ADA releases new tooth
decay treatment guideline
Expert panel advises conservative approaches in first-ever restorative clinical recommendations for removal of decay
A new American Dental Association
(ADA) clinical practice guideline suggests conservative methods to treat
tooth decay in primary and permanent
teeth could lead to better outcomes
when used with common restorative
materials like fillings or caps. An expert
panel of dentists developed the firstever guideline on this topic after extensive review of approximately 300 published studies.
The guideline, published in the July
issue of The Journal of the American
Dental Association, contains 16 recommendations regarding treatment of
moderate and advanced tooth decay in
primary and permanent teeth that have
not received endodontic treatment, such
as a root canal. It indicates conservative
carious tissue removal (CTR) — in which
a dentist removes infected tissue while
preserving as much of the original tooth
structure as possible — is less likely to
result in adverse outcomes like nerve
exposure or a failed filling.
The recommendations also identify
selective CTR as an effective treatment
option in most cases of moderate or
advanced decay in primary and permanent teeth. This method involves remov-

(Photo/Nachtbold, Wikimedia Commons)

ing most, but not all, of the decayed tissue before sealing the tooth with a filling
or cap. The bacteria left behind under
the new filling or cap no longer has what
it needs to multiply, which stops tooth
decay.
Additionally, the guideline affirms
the efficacy of the most common restorative materials for treating moderate or
advanced tooth decay, such as toothcolored fillings, silver-colored fillings

(amalgam) or preformed caps in children. It suggests specific materials for
primary and permanent teeth depending on the extent of the decay.
Lead author Vineet Dhar, BDS, MDS,
PhD, is clinical professor and chair of orthodontics and pediatric dentistry at the
University of Maryland School of Dentistry and a member of the ADA Council
on Scientific Affairs. He said the recommendation encapsulates restorative

dentistry’s two main objectives: maintaining healthy tooth structure and protecting the soft tissue inside the tooth.
“While research had already confirmed that selectively removing decayed tissue is an effective approach
to treating early tooth decay, dentists
needed an evidence-based guideline
to provide them with a range of treatment choices for patients with moderate to advanced tooth decay,” Dr. Dhar
said. “These recommendations can now
inform restorative care strategies in the
U.S. and on a global level.”
This is the ADA’s second clinical practice guideline in a series on caries treatment. The new guideline was developed
by the ADA Council on Scientific Affairs
and the Clinical and Translation Research team within the ADA Science and
Research Institute, which convened a
panel of experts to review the best-available scientific evidence around treatments and materials.
The new restoration guideline and existing recommendations on non-restorative treatment for tooth decay can be
found at ada.org/cariesguidelines.

‘This brush cleans where
normal bristles do not reach’
By Curaden Staff

A brush that cleans where others do
not? An expert in prophylaxis and periodontics, Dr. Ulrich P. Saxer is the codesigner of a brush that does just that.
Curaprox’s Hydrosonic pro cleans powerfully, efficiently and gently, thanks to
a unique interplay of sonic technology
and hydrodynamics.
“When designing the brush, it was a
very conscious decision to opt for sonic
technology. The bristles of the Hydrosonic pro oscillate up and down, which
is also the recommended technique for
using a manual brush. The Hydrosonic
pro cleans effectively but does so in a
way that is atraumatic, which is ideal for
people with sensitive teeth or gingivae,”
Saxer noted.
“Many people brush in a way that
causes damage to teeth and gingivae.
Using the wrong brush or brushing
technique can cause abrasions. Cervical
lesions at the cemento-enamel interface
are a particular problem, as is sensitivity of the tooth necks. These issues go
hand in hand with gingival recession or
injury. Using a high-quality sonic toothbrush is a good way to prevent these issues,” he continued.

Curaprox’s Hydrosonic is a
high-quality sonic toothbrush that
effectively and gently cleans the teeth
and can reach the most inaccessible places.
(Photos/Provided by Curaden)

Reaching the most inaccessible
places
“Most people clean an entire row of
teeth with an aggressive backwards
and forwards movement. The shape of
the Hydrosonic pro’s brush heads ensures the opposite: each tooth has to
be cleaned individually. The rounded
part of the brush head cleans the buccal,
lingual and occlusal surfaces with high
precision, tooth by tooth.
“The pointed part of the drop-shaped
head reaches the interdental areas better than any manual brush. The angulation of the brush head is perfect for
reaching the most inaccessible and
critical places. In other words, the brush
heads are ideal for patients with crowns,
bridges, orthodontic appliances or implants,” Saxer explained.
“The single tuft brush allows very precise cleaning of the tooth where plaque
primarily accumulates — namely at the
gingival margin, on fixed appliances,
in other angled narrow tooth positions
and interdentally. Many dental professionals clean exclusively with this
method, running the brush along the
gingivae on all teeth. This method has
been proved scientifically to offer the
best cleaning effect.”

For more information
Visit Curaden at curaden.us to find out more
about the Hydrosonic pro brush or scan the
QR code below.

Dr. Ulrich P. Saxer, the co-designer of Curaprox’s Hydrosonic pro sonic toothbrush.

A cleaning concert for teeth
and gingivae
“In addition to the purely mechanical
cleaning, there is an extra hydrodynamic cleaning effect. Thanks to vortices of toothpaste, water and saliva, this
brush cleans where normal bristles do
not reach. This effect increases with the
frequency selected,” Saxer said.
“The filaments of each brush head
vary in thickness, length and hardness.
Their interaction and the transmitted
frequency, which is not the same at ev-

ery point of the brush head, is finely
tuned. The inner bristles are shorter,
so they swing freely, keeping the brush
head a certain distance from the tooth
and supporting the hydrodynamic effect. The outer bristles touch the tooth
and are designed to clean narrow spaces, prevent discoloration and eliminate
calculus growth.
“Not every manufacturer achieves the
hydrodynamic effect well, and many
are not even aware of it. The interplay of
bristles and their arrangement must be

very precise. Their interaction and the
transmitted frequency, which is not the
same at every point on the brush base,
produces a harmonious ‘cleaning concert’ for teeth and gingivae. The result
is Curaprox’s very own symphony — accepted, atraumatic and effective,” Saxer
concluded.

JIZAI files designed to be
smooth, flexible and simple
The JIZAI System is the newest addition to the MANI endodontic product
line. Each file is fully heat treated, highly
flexible and designed to cut smoothly
and efficiently.
The cross-sectional shape and flute
pitch help prevent overinstrumentation
and file binding. Each file can be prebent and will hold its shape.
The Japan-manufactured files are
available in individual ISO-sized packs
of three files or as part of a standard
kit, which includes a sequence of 25.04,
25.06, and 35.04 files designed to instrument straight and slightly curved canals.
Three foundational concepts form the
basis of JIZAI.
• Smooth designs provide an optimal
cutting experience. Radial lands help
prevent overinstrumentation and excessive engagement on the root canal

• Flexible respects the original anatomy. The heat treatment that each JIZAI
undergoes results in a significantly lower risk of ledging, transportation, and
perforation.

walls. The off-center file axis is uniquely
designed to provide a larger “pocket”
space for debris removal during instrumentation.

• Simple describes how files shape severely curved canals with ease through
procedural-based sequencing. Tip size
and taper are etched on the shank.

Established in 1956 in Takenzawa, Japan
(north of Tokyo), by Masao Matsutani,
MANI is a manufacturer of medical devices and dental instruments.
Its singular goal is “Contributing to
people’s happiness in the world through
developing, manufacturing and delivering the product that can help doctors
and patients.”
MANI products are safe, high-quality
medical devices that satisfy the needs of
doctors and patients. The products have
also passed the strict standards of various countries around the world.
