Teeth and Type 2 diabetes
Ability to chew properly may improve blood sugar levels in patients with Type 2 diabetes
In a study published in PLOS ONE and
further demonstrating the link between
dental care and full-body care, University at Buffalo researcher Mehmet A.
Eskan has demonstrated that patients
with Type 2 diabetes (T2D) who have full
chewing function have a blood glucose
level that is significantly lower than patients whose ability to chew effectively
is impaired.
Eskan is a clinical assistant professor
in the Department of Periodontics and
Endodontics at the School of Dental
Medicine at UB.
The retrospective study looked at data
gathered from 94 patients with T2D who
had been seen at an outpatient clinic in a
hospital in Istanbul, Turkey. The patients
were divided into two groups: the first
group included patients who had good
“occlusal function” — enough teeth
placed properly and making contact in
such a way that a person can chew their
food well. That group’s blood glucose
level was 7.48.
The second group couldn’t chew well,
if at all, because they were lacking some
or all of those teeth; their blood glucose
level was almost 2 percent higher, at 9.42.
Digestion, the process by which your
body extracts nutrients from food, begins as chewing stimulates the production of saliva. Nutrients that are important to reduce blood glucose levels
include fiber, which is obtained in large
part through chewing appropriate foods.
Chewing also has been reported to
stimulate reactions in the intestine that
lead to increased insulin secretion and
the hypothalamus that promote a feeling of satiety, resulting in less food intake. Eating less decreases the likelihood
of becoming overweight, which is a major risk factor for developing T2D.
Eskan received his DDS at Hacettepe
University, a leading medical research
center in Turkey, and earned his PhD at
the University of Louisville, where he

also completed a residency in periodontology.
“My special clinical interest is to treat
dental patients who are systemically
compromised,” he said.
His research goal is to contribute to the
big picture of improving public health.
This research notes that, as of 2019, almost half a billion people worldwide had
diabetes, and at least 90 percent of those
patients with diabetes have T2D.
Addressing oral health has recently
become part of the approach to managing diabetes along with encouraging patients to maintain a healthy weight, eat a
healthy diet and quit smoking.
“Our findings show there is a strong association between mastication and controlling blood glucose levels among T2D
patients,” Eskan said.
This study did not find any independent variables that could affect blood
glucose levels among the subjects because there were no statistical differences among subjects regarding body mass
index, sex, smoking status, medications
or infection as indicated by white blood
cell count at the baseline.
The dramatic improvement in one
patient’s case described in a 2020 study
co-led by Eskin illustrates the potential
benefit of improving occlusal function

through dental implants and appropriate fixed restoration. A T2D patient
whose chewing function was severely
impaired by missing teeth presented
initially with a blood glucose level of
9.1. The patient obtained nutrition by
using a bottle and eating baby food.
Four months after treatment with a full
mouth implant-supported fixed restoration, the patient’s glucose level dropped
to 7.8. After 18 months, it decreased to
6.2.
Research has shown that an increase
of just 1 percent in blood glucose level is
associated with a 40 percent increase in
cardiovascular or ischemic heart disease
mortality among people with diabetes,
according to Eskan. Other complications
can include kidney disease, eye damage,
neuropathy and slow healing of simple
wounds like cuts and blisters.
“I’m interested in research that can improve people’s health now,” Eskan said.
He and co-author Yeter E. Bayram,
MD, Department of Internal Medicine,
Hamidiye Sisli Etfal Education and Research Hospital in Istanbul, want to work
on further studies that explore possible
causal relationships between occlusal
support and blood glucose levels.

On Saturday, May 20, from 9:30 a.m. to
3 p.m., there will be snacks available as
well as spots to recharge your devices and
connect with other peers and mentors.
There will also be several speakers with
topics geared just for you.
And just 90 minutes later, at 4:30 p.m.,
there will be a student networking event.
You can build new relationships with
peers over food, drinks and fun games.
For all attendees, and especially those
trying to get their steps in, you will easily
get 10,000 of them just strolling through
the exhibit hall and checking out the
more than 400 exhibitors.
Attendees can meet face-to-face with
company reps who are showcasing prod-

ucts, services and technology. Dental professionals can interact with the experts
and even participate in activities to win
prizes.
The exhibit hall will be open all three
days of the meeting.
CDA follows the California Department
of Public Health COVID-19 guidelines

areas are twice as large as prismatic expanded field designed loupes and up to
five times greater than Galilean loupes.
Panoramic Field loupes provide unprecedented field of view, clarity, definition and color, the company asserts.
Are you experiencing back or neck
pain or want to prevent musculoskeletal
injuries by improving your posture? The

‘This brush cleans where
normal bristles do not reach’
By Curaden Staff

A brush that cleans where others do
not? An expert in prophylaxis and periodontics, Dr. Ulrich P. Saxer is the codesigner of a brush that does just that.
Curaprox’s Hydrosonic pro cleans powerfully, efficiently and gently, thanks to
a unique interplay of sonic technology
and hydrodynamics.
“When designing the brush, it was a
very conscious decision to opt for sonic
technology. The bristles of the Hydrosonic pro oscillate up and down, which
is also the recommended technique for
using a manual brush. The Hydrosonic
pro cleans effectively but does so in a
way that is atraumatic, which is ideal for
people with sensitive teeth or gingivae,”
Saxer noted.
“Many people brush in a way that
causes damage to teeth and gingivae.
Using the wrong brush or brushing
technique can cause abrasions. Cervical
lesions at the cemento-enamel interface
are a particular problem, as is sensitivity of the tooth necks. These issues go
hand in hand with gingival recession or
injury. Using a high-quality sonic toothbrush is a good way to prevent these issues,” he continued.

Curaprox’s Hydrosonic is a
high-quality sonic toothbrush that
effectively and gently cleans the teeth
and can reach the most inaccessible places.

Reaching the most inaccessible
places
“Most people clean an entire row of
teeth with an aggressive backwards
and forwards movement. The shape of
the Hydrosonic pro’s brush heads ensures the opposite: each tooth has to
be cleaned individually. The rounded
part of the brush head cleans the buccal,
lingual and occlusal surfaces with high
precision, tooth by tooth.
“The pointed part of the drop-shaped
head reaches the interdental areas better than any manual brush. The angulation of the brush head is perfect for
reaching the most inaccessible and
critical places. In other words, the brush
heads are ideal for patients with crowns,
bridges, orthodontic appliances or implants,” Saxer explained.
“The single tuft brush allows very precise cleaning of the tooth where plaque
primarily accumulates — namely at the
gingival margin, on fixed appliances,
in other angled narrow tooth positions
and interdentally. Many dental professionals clean exclusively with this
method, running the brush along the
gingivae on all teeth. This method has
been proved scientifically to offer the
best cleaning effect.”

Dr. Ulrich P. Saxer, the co-designer of Curaprox’s Hydrosonic pro sonic toothbrush.

A cleaning concert for teeth
and gingivae
“In addition to the purely mechanical
cleaning, there is an extra hydrodynamic cleaning effect. Thanks to vortices of toothpaste, water and saliva, this
brush cleans where normal bristles do
not reach. This effect increases with the
frequency selected,” Saxer said.
“The filaments of each brush head
vary in thickness, length and hardness.
Their interaction and the transmitted
frequency, which is not the same at ev-

ery point of the brush head, is finely
tuned. The inner bristles are shorter,
so they swing freely, keeping the brush
head a certain distance from the tooth
and supporting the hydrodynamic effect. The outer bristles touch the tooth
and are designed to clean narrow spaces, prevent discoloration and eliminate
calculus growth.
“Not every manufacturer achieves the
hydrodynamic effect well, and many
are not even aware of it. The interplay of
bristles and their arrangement must be

very precise. Their interaction and the
transmitted frequency, which is not the
same at every point on the brush base,
produces a harmonious ‘cleaning concert’ for teeth and gingivae. The result
is Curaprox’s very own symphony — accepted, atraumatic and effective,” Saxer
concluded.
