of orthodontics: 1
Kuwaiti Orthodontics Symposium organised by KOS and CAPP
was a success

The collaborative efforts of the
Kuwaiti Orthodontics Society (KOS)
and the Center for Advanced Professional Practices (CAPP) lead to
highly successful 1st Kuwaiti Orthodontics Symposium, held on 12-13
January 2024 at Radisson Blue
Hotel in Kuwait. Drawing dental
professionals from Kuwait and
neighboring countries, the event
facilitated a platform featuring expert speakers who explored emerging trends and shared insights
shaping the future of orthodontics.
Throughout the symposium,
renowned speakers addressed a
spectrum of topics, including advancements in aligner treatments,
the integration of digital technologies in orthodontic workflows, and
the challenges posed by complex
cases. Discussions ranged from the
effective utilization of Temporary
Anchorage Devices (TADs) in conjunction with aligners to the critical
role of orthodontists in managing
Obstructive Sleep Apnea (OSA)
using innovative techniques such
as Maxillary Skeletal Expansion
(MARPE) and Mandibular Advancement
Device (MAD) treatment.
The event's diverse presentations underscored the evolving
landscape of orthodontics, with
speakers sharing their experiences
and expertise in areas such as
growing class III patient treatments, soft tissue analysis in orthognathic surgery, and the effective management of challenging
cases.
The symposium culminated in a
full-day hands-on course titled
"TAD-Masterclass – Clinical applications of skeletal anchorage in orthodontics" conducted by Dr. Maximilian Küffer from Germany. This
immersive session allowed participants to gain practical insights into
the clinical applications of skeletal
anchorage, further enhancing their
orthodontic skills.
In essence, the collaboration
between the Kuwaiti Orthodontics
Society and CAPP at the 1st Kuwaiti
Orthodontics Symposium exemplified a shared commitment to advancing the field of orthodontics,
fostering international collaboration, and nurturing excellence in
orthodontic practice across Kuwait
and beyond.
The organizers have shared
that in 2025 they plan to do a second edition of the Symposium and
dates will be announced shortly.

course in Dubai

“The Art of Anterior Composites” hands-on course was delivered exceptionally by Dr Shiraz
Khan from the UK at the CAPP
Training Institute in Dubai at the
beginning of March 2024. This was
an intensive two-day hands-on
training course focused on mastering the art of anterior composites.
Hosted by DMG and their local UAE
distributor Advanced Healthcare,
the course provided attendees with
the knowledge and practical skills
needed to excel in this critical aspect of dentistry.
The venue for the course was
the state-of-the-art CAPP Training
Institute, which provided the perfect setting for participants to immerse themselves in the world of
anterior composites. From the administrative work to securing permits and accreditation, CAPP ensured that everything was in place
for a seamless learning experience.
Day one kicked off with a series
of lectures covering essential topics such as the overview of anterior
composite dentistry, basic principles of colour matching and shade
selection, and advanced layering
techniques for achieving natural
aesthetics. Participants were also
introduced to resin infiltration and
its applications in dental treatment,
including case selection and treatment planning for innovative Icon
technique.

Day two attendees dove into
hands-on sessions where they had
the opportunity to put theory into
practice. Under the guidance of Dr.
Shiraz Khan, participants worked
on phantom heads to master shade
matching, preparation, layering,
contouring techniques, and case
discussions. The interactive nature
of the hands-on sessions allowed
for real-time feedback and troubleshooting, ensuring that participants gained confidence in their
skills. In the afternoon, hands-on
sessions continued with a deeper
dive into the Icon resin infiltration
technique, giving participants the
chance to further hone their. Case
discussions provided valuable insights into real-world applications,

CAPP hosts King's College London
once again for its programme in Dubai

The Centre for Advanced Professional Practices (CAPP) recently
welcomed King's College London
back to its training institute in
Dubai. The renowned institution
was in Dubai from February 11 to
20 to conduct its Fixed & Removable Prosthodontics MClinDent
programme.
This four-year, blended learning programme features face-toface training blocks in Dubai, UAE,
and is delivered and awarded by
King’s. The Fixed & Removable
Prosthodontics MClinDent is a
master’s level programme in restorative dentistry, designed to enhance skills in managing complex
multi-disciplinary dental problems.
A distinguishing feature of this
programme is its innovative blend
of online learning and face-to-face
teaching blocks. These blocks
occur in each of the first three
years, giving students the flexibility
to choose between attending in

London or Dubai. The programme
is tailored for practicing dentists,
enabling them to apply the skills
acquired directly in their practices.
Professor Brian Millar and Dr.

Excitement builds as IDEM 2024
emerges as the pinnacle event
in dental innovation for Asia Pacific

This year’s International Dental
Exhibition and Meeting (IDEM), the
leading dental exhibition and conference in Asia Pacific, is poised to
captivate and inspire dental professionals from around the region.
This highly anticipated event, happening from 19 to 21 April at
Marina Bay Sands in Singapore, will
offer a comprehensive three-track
scientific conference featuring
more than 30 speakers from around

the 13th IDEM, father and son duo
Drs David S. and Davey Alleman will
kick off the conference programme
with the SDA Masterclass, an established part of the event supported
by the Singapore Dental Association, offering a half-day conference
session and half-day workshop on
biomimetic dentistry.

The over 30 conference sessions and workshops will feature a
wide range of topics by notable
speakers, such as Dr Alberto Miselli,
who will cover the “Umbrella Concept” for the management of soft
tissue around implants, and
Dr Andrea Bazzucchi, who will
share the latest advances in digital
workflow synergy. Other conference sessions and workshops will
include global expert Dr Roberto
Sorrentino’s “Restorative materials
and luting agents in prosthodontics” and Dr Paulo Monteiro’s insights and expertise on anterior indirect restorations. The fifth Dental
Hygienist and Therapist Forum will
take place on 20 and 21 April, providing a dedicated platform for
dental hygienists and therapists to
engage in meaningful discussions
and knowledge sharing on an interesting variety of topics, such as
oral ulcers, paediatric patient management, digital workflows and
oral care for older adults.

Singapore to welcome dental
clinicians
Dr Lawrence Yong, president of
the Singapore Dental Association,

and the US, underscoring the wide
