Over 100 dental professionals attended the Middle East launch of Align
Technology’s latest innovation, the
iTero Element Plus series, held on the
evenings of 18 and 19 May at the Theatre of Digital Art in Dubai in the UAE.
The interactive events featured
presentations by two leading international digital dentistry experts, who
described the role of end-to-end digital treatment in improving both prac-

tice efficiency and patient experience.
Dr Ingo Baresel, a German dentist and
president
of
the
Deutschen
Gesellschaft für digitale orale Abformung (German association for digital
oral impressions), discussed the benefits of digitisation with iTero scanners
for general practitioners on the first
evening. On the second evening, the

held across the region, attracting
more than 80 attendees at the
Ritz-Carlton hotel in Riyadh in
Saudi Arabia on 24 and 25 May and
over 50 attendees at both the Four
Seasons hotels in Doha in Qatar
and in Manama in Bahrain on 30
and 31 May. During the launch in
Dubai, Dental Tribune Middle East
& Africa spoke with Angelo Maura,
general manager for the Middle
East region at Align Technology, on
the iTero Element 5D Plus intra-oral
scanner, the advantages of this system for dentists and patients, and
the company’s outlook for the region.
What excited you about yesterday’s
and today’s event, and what has the
feedback been from your current
and prospective customers on the
new scanner that you have just
launched?
The feedback has been incredibly positive, from both customers
who already own and use the
Element 2 scanner, our previous
model, and those who do not own a
scanner and are not employing a
digital workflow.
Our choice of venue, the
Theatre of Digital Art, and our emphasis on the digital experience
have really resonated with attendees. The vast majority of dental and
general healthcare organisations
in the UAE want to digitalise in the
next few years, so this is something that is creating a sense of urgency. We’ve passed the phase
from early adopters to early majority. People are understanding that
digital is not the future but the
present, and it is getting easier and
easier to get them to embrace it because it’s a matter of future-proofing, and I believe that it is going to
get easier and easier the more we
keep showing the benefits of the
technology.
What is so unique about the iTero
Element 5D Plus, which is newly released on the Middle Eastern market? Obviously, you have quite a lot
of experience from other markets.
What has the feedback from those
users been, and based on that, what
are you expecting regarding dentists in the region?
There are benefits that come
from iTero’s ongoing evolution
from intra-oral scanner to digital
platform. It is almost like a staff
member in the practice, helping to
record, visualise, communicate
with the patient and execute the
treatment. These benefits of course
apply to any generation of iTero,
but we do see more and more of
that as we evolve our technology.
This advancement comes from
how much more powerful it is than
before: it is faster, not only from a
scanning perspective but also
from a processing perspective. As
demonstrated by Dr Baresel yesterday, when communicating with
labs, sometimes it’s a matter of seconds or minutes from taking the
scan to it reaching the lab. It’s faster
in processing. We’ve harnessed artificial intelligence (AI) and cloud
storage to speed up the process. Of

course, there is a design factor as
well. The footprint is smaller, and
it’s definitely more sleek, more elegant, so there is also an aesthetic
factor. The big advancement, the
reason we call it 5D, is the NIRI
(near-infrared imaging) technology, which allows the visualisation
of caries in real time, while scanning,
and
especially,
like
Dr Couchat said, capturing interproximal caries that is sometimes
difficult to capture via other
means. This has been found to be
more effective than bitewings.
This technology comes together in
the same scan. This is the only
scanner on the market that yields a
3D scan, intra-oral colour, intra-oral photos and NIRI in one
scan, all in less than 60 seconds in
the hands of a trained dentist.
There has also been a major advancement in the simulation tool,
the Invisalign Outcome Simulator
Pro, which has just been launched,
including the visualisation interface, and that is yet another step towards making iTero an extremely
powerful visualisation and communication tool beyond being an
intra-oral scanner.
Could you elaborate more on the
benefits of this product for the patient?
The benefit for the patient is
that he or she will be able see what
is wrong in the mouth and what
improvements he or she needs to
make to his or her oral health situation, rather than the dentist just
telling him or her. It’s one thing
having the dentist tell the patient
that he or she has a cavity and what
needs to be done, but it’s another
seeing it on the screen. In a way—
this is something I heard from a
key opinion leader in the US—the
clinician is delegating the giving of
the bad news to the machine, so it’s
the machine that shows exactly
what is going on. It shows this in a
visual and easy-to-understand
way, communicating to the patient
how to improve his or her oral
health. I think that the biggest advancement is moving from scanning to visualising and moving
from a single tool to a platform,
and it allows the provision of information and education, especially,
to the patient in a very simple, visual and easy-to-understand way.
This promotes higher patient
acceptance of treatment, better
understanding of treatment and
better preventive care because of
the NIRI technology, thereby improving the oral health of patients.

What does this launch mean for the
region? What do you expect from
this launch and over the next few
months and years?
What we expect, apart from elevating the level of technological
advancements available in the region and apart from having another tool in our portfolio, is that
this is something that is going to
help dentists completely digitalise
their practices. With a simple intra-oral scanner, the dentist starts
by simply digitising the impression, moves towards digitising the
treatment plan and moves towards
digitising the preventive analysis
and diagnosis, so we are evolving
our overall digital platform to
serve dental practices in the best
way possible and to completely absorb the internal and external
workflow. This is something that is
going to be helping dentists to do
better dentistry and more dentistry, to give better quality of care
and in future-proofing.
The future is growing exponentially, not linearly, and this is a
product that will help dentists stay
ahead of the curve. As we said at
the beginning of Dr Couchat’s presentation, this is going to allow
dentists to take control of the future, rather than submit to it.
Can you tell us more about the support to customers in the region?
When it comes to bringing such
a premium technology to a market, it’s important that we not only
provide the product technological
advancements but also address the
kind of support and community
that we wish to create around the
product. As you mentioned, we’ve
been investing heavily in the product. To give you a frame of reference, over the last few years, we
have invested more than US$250
million a year in research and development. At the same time, we’ve
been expanding the team aggressively. We are creating an entirely
new team for restorative technology to support dentists in their
practices. In our team, we started
integrating what we call practice
integration managers, who are
experts both from a technical
perspective and from a workflow
perspective, who will be going to
dentists’ practices and supporting
them in integrating this new technology. Something that we are
doing and had started doing before
is providing an approach to service
from a complete peace of mind
perspective. We have a service
package that comes with the scanner and includes the warrantee, re-

pair and technological updates. It’s
basically a one-stop shop that allows the dentist to have complete
piece of mind and be sure that he
or she has a piece of equipment
that is always on and the certainty
of support from the Align team
and of having the technology always up to date.
Align Technology recently entered
Oman. Can you tell us more about
this and your future regional expansion?
We have officially introduced
the Invisalign system in Oman,
and we will soon be introducing
the iTero intra-oral system there
too. For us, this is an extremely important step in order to expand our
footprint in the region. It reinforces our presence in the GCC
[Gulf Cooperation Council] countries and our service to the Middle
East. It allows us to capture the
amazing potential that we have in
Omani dentists and patients to expand awareness of the brand and
to make sure we can offer our service to more and more dentists in
order for them to treat more patients with best-in-class technology.
Of course, we will continue
working towards expanding our
presence and access geographically, and we will be making sure to
hire and have a team offering
proper support on the ground to
sustain the growth of the dentists
who will be coming along with us
on this journey.
Something important to mention is that, like other Middle Eastern countries, Oman is futureoriented, having a future vision of
technology and innovation, and
we want to make sure that we are
there to see it realise and support
it, and for that reason, we are extremely pleased to have made this
step this quarter.
In your view as the general manager
of Align in this important region for
the company, what is the future for
Align here in five or ten years?
This region is the cradle of the
future. What we are seeing happening in the Middle East right
now, in all the council countries
and beyond, is incredible attention
to and investment in digitalisation, innovation, AI, cloud computing and machine learning—in the
future in general—and in all sectors of life. Embracing technology
is in the very fabric of the Middle
East right now. This is the place to
be for evolving and shaping the future of dentistry. We’re going to

the food industry, and they layer
the SDGs into four layers like a
wedding cake, using environment
(biosphere), society and economy
for the first three layers, and finally, adding SDG 17, called “partnerships for the goals”, on top.
The message, in very simple
terms, is that we need to attend to
all the layers in the wedding cake
to succeed, but ultimately, without
the environment (biosphere)—our
planet—the other layers become
redundant.
As dental professionals, we can
work in all layers to create a successful sustainable practice. Examples of this are:

SINGAPORE: Why should dental
professionals strive towards sustainable dentistry? Firstly, it is the
right thing to do ethically; secondly, it is a great marketing-tool;
and finally, it creates an attractive
workplace for new colleagues, since
research indicates that the younger
generation values sustainable
workplaces more highly than monetary compensation. Before taking
steps towards creating a green dental practice and practising green
dentistry, the practitioner should
understand that sustainability minimises the pitfalls and simplifies
the process. This article will give a
brief overview of sustainable de-

velopment, focusing on Agenda
2030 and the 17 Sustainable Development Goals (SDGs) of the United
Nations (UN).
Modern sustainable development has its origins in 1972, and it
has evolved into sophisticated processes and workflows. It is still
often associated with the combating of global warming and irresponsible waste management, and
rightly so, but the subject is complicated owing to the multiple factors that are interconnected.
Agenda 2030 was accepted by
the UN in 2015 and has the aim “to
end poverty, protect the planet,
and ensure that by 2030 all people
enjoy peace and prosperity”. It can
be described by using the five Ps:

people, planet, prosperity, peace
and partnership. Protect people
from poverty and hunger; take
steps to protect our planet from
degradation and global warming;
make sure that we can prosper in
symbiosis with our environment;
establish peace for mankind; and
be able to live without fear and violence. This can only be achieved by
the five Ps working together—creating local and global partnerships. Sustainable development is
dependent on progress within all
the five Ps, since they are all connected. Without partnership, there
is no peace; without peace, there is
no prosperity; and without a
planet, there is no future.

The five Ps are an overview of
Agenda 2030, whereas the UN
SDGs are a breakdown of Agenda
2030, which is based on the eight
millennium development goals
(2000-2015) and further divided
by the three pillars of sustainability—economic, social, and environmental—into 17 tangible goals
and 169 unique actions. These action points provide further guidance for an individual who wants
to practise sustainability using the
SDGs or for an organisation which
has the same aim.
Even though the SDGs provide
a structured and focused course of
action with clear indicators, it can
still be a daunting task to implement them in the day-to-day prac-

Environment: SDG 13 relates
to “climate action”. Here, Target 13.2 is to lower greenhouse
gas emissions, which is closely
connected to our day-to-day
work. We will discuss this in
detail later in the series.
Society: SDG 3 relates to “good
health and well-being”. Here,
Target 3.8 is to battle non-communicable diseases, which in
dental medicine, relates primarily to caries and periodontal disease.
Economy: SDG 12 relates to
“responsible
consumption
and production”.
Here, Target 12.5 is to focus on
procurement and waste management such as reduction,
reuse and recycling.

These targets are only examples,
and it is up to professionals and
practices to decide where they can
make the greatest impact.
Additionally, we can support
organisations that work directly
with other actions, like that of providing safe and affordable water
for all. This support is an example
of SDG 17 “partnerships for the
goals”, which is the last layer of the
wedding-cake. This is also where
dental professionals can make a
major impact globally by partnering with sustainable suppliers. In
Part 3, sustainable procurement
will be discussed in greater detail.

“Colleagues challenged me to predict
where caries might occur in the mouth”

A new prototype device paves
the way for pinpointing exactly
where acidity in oral biofilm is increasing and thus where caries is
most likely to develop. The optical
pH monitor, called O-pH, has provided impressive results in monitoring oral biofilm pH levels and
has even influenced the career
path of one of its developers. Dental Tribune International (DTI) was
able to speak with lead researchers
Dr Eric Seibel, research professor of
mechanical engineering with department affiliations in electrical
and computer engineering, bioengineering and oral health sciences
at the University of Washington,
and doctoral student Manuja
Sharma about the development of
the O-pH device, its potential and
the motivation behind the project.
Ms Sharma, DTI previously reported on the new prototype optical pH sensor O-pH. How has developing a dental device as an electrical
and computer engineering PhD student influenced your studies and
your targeted career path?
Sharma: On a personal level,
my research has motivated me to
work in health diagnostics in order
to design and develop sensors to
investigate the signals and parameters of the human body that are,
as yet, unexplored. In the development of O-pH, I had an opportunity to interact with dentists,
chemists and other engineers and
understand what it takes to build a
prototype and deliver it to the
clinic. I am sure that these skills
and experiences will be extremely
valuable in the course of my future
career.
Dr Seibel, the study mentioned
that previous attempts at measuring dental biofilm pH utilised electrodes or pH strips. How did your
team come up with the revolutionary concept of using photodiodes?

AD

Dr Seibel: My dental colleagues
challenged me to predict where
caries might occur in the mouth,
which led me to measure pH, not at
the outside of the oral biofilm,
bathed by saliva, as done by pH
electrodes or paper pressed up
against the biofilm, but throughout the biofilm. This requires a
pH-sensitive dye that permeates
only the extracellular matrix in
order to obtain an accurate reading
of the pH that surrounds enamel.
Sharma: The low-power blue
light makes the dye inside the syringe fluoresce, and then this dye
on an artificial tooth set mimics
how the device works in the mouth.
During your research with O-pH,
did you identify any other opportunities for further research other

than that of investigating different
levels of sucrose concentration?
Dr Seibel: We have been pursuing a dental app that would help
guide a teenager undergoing orthodontic treatment, or his or her
parent, in applying prescription-level fluoride varnish at home
for the locations that are identified
as highest risk for caries by O-pH.
This concept was tested in surveys
of paediatric dentists in Washington State and presented in a poster
of which Ms Sharma was a coauthor.
What is the next step for this
product, and what would it take to
make it widely available for use by
practitioners?
Dr Seibel: We hope to work with
dental device manufacturers to
conduct clinical studies with varia-

tions of the O-pH device, such as
the image-based device—multimodal scanning fibre endoscope—
used in the case study described in
our recently published paper. An
imaging system that maps oral
biofilm pH should be able to
achieve a more robust measurement over time. For example, one
set of panoramic images of the pH
of the biofilm covering enamel
surfaces can be compared with the
next set of panoramic images obtained six months later. Over time,
this measurement of the biofilm
pH before the biofilm is removed at
the dental clinic can become a
quantitative and spatial enamel
health monitoring system.

perhaps in conjunction with such an

Adaptability is at the core of
Dr. Chopra’s experience. She’s run
the gauntlet of dental procedures:
fillings, root canals, extractions,
braces, implants, bridges, veneers,
grafts, and I’m sure I’m leaving

something out. Right now, she’s
seeing a stressed need for endo
work.
“We were seeing that people
still really needed us,” she says of
the early stages of the COVID-19
shutdown.
Dr. Chopra’s practice is nearly
back to pre-quarantine operations—open five days/week, two
doctors in the office at a time.
“We’ve managed to be strategic
with our schedules so we can socially distance, so we’ve had to
change stuff around, but we’ve still
made it work.”
While activity is nearing “normal,” Dr. Chopra’s office never fully
closed… because tooth pain doesn’t
care about quarantines. “We went
down to three days a week, one
doctor each day, just to make sure
that we were getting people out of
pain and keeping them out of the
emergency room,” she says.
With office availability still
limited in some areas, and patients
postponing non-emergency procedures over the last few months,
general dentists may be experiencing increasing requests to tackle
endo work. Using flexible products
applicable for a variety of scenarios
will help limit initial costs and provide stability. They’re essential for
reacclimating to procedures that
might not have been common in
an individual office prior to quarantine.

MTAFlow White repair cement

components are the powder and
the gel (as dentists have seen with

through a proprietary process that

microns. This fine powder—when

pack it with ease, so I want a denser
consistency. As opposed to resorption which could be anywhere in
the root, and usually it’s down into
the root, and in that case I want to
keep my MTA a little more runny
so I can deliver it all the way down
there. A lot of times this is in the
anterior region so I’m only going to

use a white MTA in that area, so it’s
nice to have that capability now.”

“That’s probably my favorite

Immediate restoration
of an edentulous maxilla

The aim of this article is to
present a case of immediate restoration of an edentulous maxilla
with DS PrimeTaper implants
(Dentsply Sirona). This case report
is published as an inspiration for
dental professionals and not necessarily as a recommendation.
A 55-year-old male patient
complained about the retention of
his maxillary complete denture
and requested a fixed solution. Intra-oral examination and radiography confirmed an edentulous
maxilla (Figs. 1 & 2).
The dual-scan protocol was
used to visualise bone, softtissue
thickness and denture position in
the Simplant software (Dentsply
Sirona). Four DS PrimeTaper EV 3.6
mm diameter implants were
planned according to the position
of the patient’s denture, and four
Multibase abutments (Dentsply
Sirona) were visualised accordingly (Fig. 3).
Prior to surgery, intra-oral
scans of the edentulous upper jaw
and dentate lower jaw were performed. The existing denture was
used first as a surgical guide and
then as a provisional prosthesis,
after removing the palate (Fig. 4).

New cartridge and mixing tip system:
applying impression material efficiently
and easily

Dentsply Sirona now makes the
application of conventional impression material even easier and,
above all, more efficient: The mixing tip and cartridge system has
been optimized. The mixing cube
technology results in improved
mix quality of the material. In addition, material waste is reduced. In
this way, one cartridge can be used
for more applications. The new application system is available from
now on and will gradually replace
the existing system.
Three essential factors impact
conventional impressions: the
quality of the impression material,
the processing time, and the simplicity and efficiency of the application. The new cartridge and mixing
tip system available from Dentsply
Sirona seeks to improve these application features for brands like
Aquasil Ultra+.
The decisive factor is the newly
developed design of the mixing tip.
It can be easily fixed to the cartridge thanks to a turning wing and
small alignment pin. A correspond-

ing connection mechanism reduces the risk of cross-contamination between the base and catalyst
components. The new integrated
50mL cartridge and mixing tip system requires only a one-time bleed
before first use, for life of the cartridge. In combination with the reduced-volume cannula, material
waste can be reduced by up to 64%
and the number of uses per cartridge increased by ten additional
applications.* This optimization enables consistent homogeneous
mixing and efficient use of the impression material.
The color-coded cartridges and
mixing tips help to connect the
cartridges with the appropriate tip.
The new system is compatible with
the standard hand dispensers and
mechanical mixers already available on the market.

ment (Figs. 5–10), abutment position was registered with an intra-oral scan (Fig. 11). The provisional prosthesis was prepared
from the existing denture (Figs. 12
& 13) and then the Atlantis BridgeBase was designed (Fig. 14).
Eight weeks after implant
placement, the soft tissue had
healed well (Fig. 15) and was ready
for try-in of the Atlantis BridgeBase with a printed wax-up (Fig. 16).
The wax-up was later copy-milled
in zirconia (Fig. 17). The definitive
prosthesis consisting of the Atlantis BridgeBase and zirconia was
placed eight weeks after implant
placement (Fig. 18).

Study analyses tooth survival after root
canal therapy in US population

INDIANAPOLIS, US: Though there
have been many studies on the
survival rate of endodontically
treated teeth, only a relatively
small proportion of these have analysed these outcomes outside of
an academic clinical setting. Using
data from a large group of general
dental practices across the US, researchers have been able to glean a
deeper insight into just how long
teeth tend to survive after undergoing root canal therapy and predictors of that survival time.

The study was conducted by a
team led by Dr Thankam Thyvalikakath, a research scientist at Regenstrief Institute and director of
the dental informatics core at the
Indiana University School of Dentistry, both in Indianapolis. The
team used de-identified data of
46,702 patients who had received
root canal therapy from members
of the National Dental Practice-Based Research Network, a
consortium of dental practices and
organisations throughout the US.
According to the researchers, this
study is the first of its kind in the
US to analyse such records from
patients at community dental
practices.
Analysis of the data found that
the overall median survival time
for teeth after undergoing root
canal therapy was 11.1 years and
that 26% survived for more than
20 years. The researchers also
demonstrated that these survival
times differed depending on follow-up treatments and other factors. Teeth that received root canal
therapy along with a subsequent
filling and crown had a median
survival time of 20.1 years, whereas
those that received no additional
restorative work after the therapy
survived for an average of just
6.5 years.
Another significant predictor
of tooth survival time after root
canal therapy was the presence of
dental insurance at any point
during dental care. In a press re-

lease, Dr Thyvalikakath noted that
these findings could “inform dental insurance coverage by demonstrating the value of crowns and
permanent restoration options”.

“Oral health is a public health
issue that significantly affects people’s overall health. Leveraging
dental records can help us better
understand ways to improve treat-

ment, identify causal relationships
and maintain the health of teeth
and gums,” she added.
The study, titled “Root canal
treatment survival analysis in Na-

tional Dental PBRN practices”, was
published online in the Journal of
Dental Research on 12 May 2022,
ahead of inclusion in an issue.

Restorative simplicity for a
challenging case with limited space

Initial situation and treatment
planning
A 40-year-old male patient
with a missing mandibular right
lateral incisor and grafted area requested restoration with a dental
implant. The challenge with this
type of restoration is the limited
space available and the proximity
of the adjacent teeth. The clinical
evaluation revealed the limited
space (Fig. 1), and the periapical radiograph taken before the treatment showed both the grafted area
into the bone and the limited space
(Fig. 2). The treatment plan involved
conventional
implant
placement using an OsseoSpeed
EV implant (Dentsply Sirona) and
im- mediate provisionalisation
using a Temporary Abutment EV
(Dentsply Sirona), and for the definitive restoration, an Atlantis
Crown Abutment (Dentsply Sirona)

was planned to be used to restore
the appearance and function of the
missing tooth.

Implant placement
In the first step of implant
placement, the biological aspects
according to the 3A-2B rule were
evaluated using a surgical guide

(Fig. 3). The first drilling position
was marked to obtain 2B and to create the osteotomy angulation
using the Precision Drill EV
(Dentsply Sirona). The angulation
was confirmed and the implant
depth of the osteotomy was prepared with the Twist Drill EV
(Dentsply Sirona; Fig. 4). The depth

of the osteotomy was verified
using the Implant Depth Gauge EV
(Dentsply Sirona; Fig. 5). Implant
placement was then performed
with an OsseoSpeed EV 3.6 implant
of 11mm in length (Fig. 6). The remaining interproximal bone was
expected to provide support for
the interproximal papillae. The

periapical radiograph taken immediately after implant insertion
confirmed that the implant had no
contact with the adjacent roots
(Fig. 7). Thereafter, an implant-level
impression was taken using the

(Fig. 9). Using a dental dam, the immediate temporary restoration
was fixed with acrylic resin
(Fig. 10). After fixation, it was removed, finished, polished and repositioned with finger-light force.

final outcome with the defini- tive
restoration can be seen, showing
the correct soft-tissue contour and
the filling of the interproximal
space. Also, the ceramic perfectly
mimicked the colour of the adjacent teeth.

The clear way to
a perfect smile

Definitive prosthetic restoration
The patient was called in for an
appointment one week after surgery. At that point, the fit of the
temporary restoration was considered satisfactory (Fig. 11). Figure 12
shows the digital planning for the
Atlantis Crown Abutment in zirconia with correct space for ceramic
layers. The definitive restoration
was produced in the dental laboratory with a view to creating harmony with the adjacent teeth
(Fig. 13). Space for the interproximal papillae was created. The
screw-retained abutment with lingual access can be seen in Figure 14.
The provisional restoration was replaced with the definitive one (Fig.
15). The subgingival portion of the
abutment provided soft-tissue
support, and space for the interproximal papillae was created. The
Atlantis Crown Abutment was
torqued to 25 Ncm (Fig. 16). The lingual screw access hole was first
covered with filling material (PTFE)
and after that with a composite. Afterwards, another radiograph was

Ethical guidelines missing in field
of dentistry and AI, researchers say

Maintaining high ethical standards within the medical profession is key to providing the best
care possible. The doctor–patient
relationship is sacred, and the information exchanged between
these two parties is based on a high
degree of trust that the practitioner is prescribing the proper
treatment for the right reasons.
The integration of artificial intelligence (AI) in dentistry is now prevalent, and there seems to be a third
party entering this sanctum of
trust. In a recent study investigating ethics and the use of AI in dentistry, researchers revealed that
much work is still needed to ensure
that dentists better understand the
technology they are using and that
patients and their data are protected.
During a recent discussion
with Dental Tribune International
(DTI), lead researchers Drs CarlMaria Mörch and Maxime Ducret
spoke about their study, about the
still under-researched topic of AI
and ethics in dentistry, and about
the challenges that the field is facing. Dr Mörch is the scientific manager at FARI—AI for the Common
Good Institute in Brussels in Belgium and a researcher at the Université Libre de Bruxelles, and dentist Dr Maxime Ducret is associate
professor in prosthetics and digital
dentistry at the Université Claude
Bernard Lyon 1 and a hospital practitioner at the Hospices Civils de
Lyon.
“One of the major issues is
around transparency and the lack
of explainability regarding the
technology dentists are using,”
said Dr Ducret. “We have more and
more dentists adopting in their
practice technology that uses AI
but many do not fully understand
what it is they are using,” he con-

tinued. Adding to that, Dr Mörch
pointed out that there are currently around 100 sets of ethical
guidelines for the use of AI across
all sectors. “They are everywhere.
They are mentioned in the news,
and the EU has its guidelines. However, when we look specifically at
dentistry, there is no mention of a
code of ethics related to AI that a
dentist can follow,” he explained.
This limited understanding
creates unforeseen risks. However,
as stated in the study, those risks
have been around for some time
and little has been done. In the
study, Drs Ducret and Mörch
noted: “The frequency of publications mentioning ethical issues related to AI has not increased since
2015 and remains low, highlighting
a potential lack of interest in this
topic.” They added that some ethical issues around big data and digital technologies have been addressed but there has been little
examination of AI’s ethical issues
and its introduction into a practice
setting.
One explanation for this gap in
the research could be a lack of
training and education. “As a patient, we expect a dentist to know
what the limitations of a certain
tool or technique are, and so it
should be expected that dentists
know the limitations of the technology they are using too,” explained Dr Mörch to DTI. “Practitioners receive training in a huge
range of areas but have never had
an introduction to, or classes on,
the ethical issues around AI.” When
an algorithm is used, for example,
to examine a radiograph and
sometimes suggest costly procedures, dentists must know exactly
how the AI reached its conclusion
and clearly communicate this to
the patient, explained Dr Mörch.
At the moment, the researchers believe that lack of understanding
could mean that there is also a

blind spot regarding the risks AI
can pose, and thus, interest in this
issue is limited.
This lack of education has
prompted researchers to begin developing guidelines to help dentists better prepare for the future.
“There is no such thing as a universal tool that can suit academics,
practitioners and researchers. Even
in universities, you can have several fields working within dentistry, so there is no one-size-ts-all,”
said Dr Mörch. However, the idea
behind this work is to see whether
dentists can recognise the ethical
risks that might arise when put
into theoretical scenarios and
glean certain guidelines from
those resonances. When asked how
dentists could start improving
their knowledge of ethics right
now, Dr Mörch explained that
practitioners could begin by asking
more questions about how manu-

facturers of certain types of equipment arrived at their conclusions
and what the implications might
be for their patients. “Be aware regarding what the technology says
it can deliver and what the results
are, and if it is not clearly improving care, then remove it,” added
Dr Ducret. “The chain of responsibility is also critical. In the case of a
malpractice event, the question is,
who will be held responsible? Before implementing these tools, one
needs to know where responsibility lies,” continued Dr Mörch.
Additionally, the researchers
noted in the study that sharing
data could help create more transparent and understandable technology from which everyone from
the patient to practitioner and
manufacturer could benefit. “It is
pretty simple to say, but sharing
data and the benefits is challenging in reality,” admitted Dr Ducret.
However, he continued: “There are
many questions around data security and intellectual property (IP),
but the point we wanted to make is
to try and reduce the time, expertise and energy currently used to
make progress in the field, as it
clearly lacks sustainability. We
want to encourage a type of dentistry that does not try and promote again and again a novel solution by starting from zero, and
there are ways to collaborate without losing IP, not only in the field of
research but also within industrial
groups.”
Along with transparency and
education, there are other serious
ethical questions around the development of new technology that
need to be considered, the researchers said. One of these is
about the collection of patient data
that could then be used to develop
new algorithms and sold back to

patients in the form of a new procedure or piece of technology. “I
think people are not aware the way
current AI solutions are developed,
and dentists need to be responsible
for telling patients what might be
done with data that is collected,”
noted Dr Ducret.
AI has the potential to revolutionise dentistry in far greater
ways than it has done so far. However, according to the researchers,
the question is how this new technological era should be optimised
in order to provide the best oral
healthcare possible. “In our paper,
we try to highlight some questions
that those in the field may need to
discuss in the coming years. For
now, we do not have a perfect solution, but people need to think
about it,” said Dr Ducret. Adding to
that sentiment, Dr Mörch noted,
“Right now the technology requires a high level of knowledge,
and if insufficient effort is put into
the training of practitioners and
researchers, we will end up with a
field that is illiterate regarding the
equipment they are using. We
should know and be responsible
for all the techniques we promote,
use and teach in healthcare.”
The researchers believe the
question of whether industry, practitioners and patients can come together to find a way to integrate AI
in a safe and sustainable manner is
one of the most critical challenges
facing dentistry today. If it is not
addressed soon, they are concerned that the sacred doctor–
patient relationship could one day
be damaged beyond repair.

The introduction of the iTero
Element 5D Plus imaging systems
into the UAE and Gulf Cooperation
Council (GCC) will provide doctors

Over 100 dental professionals
attended presentations over the
course of two consecutive evenings (May 18 and May 19 at the
Theatre of Digital Art (TODA) in
Dubai), where Align Technology

Plus imaging system to its audience in the UAE.
Further launch events were
held across the region attracting
more than 80 attendees at the Ritz
Carlton Hotel in Riyadh, Saudi Ara-

The interactive events featured

Dental Tribune had a chance to
speak to the organisers of the 18th
ISLD Congress in Cairo and find out
more about its mission and challenges and how they are adapted.
Could you tell us in brief about
the International Society for Laser
Dentistry (ISLD) and World Academy
for Laser Education and Research in
Dentistry (WALED) please? What are
their missions, and how are they
connected?
Leon Vanweersch: ISLD is a
global dental organisation that promotes professional excellence in
the use of lasers through scientific
research and education. ISLD
strives to enrich the knowledge of
its members and to elevate scientific research, practice and teaching
to their highest levels and to disseminate this knowledge throughout the world in order to improve
dental
healthcare
standards.
Among other goals of ISLD, the society is motivated to cultivate international professional relationships and cooperation with scientific organisations in other fields of
dentistry and with universities in
the fields of laser research and development. ISLD serves its members as a non-profit organisation.
The society encourages membership by dentists and scientists who
are interested in research on, the
teaching of and the clinical use of
laser technology in dentistry, biology, surgery and other areas of
medicine. ISLD was established in
1988 and is the oldest and largest
international dental laser society.
WALED is the alumni club and
academic and educational worldwide network of the mastership
and MSc programmes in the field of
lasers in dentistry of Aachen Dental
Laser Center (AALZ). WALED’s vision is the realisation of an academy of excellence for postgraduate
education and research in laser
dentistry. We strive to offer lifelong
learning opportunities for our
members.
Since many alumni of AALZ
programmes are also members of
ISLD and several of us also serve on
the board of ISLD, it is logical to
support our members by, for example, organising our WALED event at
the same time and location as the
ISLD world congress so that our
members only have to travel once.
Prof Dimitris Strakas and I are
members of the ISLD organisation
board, Dimitris is the general secretary of ISLD and a scientific director
of AALZ, I am also director of
WALED, and Dr Youssef Sedky is
chairman of the ISLD congress this
year, as well as an academic coworker at AALZ. So we are all three
very much involved in both organisations.

Could you share the greatest
challenges you have encountered in
organising the 18th ISLD Laser Dentistry World Congress (ISLD Cairo
2022) given the ongoing COVID-19
situation all over the world coupled
with the recent passing of Prof. Norbert Gutknecht?
Dr. Youssef Sedky: The pandemic has been our toughest and
largest challenge. Our congress was
scheduled for October 2020. Just
seven months before the congress
and after our enormous efforts and
planning, suddenly the whole
world went into lock-down. It was a
time of uncertainty, and making
solid plans was impossible. During
these already difficult times, the
sudden loss of our beloved Prof.
Gutknecht was a devastating shock
to us all, as he was not only our society president but also a mentor
and a dear friend.
How has ISLD Cairo 2022
adapted to these challenges?
Dr. Sedky: After the world went
into lockdown, a crisis management committee was set up to
monitor the global and local situation regarding vaccination, travel
regulations and any restrictions.
Our goal was to assure our speakers, sponsors and attendees that
the congress would be held at a
later date, once the pandemic was
under control. We managed to stay
in contact with everyone through
our social media platforms, and
with the support of AALZ, we also
participated in a series of webinar
sessions. This was crowned by an
online symposium honouring the
memoirs of Prof. Gutknecht.
Thanks to our team’s huge efforts
and to the confidence and patience
of our keynote speakers and spon-

sors, we were finally able to fix the
new dates of our congress: 26–28
May 2022.

What can ISLD Cairo 2022 participants expect from this year’s congress?
Dr. Sedky: After a long wait, this
is the first ISLD world congress
since 2019. Our plan is to make this
congress a memorable event, both
scientifically and socially, boasting
more than 30 keynote speakers,
covering the various fields of dentistry, and more than 20 exhibitors
presenting state-of-the-art technologies. Our congress will host
dentists from more than 25 countries in Egypt, where the congress
will be held in a historic palace
hotel overlooking the Nile, and our
participants will enjoy different social events.

Could you share with us some of
the new trends in laser dentistry
that we can expect to see at ISLD
Cairo 2022?
Prof. Strakas: It is fascinating
for all of us who are in this field to
see the evolution of dental laser devices, especially in the last two decades. We are proud that, through
our biennial world congresses, our
members stay in direct and regular
contact with the newest developments, which so many dental laser
manufacturers offer to us.
This May, our world congress
participants in Cairo will experience a wide palette of wavelengths,
devices and brands. There will be
new multi-wavelength devices,
new handpieces, new photo-biomodulation devices and new magnification options for laser dentists
and much more. Moreover, the scientific programme will cover many
new discoveries in both theory and
clinical practice, which means that
participants who attend those lectures will be able to implement
those new protocols in their daily
treatments immediately.

What speaker line-up can the
attendees expect this year?
Prof. Dimitris Strakas: In keeping with the high scientific standards we set for our world congresses, the line-up of speakers is
impressive. Clinicians and academics from all around the world will
present their research and clinical
findings in oral presentations and
poster sessions. The number of contributors is around 100, and their
countries of origin exceed 30. We
have structured our programme
carefully in order to have specif-ic
topics in each session, making it
easier for participants to attend all
lectures on a specific topic if they
wish. Endodontics, periodontics,
cariology, paediatric dentistry,
photobiomodulation, orthodontics, oral pathology, implantology,
aesthetics, prosthodontics, soft-tis-

Prof. Strakas: As we all know,
Prof. Gutknecht was a true pioneer
in the field of laser dentistry. His research laid the foundation for today’s laser dentistry research. After
his death, the executive board of
ISLD decided that the best way to

of hopes and expectations. I hope
that we will execute a successful
congress, which we expect a large
number of people to attend after a
long wait, and I hope that they’ll
enjoy and benefit from this congress and return home with unforgettable memories. I expect our society to grow in members and affil-iations and hope that we can
spread dental laser knowledge to
reach everyone. I hope that we can
prove to ourselves that we are able
to continue the hard work of our
late beloved Prof. Gutknecht and
make everyone proud to be part of
this legacy, and I ex-pect that we
will be able to carry out his vision.
What do you personally expect
from WALED 2022?
Vanweersch: I personally expect that our WALED family will be
very eager to meet again and to celebrate being together again, not
only for the scientific part of the
congress but also for the social
events and for the friendship we
have together. Knowing that a lot of
people are afraid of travelling because of the ongoing pandemic and
the war in Ukraine, I would like to
strongly encourage everybody to
participate in this congress, because it is really time to meet again!
Besides, it’s going to be great fun to
experience laser dentistry in the
form of a real knowledge quiz with
our quiz masters, Prof. Dimitris
and Dr. Rene Franzen, and there’ll
be a real WALED CUP to be won! I
hope of course that I will see many
of our alumni at both congresses!

An interview with Dr Rhonda Kalasho

Dr Rhonda Kalasho has recently been acclaimed as the most
detail-oriented dentist in Hollywood. She works at one of the most
innovative dental practices in the
US, where she offers cosmetic, paediatric, emergency and general
dental care and treats dental conditions such as bruxism and sleep
apnea.
In
this
interview,
Dr Kalasho talks about some of the
state-of-the-art tools she uses in
her practice and discusses the dangers of do-it-yourself (DIY) dentistry and the influence of social
media on smile restoration. She
also reveals how COVID-19 has affected her day-to-day work and
presents some of the challenges
facing cosmetic and restorative
dentistry.
Dr Kalasho, could you tell us a bit
about yourself, and how you became a dentist?
I was very young when I came
to the US from Baghdad, Iraq. We
did not have a lot of money when
we arrived, and food and survival
were more important than oral hygiene. I found myself in the dental
chair a lot when I was young, and I
experienced what it was like to
have extreme dental pain at a
young age. I always wanted to be in
the medical field helping people,
but I think dentistry spoke to
something more than my yearn-

ing to bring health and wellness to
the public. To me, dentistry is a
consolidation of science, art and
medicine all in one field.
I love working on patients, and
I enjoy every aspect of dentistry—
from root canals to veneers. I do it
all, and I love it all. Dentistry has

come so far, particularly in the last
decade with the advancement of
3D printing and scanning technologies, so I always stay up to date
with the newest modalities and research and take part in continuing
education above and beyond what
is required. It is a blessing to be in
this field, and I am grateful to be a
part of it every day.
GLO Modern Dentistry is considered to be one of the most innovative dental practices in the US. What
are some of the state-of-the-art
tools and techniques that you use in
your dental practice?
Our office is fully equipped
with the latest and the greatest, including CBCT, intra-oral scanners
and 3D microscopes. I am not one
to buy tools that have no purpose
and just look fancy. All the equipment we have in the office is not
only state-of-the-art, but also ensures higher-quality dentistry.
Our scanners are incredibly
precise, and our implant restorations and implant-retained fullmouth cases are all fully mapped
and designed using 3D programs.
Patients have full autonomy and
are given a vision of what they will
look like and what they can expect.
Some of the most difficult patients
for dentists are those who need
molar endodontic therapy or retreatment, and with our 3D microscope by Seiler Medical, the treatment of these difficult molars becomes seamless. I have yet to miss
a canal using the microscope, and
the coolest thing is that I get to perform root canals wearing 3D

glasses, which is beyond fascinating. Patients are always taken aback
by the technology we have at the
office, and I am constantly searching for the best so that I can bring
them the highest-quality dental
care.
What is the primary reason patients come to see you—to x minor
imperfections, such as tooth overcrowding or unevenness, or for
more advanced dental treatments?
I like to think of myself as an
old-school dentist using cutting-edge technologies. I perform
dentistry on a broad spectrum—
including fillings, implants and advanced cases of Invisalign treatment. After the completion of my
residency at the U.S. Department
of Veterans Affairs’ San Diego VA
Medical Center, I continued to advance my skill set in multiple specialties to provide my patients
with the most comprehensive dentistry at the highest level of care. If
I am restoring a Class II cavity or
extracting impacted third molars,
I immerse myself completely in
the procedure, and I love every second of what I do.
It is the quality of the extracurricular studies undertaken and a
willingness to learn that will allow
even a generaldentist to perform
high-quality specialty care. At
times, I refer some cases to my colleagues in the various dental specialties. However, for the most part,
I treat almost any dental condition
or issue that my patients come and
see me for.

You are known as one of the
most stylish and detail oriented aesthetic dentists in Hollywood. Could
you elaborate on that and explain
how it affects your role as a dentist?
Being in the heart of Hollywood, many patients who see me
are on the big screen. They are singers, songwriters, producers and
artists, so it was truly a surprise
and incredibly humbling to be acclaimed as the most detail-oriented dentist in Hollywood. My
staff and I have a motto by which
we operate—we treat every patient
like we would our own families. If
you go into every case thinking
this, your mood becomes warmer,
your hands become lighter and
you end up putting a lot of love
into your work.
I strive for perfection, and yes,
perfection is not always attainable.
However, you can get pretty close
to it if you concentrate your energy
and use your skill set for doing the
best work possible. When I look at a
patient’s smile, I am not only looking at the size and shape of his or
her teeth. You can take a perfect set
of teeth and put it in someone’s
mouth and he or she will suddenly
look off. A smile depends on the
harmony with the face shape, lip
shape, lip line, nose and chin position. A beautiful smile depends on
so many different things, and I
look at all aspects when I am restoring a smile.

Many patients have resorted to
DIY dentistry during the lockdown.
What is your opinion about attempting to perfect a smile at
home?
I was recently asked by the
magazine Allure about whitening
solutions made at home. My advice
is to remain cautious about doing
any home remedy type of treatments, be it buying mail-in aligners, using a nail file to help x a
chipped tooth, moving teeth or
treating teeth, since these are all
delicate procedures. DIY dentistry
has gone wrong too many times for
me to condone it. I have seen an excessive use of baking soda and
acids to whiten teeth that eventually lead to tooth erosion and
enamel demineralisation. More recently, with the surge of the mail-in
clear aligners that are being vigorously promoted on social media
and in some podcasts, I have seen
some devastating malocclusion as
a result of patients moving their
teeth without being under the care
of a dentist.
Moving teeth is not as simple
as shoving teeth into a straight
line—you need to look at the root
morphology, the periodontal status, the bone quality, existing restorations, and evaluate the patient’s cephalometric parameters
in order to deliver the most aesthetic, long-lasting results within
the available standard of care. I
have seen patients no longer able
to close on their posterior teeth because they purchased clear aligners without being under the direct
care of a dentist. They then had to
pay so much for additional dental
treatments just to regain their
functionality, which defeated the
purpose of trying to save money
by going the cheaper route in the
first place. Leave dentistry to the
professionals.
COVID-19 has had a major impact on dental practices worldwide.
How has it changed your day-today
work and the way you treat your patients?
Since I perform extractions
and root canals, I treated a lot of
emergencies during the shutdown.
I was part of the Los Angeles Dental
Society task force for reopening,
and I managed to acquire all the
necessary personal protective
equipment (PPE) in order to perform emergency dental services
safely. We cut our patient load
during that time by over 70%.
However, it was more exhausting
having to treat patients under the
stress and fear of contracting the
virus while wearing such heavy
gear.
We have taken extensive precautions at the office, not only in
wearing the proper PPE, but also in
using extra-oral suction with every
patient during treatment and making sure that only one patient is in
the waiting room at a time. The
sterilisation technique is increasingly extensive and thorough, and
we test our staff at a certified laboratory for COVID-19 every two
weeks. We still provide the same
great quality of care we always
have done, just with some more
gear.
In your opinion, what will be
some of the biggest challenges fac-

ing cosmetic and restorative dentistry over the next five years?
One of the challenges is convincing people who want to look
like their favourite social media filter that super-white square boxy
teeth are not aesthetic. As more
and more dentists who have not
studied the art of symmetry and
ceramics are performing low-quality veneers or crowns, you will see
more of these lowquality ceramics
that are bulky and too opaque.
These bulky crowns generally lead
to periodontal disease and recurrent caries.

Social media often influence
people into thinking that they can
all do cosmetic dentistry. However,
my strongest suggestion is that everyone who is treating in the cosmetic arena must be trained in the
periodontal maintenance of the
patient and the dental materials
involved in high-quality ceramics.
The challenge rests mainly on the
education that so many dentists
are not willing to embark on. Continuing education is incredibly important if you want to deliver the
highest-quality service to patients.

What is the best advice you can
give to your patients seeking to enhance their smiles?
Every smile is beautiful. If you
have a gap, small teeth or gingival
hyperplasia or if you have some
crowding, keep in mind a smile is
not one-size-ts-all. Stick to what
looks natural. Chiclet teeth or
those overly white veneers or
crowns that look so incredibly unnatural will never be in style. I always tell patients to think of the
most beautiful person they know,
and every time I ask them to name

the person, every time, without
fail, that person’s smile is natural
and not overwhelming. Enhancing
your smile means just elevating
what you have and getting a colour
that matches your skin and hair
and creating teeth shaped to match
your face and lips in order to
achieve an overall harmonic appearance that will not overpower
your face.
