DÜBENDORF, Switzerland: Researchers of the Swiss Federal Laboratories for Materials Science and
Technology (Empa) and the Swiss
Federal Institute of Technology in
Lausanne (EPFL) have jointly developed a fully transparent surgical
mask that is intended to replace the
three-layer mask normally worn by
medical staff. The mask was developed primarily with the aim of improving non-verbal communication

between nursing staff and patients
but could also be worn by dentists
to protect against transmission of
bacteria and viruses, such as SARSCoV-2. The product is expected to be
launched in early 2021.

The transparent HelloMask facilitates non-verbal communication between patients and nursing staff.

In the past two months, people
in many places have experienced
how strange it is to talk to a person
whose face is partially hidden by a
mask worn to protect others and
the wearer from SARS-CoV-2. While
this makes it more difficult to deci-

pher facial expressions and impairs
acoustics for most, especially for
children, the elderly and the hearingimpaired, the masks pose a major
obstacle to communication. Owing

Empa and EPFL’s EssentialTech Centre has been working for the past two
years on a completely transparent
surgical mask. The researchers have

now finalised the material, which
is made of organic polymers, and

the EssentialTech Centre, which is
involved in the provision of modern
technology and humanitarian
measures to developing countries,
was working in western Africa
during the 2015 Ebola outbreak. “It

was touching to see that nurses—
covered from head to toe in
protective gear—pinned photos of
themselves on their chests so that
patients could see their faces,” he
said.
The following year, Schönenberger
was approached by Dr Thierry Pelet,
now the CEO of HMCARE, and Dr Sacha Sidjanski, a project manager at
EPFL’s School of Life Sciences, with
an initial design for a transparent

said Pelet. However, since this plastic
is not porous, it impedes breathing
comfort and makes the mask moist.
Over two years, the Empa and EPFL
researchers were able to combine
transparency, durability and porosity optimally. The result was a membrane made of a polymer specially
developed for this application. “We
can produce fine electrospun membranes with a pore size of about 100
nm,” explained Dr Giuseppino Fortunato from the Laboratory for Biomimetic Membranes and Textiles at
Empa, who developed the material
together with Empa postdoctoral fellow Dr Davide Barana. The structure
of the fibres creates extremely small
gaps that allow air to pass through
but not viral and bacterial particles.
In order to guarantee optimal protection, the new masks—just like
surgical masks currently in use—are
intended for single use. The question
of recycling or the use of a biodegradable material was raised at the
very beginning of the project. “Our
masks are made of 99% biomass derivatives, and we will keep working
on them until they are completely
eco-friendly,” commented Pelet.

in Switzerland

The material is made using electrospinning, for which electric force is
used to produce a mat of polymer fibres, and the researchers have adapted the method slightly for largescale production. The material will
be generated in spools, from which
individual masks can be cut and assembled. While the research team
initially planned to fabricate the
masks in Asia, it is now considering
keeping production in Switzerland.
In light of the increased demand for
conventional surgical masks owing
to the SARS-CoV-2 pandemic, production lines should soon be in place
and the developers foresee no problems in this regard. Pelet is currently
negotiating with several companies
and authorities.
The great demand for protective
masks owing to the pandemic has
made it easier for HMCARE to find
investors. The HelloMask project was
initially supported by a dozen nonprofit organisations and later by the
Swiss Innovation Agency. While the
masks will first be sold to the medical
community—including dentists—
they may eventually be marketed to
the general public.

There’s more to infection control
than masks and hand sanitizer. It’s
time to take a closer look at consumables: reuse and reprocess, or go
disposable?
Nonetheless, products under each
umbrella can be reprocessed safely.
After use, non-critical items should
be cleaned, and then disinfected if
the product is visibly soiled. Guidelines indicate that heat-tolerant,
semi-critical dental products should
be heat sterilized. The CDC recommends that heat sensitive, semi-critical products should be reprocessed
using high level disinfection or replaced with a disposable alternative.
The FDA has published an additional
guidance for multiple use intra-oral
dental dispensers, such as those that
deliver adhesive or composite materials.2 It states that these devices
that cannot be sterilized or high level
disinfected based upon their design

Post-COVID, everything will change – from your approach to PPE and scheduling patients, to the
procedures you perform. But should your products change too? One factor you may be considering is when to incorporate a single-use solution and when to stick with multi-use delivery. Let’s
take a closer look at infection prevention, product choices and what you may choose to change
in a post-COVID dental setting.
The CDC classifies product infection risk on a scale of non-critical to critical. Non-critical products
pose the lowest risk, as they only encounter intact skin, which acts as a barrier to most microorganisms. Semi-critical products are those that are used intra-orally or on non-intact skin, increasing the risk of infection. Critical products are considered high risk, as they’re used to penetrate
soft tissue or bone. The infection risk is lowest when it comes to non-critical products and highest
when it comes to critical products.1

According to the CDC, dental offices
should consider adopting single-use
products when they replace multi-use,
semi-critical products that cannot tolerate heat sterilization.3 The guidelines

multi-use products may be used safely
if the manufacturer’s reprocessing instructions are followed correctly.

Before making a commitment to adopting single-use products, allow these
considerations to guide your decisionmaking process:
1) Consider replacing heat-sensitive
products that encounter the intra-oral
environment with single-use options
2) Single-use products eliminate the
risk of cross contamination between
patients
3) Single-use products may increase efficiency by reducing reprocessing time
and effort
4) Multi-use products can also be used
safely by following appropriate reprocessing protocols

But why should you consider single-use
products and delivery systems? Singleuse products take the risk of patient-topatient infection out of the equation
– infection control is baked into the
product design. Of course, the risk of
patient-to-patient transmission is also
addressed when a multi-use device has
been appropriately reprocessed. But if it
gives you peace of mind, or if you think
it may give your patients confidence, replacing multi-use products that are routinely used intraorally with single-use
alternatives may be the right way to go.3
Further, the CDC advises that offices
should consider not just the initial product cost of a single-use item, but also the
time, cost and material savings obtained
by eliminating the need for sterile processing of that product.3 At a time when
so many procedures and protocols have
ratcheted up and your office workflow
is almost unrecognizable, every factor –
from expenses to application needs to
infection risk – is worth exploring.
Dental products, of course, are not onesize-fits all. Variables like application

Ten guiding principles as we recover from
lockdown and the COVID-19 pandemic

One of the lessons we have all observed in recent months is that the
effects of COVID-19 and the responses of populations and their governments have varied from one country
to another. This has had a knock-on
effect in dentistry, with variations in
the instructions and guidance given
by academia and those who regulate
the profession.
Here in the UK, the situation has
been made even more complicated
by the four governments of England, Scotland, Northern Ireland
and Wales issuing different instructions and guidelines to their respective dental populations, leading to
wholesale confusion and frustration
for both professionals and the patients they serve.
“All things must pass” is a phrase I
have reminded my clients of during
the lockdown weeks and as they now
return to work. Patients, team members and clinicians are having to
learn a new dance, Dentistry 2.0, and
that takes time. Perhaps the most
useful advice I find myself offering

is to use the mantra of a marathon
runner—“pace not race”— recommending that dental teams ease
themselves into the new routines
that have been imposed upon them.
An interesting and inspiring observation has been the level of trust
and loyalty that has been demonstrated by many patients. They have
waited for lockdown to end, have
sometimes maintained dental plan
payments throughout and now recognise that the catch-up may take
some time as patients are triaged according to urgency and importance.
Patients have also displayed high
levels of trust in the ability of their
dentist to take appropriate measures
to ensure their safety and that of the
team. It may be unfamiliar for them
to see their regular nurse, hygienist
or dentist covered in personal protective equipment (PPE), but they
know that it is you under there, doing your best.
I have observed that dental team
members have sometimes presented more of a challenge when invited
back to work. Perhaps because they
are healthcare-trained, there have

been requests for reassurance that
standard operating procedures and
PPE will be adequate to ensure their
safety.
Also, many have had to deal with domestic issues that have complicated
their decision to return to work. Personal health, childcare and vulnerable relatives count among the most
common of the issues raised. It has
been important for any dental practice owner to maintain a regular and
personal dialogue with salaried team
members and self-employed clinicians to provide either reassurance
or offer compassionate leave.
This month, most of my English clients are making their way back. In
the other UK nations (at the time of
writing) the debate rages and pressure is building. In Europe, many,
if not most, are back and operating
with different levels of compliance.
Who could have foreseen that dentistry would differ so much, depending on where you are standing?
However confusing this many seem,
we all still seek the same ultimate objectives:

1. a safe environment for our patients
and team;
2. the opportunity to provide appropriate care, whether that be pain
relief, functional repair, preventative
maintenance, specialist services or
elective treatments;
3. businesses that are solvent and
profitable so that trade creditors,
lenders, landlords, teams and selfemployed subcontractors can be
paid in a fair and timely fashion
and, equally, that owners can make
a decent living and grow prosperous
businesses;
4. businesses that play a responsible
role in their community, respect the
environment and value their people.
These core values remain unchanged, and yet it is important to
remind ourselves of them after what
has been many months of distraction and crisis management.
Almost all of us are beginning the
slow journey back to normality. I
recently shared with my UK clients
some guiding principles that I hope
will assist in this process:
1. This is what we are good at. Humans are designed to adapt and
survive. Consider, if you will, the

250,000 years over which Homo sapiens has evolved and the unimaginable hardships that humans have
endured on that journey. I do not
wish to make light of any individual’s personal or professional challenges, but I do want to remind you
that we are wired to win. COVID-19 is
undoubtedly the greatest challenge
to humanity that I have witnessed in
my lifetime, and I am confident that
we can and will prevail.
2. Stay calm. When all about you are
losing it, leadership is about calmly
taking control and demonstrating the way forward. Every dental
team member has a leadership role,
whether it is calming team members
or patients, answering the phone, offering virtual consultations or meeting patients face to face (at a safe
distance or in PPE). Emotion is contagious and your calm demeanour can
help all of those around you.
3. Be kind. That may sound cheesy to
you, but I have witnessed many random acts of kindness during the pandemic, and people remember the
way you show up in times like this.

4. Learn a new routine. During the
first weeks of lockdown, what we all
had in common was that our existing routines were stolen from us by
circumstance. We are creatures of
habit, and even though we may have
complained about the commute to
work, the office hours or other aspects of our pre-COVID-19 life, the
fact is that lockdown threw us all into
chaos. Some reinvented new lockdown routines quickly; others floundered. Whichever group you found
yourself in, return to work is now yet
another new landscape, and you will
have to develop your new routines
quickly and settle in to them.
5. Communicate. The winners in
lockdown were those who communicated regularly with patients, team
members and clinicians via video
calls, newsletters and messages. Just
because you are back to work, do not
stop! It is essential that you listen
to the feed- back on how your dental business is perceived by all concerned; essential that your communities have the opportunity to tell

you how they feel; essential that you
tell everyone what you are thinking
and what is going on.
6. Do not be afraid of giving and
receiving feedback. As we all learn
the new dance steps, mistakes will
be made—and that is OK. That is
how we grow. So be open to feedback
and be ready to give feedback when
you see things that are not right. At
the moment, your patients and your
team are some of the best consultants you could ask for.
7. Do not be afraid to ask for help.
When the COVID-19 dental history
book of 2020 is written, I like to think
that one of the benefits of this horrible situation will have been the
growth in unity of the dental profession, both nationally and globally.
Have you ever seen so many free
webinars? I hope that continues over
the years ahead and that dentists do
not disappear back into their former silos. Asking for help is a sign of
personal strength and confidence.
Membership of trade associations
and other representative bodies has

grown. My wish is that this momentum continues into post-graduate
education and beyond.
8. Be patient. Dentistry will not be
back to how it was before, perhaps
ever. However, that indomitable
spirit of enterprise that identifies humans will, in my opinion, drive even
more experimentation, invention
and innovation over the years ahead.
I believe that we will see a sudden
acceleration in digital innovation, in
virtual consulting and in the arrival
of new procedures, materials and
techniques in every aspect of dentistry and patient care.
9. Take time to think. Please do not
jump into a new hamster wheel to
replace the one you were running in
before COVID-19. This is a once-in-alifetime opportunity to start again,
knowing what you know now. If you
were starting your business again,
what would you do differently? Well
do it!
10. Seek balance. Back to that oncein-a-lifetime moment—I have had
many of my clients use this time to

create lists of aspects of their personal and professional lives that they
like and dislike. I have asked them to
seriously consider eliminating the
tolerations—the people, things and
situations that get in the way of their
happiness. You can.
Over the months ahead, there is a
great deal to consider:
– financial modelling and forecasting;
– marketing for new patients;
– the new patient journey;
– clinical and non-clinical operational systems;
– your team—their structure, roles
and responsibilities; – your overall
game plan.
Each of these is on my task list to
consider, decide and execute new
versions for my clients that apply
to the post- lockdown time and (we
hope) post-COVID-19 landscape. I am
working with a community of over
120 UK practices, and we are taking
that journey together, sharing our
experiences and collaborating on so-

lutions. Dentistry can no longer be a
fragmented collection of small business owners. As I heard on a webinar
a few days ago: “We may not all be in
the same boat but we are in the same
storm”. It is only by staying together
that we can enjoy strength in numbers.

The Inbox clearly displays all of the

dashboard and it also enables a 3D

The next generation of
the composite veneering system

After almost ten years, the leading
composite veneering system shines
with a new brilliance: the customisable BRILLIANT COMPONEER composite shells have been optimised
in terms of gloss stability and colour
blending. In the following case report, Dr. Mario Besek explains the advantages of the concept and demonstrates the consequences in practice.
After many years of research, the
international dental specialist COLTENE developed a system in 2010
that was to make anterior tooth restoration much easier for dentists:
COMPONEER are polymerised, pre-

fabricated composite enamel shells,
which improve the usual freehand
technique for major anterior tooth
restorations. The customisable composite shells are completely free of
inclusions due to mechanical fabrication, are homogeneous and have a
layer thickness of only 0.3 mm in the
cervical area at the thinnest point.
When launched on the market, a
silky gloss was already achieved owing to the material properties, but
the maximum of what was technically possible had by no means been
exploited (Fig. 1).

Brilliant further
development
In the following years COLTENE
developed its composite materials

further with regard to their physical and chemical properties, paying
particular attention to the gloss of
the material. This research resulted
in both the submicron composite
BRILLIANT EverGlow and the corresponding ONE COAT 7 UNIVERSAL
bond. In addition to its smooth application, the new composite is particularly notable for its gloss stability.
Conversely, the perfectly matched
bond contains the proven MDP
(10-methacryloyloxydecyl dihydrogen phosphate), which provides excellent adhesion on the tooth side
as well as on composite and other
restorative materials. The insights
gained from this development finally led to the revision of the classic
COMPONEER.

Of particular interest here was gloss
stability, which depends largely on
filler density and size. The first generation COMPONEER had a filler
size of up to 1μm. Abrasion through
toothbrushes allows the matrix to
be released between larger fillers,
which refract and scatter light on the
surface. However, under a viewing
angle of 60° incident light and reflection, only a part of the light reaches
the human eye (Fig. 2). Therefore, the
composite, which is capable of machine processing, has been further
developed so that the average particle size is only 0.4 μm.
On the one hand this results in less
matrix being released and on the
other hand the smaller particles lead

to less refraction of light (Fig. 3). Furthermore, the particle size lies below
the wavelength of visible light (400 –
700 nm). This new development not
only led to new shells, but also to the
improved new concept of BRILLIANT
COMPONEER.

Perfect surface gloss and
improved bonding
A few work steps have changed
through this reformulation and
these are described below: the initial
situation in the present case shows
older, abraded anterior tooth veneers

(Fig. 4). The basic shade of the tooth is
now determined with the BRILLIANT
EverGlow shade guide, as the BRILLIANT COMPONEER is preferably
used with the optimally matched
material (Fig. 5). The shade guide
can be used to determine the shade
of the composite shells. The disposable colour strip printed with the
BRILLIANT EverGlow shade equivalents is simply torn from the block
for hygienic application. BRILLIANT
COMPONEER is basically available in
two enamel shades, Bleach Trans and
Trans, which are exactly matched to
BRILLIANT EverGlow. A prospective
overall shade impression can be
obtained by placing the composite
shells on the colour strip (Fig. 6).
The proven retention structure on
the rear of the COMPONEER remains
intact. Due to the optimal consistency and improved bonding of
ONE COAT 7 UNIVERSAL, it can be
light-cured after application as an
option, which simplifies wetting of
the composite and thus the placement of the shells (Figs. 7, 8). ONE
COAT 7 UNIVERSAL is also used on
the tooth side after etching. Due to
its universal applicability and selfetching effect, bonding at critical
points can be improved. Light curing

is used to establish bonding (Fig. 9).
BRILLIANT COMPONEER integrates
harmoniously immediately after
completion of the restorations (Fig.
10). The close-up view of the restoration, which is distinguished through
its perfect surface gloss, is remarkable. The outstanding bond and ideal
shade matching result in perfect
blending of the luting composite.
There are also no visible transitions
to the tooth substance in the cervical
region (Fig. 11). While dentists often
only pay attention to microscopic
details, the brilliance at speaking
distance is important for the patient
(Fig.12).

Case 3 is interesting in that the patient was treated nine years ago with
COMPONEER of the first generation
(Figs. 17-19). After nine years, the
natural ageing process can be observed through a loss of initial brilliance. What is remarkable is that the
renewed preparation did not cause
any additional loss in hard tooth
substance. In this example one can
see the initial difference after treatment compared to the condition
nine years ago (Figs. 20-22). Nor can
any defects be seen in the close-up
view and even the incorporated

structures appear in high brilliance
(Figs. 23, 24).
Conclusion
The new formulation of the BRILLIANT COMPONEER composite veneering system has indeed proven
successful. The combination of individual elements improves the initial
surface brilliance as well as the permanent gloss stability. Colour blending, brilliance and bonding phases
have also been optimised – leading
to a «more durable» smile for the patient in the long term.

ultimately, solutions for our twopart and 100% metal- free systems
to meet this demand. And our R&D
department has done a great job in
this regard over the past two years.
We are now able to offer fully customised and 100% metal-free solutions from the new Zeramex Digital
Solutions competence centre. Our
customers now have the possibility of sending digital data to our
competence centre by secure data
exchange, which enables them to
order, for instance, customised abutments directly from us. But that is by
no means all. Our product portfolio
embraces fully customised abutments, one-piece monolithic crowns,
and custom-made crowns with and
without screw holes for ZERABASE
abutments, all made to the highest degree of precision. Whether for
restorations of anterior or posterior
teeth, we have the perfect solution
for each and every situation. In addition, customers can choose to have
their crowns ready coloured and
glazed. A 3D-printed model can also
be produced on request so that the
practitioner can check the restora-

tion before insertion and show it to
the patient as a tangible model. Of
course, every order comes as standard with the required 100% metalfree VICARBO screw. In addition to
the direct implementation of the
orders, our competence centre offers
advice in the field of data processing
and in the selection of the perfect
product for the individual needs of
the customer.
It has been a few months now since
Zeramex Digital Solutions was first
launched. How much has the new
competence centre appealed to users and how strong is the demand?
Hunn: As I pointed out earlier, the
ordering process is very simple and
uncomplicated and therefore runs
very smoothly. This meant that, at
start-up, only minor changes to the
ordering process were necessary.
The simplicity with which customers can get high-quality, customised
and, above all, metal-free solutions
adapted to the specific gingival situation generated a great deal of very
positive feedback. This interest,
combined with the fact that the demand for digital solutions is already
very great, made it possible for us to
achieve an even better start than we
expected. Customers and users with
whom we have long-standing, close
relationships naturally knew about
our project at an early stage and were
involved in product and process optimisation. Not only did they support us in this project, but they also
con- firmed that there will almost
certainly be great demand for the
new competence centre in future.
The word “future” brings us to
the next question. Will the digital
workflow at Zeramex offer even
more options in future? Are you already working on further developments in the Zeramex Digital Solutions department, and if so, what
form will these take?
Hunn: I thought you might put this
question to me [laughs]. Indeed,
there are product enhancements
that are already in development and,
in some cases, almost completed.
In the near future, the competence
centre will certainly provide us-

ers with innovations as well as enhancements of existing products. Of
course, I cannot yet tell you exactly
what these will be. In addition to the
product, delivery times are an issue.
We are already working intensively
on offering the customer shorter
delivery times as soon as possible.
The aim is that, in future, Zeramex
Digital Solutions products will be
received by the customer within two
days of placing the order.
Mr Hölldampf, you are the laboratory manager at Geiger Dentaltechnik, one of the longest established
machining centres in southern
Germany and a close partner of
Zeramex. In 2000, your laboratory
was the first in Germany to use 3M
ESPE to implement a pilot project
for milling zirconium dioxide with

CNC machines which operated on a
CAD/CAM basis. What can you tell
us about this material, the machining process and your experience
with it?
Hölldampf: Yes, that was very exciting back then. Zirconium dioxide
was still in its infancy, and when 3M,
then ESPE, came to us to test this
new process, we were initially quite
sceptical about whether it would
work. In the test phase, we realised
that the material zirconium dioxide
was unique and functioned really
well and that it would, therefore, be
possible to do quite a lot with it. Of
course, zirconium dioxide has its
pitfalls, and there are a few things
to consider when preparing and
machining it. At the end of the day,
zirconium dioxide is a ceramic. This
means that you have to carefully investigate factors like stability values,
connector cross sections for bridges
and the creation of the correct substrate for subsequent veneering ceramics. There are many factors that
come into play. Extremely precise
processing must be ensured over all
the manufacturing steps. In the past,
full-ceramic restorations were really
only possible using pressed ceramics
or the hot isostatic pressed variant.
Milling in the green state, however,
was a complete innovation. This new
approach was gentle on the material,
the machine and the milling tool. In
addition, it was possible to achieve
much more detailed and precise results.
It is said that stability is still an issue with zirconium dioxide. You
have been working with this material for over 20 years. What is your
opinion about its stability?
Hölldampf: Zirconium dioxide is a
very hard material. If we take a gold
crown for comparison, it is much
more forgiving in terms of chipping. This is because gold is much
more malleable than ceramic. For
the ceramic process, this means that
an absolutely perfect impression is

required in advance. If uncertainties are already present in the impression, this can lead to inaccuracies, and there is a risk of parts of it
breaking off. Since pressure cannot
be used to apply zirconium dioxide
restorations, a perfect fit must be
assured. A gold matrix is more forgiving. Because zirconium dioxide
is such a hard material, the product
has to be extremely precise and a
perfect fit, as I said. The fit is the absolutely crucial. In the further course
of machining, it is also important to
process it carefully without pressure
and with the right cooling. Especially
in the area of the geometry of implant connections, it is important to
use extremely stable ceramic material. The pressure on a natural tooth
is simply very different from that on
an implant. An implant is firmly anchored in the bone and allows practically no further movement.
Are there still problems with fitting with zirconium dioxide, and
does this require a lot of reworking?
Hölldampf: Actually, this is no longer really the case. Above all, owing
to improvements in the software
in recent years, it is now possible to
work much more precisely in scanning and then digitally designing the
implant. As a result, significantly less
reworking has been required in recent years.
Zeramex Digital Solutions offers
customised and completely metalfree abutments, crowns and even
one-piece monolithic crowns. To
what extent have you, in the laboratory, been able to benefit from
these innovations?
Hölldampf: The issue is really the
connection geometry of the abutment, that is, the prosthetic restora-

tion of the dental implant. Milling
this connection geometry in zirconium dioxide is highly problematic,
since no manual post-machining
should be carried out in this area.
Only a perfect first-time fit in the
transition area will provide a product that functions perfectly. Because
this connection geometry is milled
directly by the Zeramex Digital Solutions competence centre, you have
to be able to simply count on it being a perfect first-time fit. And this
means that we immediately get a
super-functioning product in original manufacturer quality that we
can use to create our custom restorations.
Are there comparable products
on the market with regard to customised abutments or customised
monolithic crowns?
Hölldampf: In the past few years,
we have had products from various
zirconium dioxide implant manufacturers. I would say that there are
comparable products, yes, but there
have never been other products
where you can safely assume there
will never be complications when
it comes to fitting in the patient’s
mouth. In Zeramex, we have found
a partner whose product simply
works. The product is carefully considered and planned down to the
last detail and perfectly harmonised
with other components. The system
is logically structured, all parts fit together perfectly, and the great thing
for us is that we can rely on finding
the best solution for every job, regardless of the size. We have never
had any other product in our laboratory that comes close to the quality
of Zeramex.

The Importance of Proper Light Curing
There are several clinical challenges dentists encounter if they choose an inadequate curing light or apply inappropriate light
curing techniques including weak adhesion and compromised physical and chemical properties of the restorative material.
In fact, studies revealed that more than 37% of composite restorations are insufficiently cured.1 This will likely have a negative
impact on the longevity of the restoration.
The following guidelines are based on the Consensus Statements on Light Curing2 (Northern Light Meetings, Halifax) and are
intended to help you achieve more predictable and reliable outcomes.

Shining A Light On Curing

Not all lights are created equal

Be aware of the key performance indicators: Make sure
your light delivers a minimum of 500 mW/cm2 in standard
mode. Be cautious when using high output lights (above
2.000 mW/cm2) that advocate very short exposure times
(e.g. 1-5 seconds).

her experience in Dubai and how she

DT MEA: Greetings Dr Deborah, it is
a pleasure for us to feature an experienced dentist such as yourself in
the Dental Tribune Middle East and
Africa edition. First of all, could you
share a short introduction about
yourself, what is your educational
background and work experience,
especially related to the UAE?
The pleasure is entirely mine, it is
an honour for me to feature in your

print and digital media publication.
Let me tell you a little about myself.
I completed my bachelor’s degree in
dentistry at the International University of Catalonia (Barcelona, Spain) in
2003, and my postgraduate degree
in implantology in 2006. I have always been very passionate about
dental surgery, mainly implantology, a field in which I have been
practising for over 12 years. Yet, us
dentists are aware that our patients
do not want implants but beautiful
smiles, hence I decided to embark on
further training to combine surgery
with cosmetics, completing my diploma in dental cosmetics in Barcelona in 2014.
During my career, I worked for over
seven years between Spain and the
UK focusing on dental cosmetics and
implantology. In 2015, I made the

From my experience, after completing the course the delegates have solid knowledge in implantology. In addition, the DHA exclusively requires

continuous thorough research and
reading of the most updated articles
is a must.
Currently, you are working as implantologist and cosmetic dentist
at the Dental Studio Dubai. Tell us
more about the clinic and the pro-

cedures which you perform?
Dental Studio is an award-winning
dental practice established in Dubai
for over thirty years. I am proud to
belong to a team that focuses on delivering exceptional patient care and
promoting lifelong oral health, and
we really pride ourselves on treating
patients with empathy, respect, and
individuality.
Daily, I focus my work on patients requiring regular care, cosmetic dental
treatments, and of course, dental implants and restorations. I work very
closely with my specialist colleagues
and hygienist to ensure our patients
receive the highest quality advice
and the personalized attention they
require.
You have been working in Dubai
since 2015. What is to your experience as a dentist in one of the most
diverse and cosmopolitan cities in
the world? How is life in general in
Dubai?
Having spent the last five years of
my life in Dubai, I can only mention
that my experience in all aspects has
been highly positive. As a diverse and
multicultural city, I have been able
to enrich my skills from dental colleagues from the most diverse parts

You completed the Clinical Implantology Dentistry Certificate and
Diploma programme provided by
the CAPP-Tipton Dental Academy,
British Academy of Dental Implantology (BADI) and the British
Academy of Restorative Dentistry
(BARD) in 2019 in Dubai, UAE. What
was your experience throughout
the programme and what did it
consist of?
When I arrived to the UAE in 2015,
my main objective was to be able to
practice my speciality of implantology following the DHA rules and
regulations. I not only wished to attain the UAE privilege required to
practice as an implantologist, but
also to refresh my knowledge with
the newest techniques in this field.
Having worked with part of the team
previously back in Europe, it was an
absolute privilege to follow a series
of modules which consisted of live
surgical patient treatment hands-on
trainings, live surgery demonstrations, lectures, and seminars.

All delegates were provided with adequate diagnostic tools to predict the
treatments success under the close
advice of the team. In my opinion, it
was an honour to share this experience with talented and highly qualified professionals such as Prof Goran
Urde and Dr Ninnette Banday. They
not only guided me through the full
process, but we created a bond of
friendship that will always prevail.

fact is rather difficult to gain in Europe, where I am originally from.

two stray cats Nelson and Greta live a
much better life than me. Travelling
is undoubtfully my other passion.

On a personal level, I have enriched
myself not only from expat traditions, but as well, built wonderful
and lifelong friendships with the
local community and their fascinating culture. Emiratis are kind and
respectful to our heritage, just as I
am towards theirs, and for this I am
forever grateful for the opportunity
I have been given to live and thrive
in this region.

We have implemented extra preventative measures on top of the safety
protocols; on arrival, our patients
immediately have their temperature screened (including all staff)
using a no-contact thermometer,
while our waiting rooms will show
minimal activity and have publicly
displayed hand sanitizer units to offer the highest protection while waiting for the dental appointment. For
every visit, our patients will fill in the
same questionnaire to help identify
possible viral exposure. It also assists
in identifying patients who could
experience the most severe effects of
COVID-19.
We have disposable masks available
at the reception desk on offer in case
our patients need one, as well as increasing the number of hand sanitizers and clearly marked our public
spaces to ensure social distancing.
We also allow for greater gaps between patient appointments to reduce waiting time whilst limiting
the number of patients in the clinic
at any one time to ensure everyone’s
safety.
When it comes maintaining the
cleanliness of the clinic, our cleaning
schedule has been increased so that
all non-clinical surfaces are disinfected every 15 minutes. All clinical areas
and instruments are fully sterilized
between every patient visit.
However, albeit extra precautions
having been taken in consideration
during these difficult times, at the
Dental Studio our patient’s well-being has always been our priority, and
many safety protocols as mentioned
before were already in place as a routine and will definitely be introduced
as the new norm in all dental clinics.
Dr Deborah, what are your hobbies
and what do you like doing in your
private time?
Whilst out of the office, my family
is my absolute priority and what is
most important to me in life. I love
spending as much time as possible with them and with my closest
friends, enjoying meals together
whenever possible.
To stretch my body and mind I practice Pilates and any outdoor sports
whenever the weather allows it in
Dubai. I volunteer during intermittent weekends providing meals to
lower-income groups that are struggling in the COVID-19 era, as well as
being an active member of animal
welfare in UAE helping animal shelters and rescue groups. In fact, my

How does COVID-19 affect dentistry and
what can we do to survive?
During the CAPP Live Stream Dental Meeting last April,
acting moderator of the online conference, Dr Çagdas Kislaoglu interviewed Dr Christian Coachman
on dentistry in the COVID-19 pandemic period.

In March 2020, when the COVID-19 disease had
just become an official global pandemic, CAPP
began a series of online accredited educational
programmes to support dental professionals
across the world during the lockdown period.
On 16 April, at the second planned scientific
programme, the organisers included an interactive interview between conference moderator Dr Çagdas Kıslaoglu and conference speaker Dr Christian Coachman, with a focus on the
COVID-19 pandemic in dentistry.

Dr Çagdas Kıslaoglu is an international renowned aesthetic dentist from Turkey, well
known for the smile and face makeover shows
streamed on national television focussing on
one-day smile design makeovers.
Dr Christian Coachman is a Brazilian sixth
generation dentist. His family line has worked
in dentistry for 170 years, as verified by Guinness World Records. Not only did he qualify
as a dentist from the most recognized dental

and connected globally, greetings from Istanbul, Turkey. How have you experienced
the past month since the global pandemic
became a reality?
Dr Christian Coachman: Thank you for the kind
introduction and a thank you to CAPP for organising this interview and live stream during
COVID-19 period. We plan to discuss important
and useful topics with good insights in a tough
moment for our profession, and for our planet.
We will do our best to remain positive and to
utilize our time in smart ways, glad to be here.
The current pandemic forced us to close
our offices with many dental professionals
becoming depressed and worried. Having
unplanned free time, many started watching online videos, webinars and began
self-improving through online education,
including myself. In your view, how did the
crisis affect dentistry and what can we do to
survive?
This is the most important question we are all
asking ourselves as dental professionals. There
is no way we can answer this question without
understanding that we can take advantage of
this time to improve as persons and as human
beings. Unfortunately, things are completely
out of our control. Nothing is certain. Our leaders and authorities cannot provide us with
direct answers since they themselves do not
know as well. The only thing we can do is to
accept the current situation and embrace that
tough times are still ahead of us, in terms of
our business. Understanding this is out of our
control, we may be harmed and hit by this crisis in either a large or small way, but we need to
look at the things which are still in our control.
One thing we have absolute control over is, to
invest in ourselves and take advantage of the
moment, to become better human beings, better persons, and ultimately better dental professionals. At this moment, we need to focus
on two things in my opinion, our health, and
our relationships. Perhaps a bit philosophical
however these are the things which matter in
life. Regardless of what happens next, even if
the worst is to follow in terms of business, we
do not know if our business will survive three
months from now and that is the truth. Owners in dentistry are not sure they will survive,
if they will be able to stay open and how will
this hit us financially? We just do not know.
We really believe that this will be over and
that more people will survive financially. As
owners we need to prepare ourselves to succeed regardless of what happens next. Even if
the worst occurs, closing down our business,
or starting from scratch, looking for a different job, or whatever you might do to cope to
increase your chance of succeeding in the near
future, you will need your health and your relationships. With health, I focus on physical and
mental health – becoming stronger physically
and mentally so we can face whatever happens
next. We should also strengthen our relationship – we must start with family and friends
because we need them more than ever during these moments. Even in the professional
environment, strengthening our relationship
means keeping in contact with our existing patients and staying connected whilst generating
useful content for them. Keeping things rolling
even though we are not seeing our patients.
We must show that we are ready, that we are
working hard to be available as soon as possible for the patients. Improving relationships
with your suppliers, with your landlord, with
people that you owe money to, with the bank,
with your account manager, people that provide education, with your colleagues and with
your community. Just invest in your relationships because regardless of what is going to
happen, the better your relationships are, the
bigger your chances are to succeed, no matter
what happens.

First, you mention the situation
about your clinic. This is the attitude
that we need to have. We cannot take
on this situation with anger or look
for someone to blame because the
situation is unfair for all of us. These
feelings do not help us. I have a similar situation with where we recently
opened our new office in Madrid. We
invested a lot of time and money in
it. It has an auditorium, a new lab,
everything the way we wanted it to
be. We opened a few weeks before
the lockdown and now occasionally,
I also go to the office and do not allow myself to wallow in negative
feelings.
In this regard, with the strategy that
you mentioned, you were right. We
must be wise when we go back to our
normal business. However, things
will not be the same. We should stay
relevant and plan on our strategy.
The first one, as you mentioned, is
to be more efficient because dental
procedures generate fear amongst
dental professionals that they will be
contaminated. At our DSD courses,
we teach to combine procedures
with new technologies to become
more efficient and shrink the treatment time. We know that crisis
speeds up the changes that are already in place. The things that we
will see implemented after this crisis
are the things that we already see as
important before this crisis. All solution that we have been talking about,
not in digital dentistry but based on
experience, we can add biosafety on
top of it.
This is the topic that dentistry, in
general has, kind of, neglected a little bit over the years. We were not
paying attention and we were not
specializing enough on the basic
rules of biosafety. And this is going
to become mandatory. In the beginning we are going to have to adapt,
we are going to have to retain our
staff, make physical changes in our
office, invest in some remodelling,
new equipment and increase the
quality of biosafety. This is going to
be key, because patients will want
to feel safe, patients will ask tough
questions and the dentists that are
one-step ahead in this perspective
will make their patients feel more
safe which is why they will want to
come back.
Having listened some lectures discussing topics on life after the crisis involved opinions that once the
crisis is over, people will not have
money to spend on treatment, but
I feel there are always some who
think differently. There are some
patients who call me and request
to make exceptions for them even
during the current situation. It is
interesting how diverse people
think. What are your thoughts on
that? Will patients have the same
spending habits? Or will the rich
be always rich and poor always be
poor?
Yes, I have been hearing the same
contradictions. I think that the highend world, the luxurious world,

the chances for our dental offices
to be amongst the winners post the
pandemic crisis.

That is very true. Speaking out of
experience, having gone through
a similar situation where I finally
opened my new clinic last month,
I was forced to shut down after a
day due to the lockdown. It was depressing and after two-three days
of sitting at home, I finally decided
to dress up, shave and come to my
clinic and spend time to read articles and start to realise what I needed to improve. I reflected on what
happened in the past and tried to
gauge the things I did right and as
a result I developed vast interest
in knowing more about the Digital
Smile Design (DSD) concept, something you are very familiar with.
Please share your thoughts with
us regarding DSD and reducing patients’ chair-time?

the rich world, we see that during a
crisis, at recession moments in history, people with a lot of money,
they spend less on things that makes
them happy. Like in 2008, the average dentist suffered. They were postponing things that were not vital or
essential. I do believe the tip of the
pyramid (meaning the few doctors
that take care of the very rich people)
will probably feel the recession less.

The second layer of dental offices
will probably suffer. It does not mean
that people will not want to rehabilitate their smile, but I believe that
people will tend to postpone. People
will look and compare dental offices,
and this is a moment of reflection
where a patient might go to a dentist
who caters more to what really matters. Every crisis has its winners. We
need to understand how to increase

can we increase our chances to be
amongst the winners, post crisis?
Unfortunately, most of the things
that would make us winners are the
things that we should already be doing before the crisis. Now, in a desperate mode, it is going take a while to
be that model dental office. I believe
that there are two things that we
can do as dental office owners. First
is, become experts in office management, financial management,
team management, leadership – the
professional side of our business.
Now if you are not investing much
time or energy on that, this is the
time you have to do that. Not only
that you must be efficient on emergency, financial strategy for the next
three months, but also, understand
what the next six months will bring.
Thinking outside the box to grow a
little bit your income on things that

maybe were not your main priority
before. I believe that as we go back
to work, we are going to have two
very specific moments to consider.
We are going to have a moment that
is an crisis management moment,
meaning do whatever you can to
generate any type of income, trying
as much as you can to break even, to
survive, negotiate payments and create new revenue stream, do whatever you can to make some money and
spend as little bit as possible. This is
the challenge, spend as little as you
can, but at the same time invest on
key factors to prepare yourself to be
different right around the corner. So,
it is a challenging moment. That is
why people say that crisis regenerate
opportunities because you need to
be very smart about where to invest
in a right way.

IPS Implants Preprosthetics is a
“useful addition for a specific patient group”

In order to provide an alternative
implant solution for those patients
with poor medical preconditions,
Prof. Nils-Claudius Gellrich, director
of the clinic for oral and maxillofacial surgery at the Hannover Medical School in Germany, and Dr Björn
Rahlf, senior physician for oral surgery at the same clinic, designed the
IPS (individual patient solution) Implants Preprosthetic. In conversation
with Dental Tribune International,
Gellrich and Rahlf spoke about the
patients for whom it is suitable and
how they developed this individual
implant.
Prof. Gellrich and Dr Rahlf, you
provide your patients with the IPS
Implants Preprosthetic. For which
patients is it intended?
Gellrich: We only use the IPS implant
in patients for whom a clinical necessity exists. These include tumour
patients and patients with acquired
malformations caused by trauma
or atrophy but also those with congenital malformations who, as they
get older, lose their teeth and exhibit
poor bone and soft tissue. When attempting to provide these patients
with a conventional dental implant
restoration, you reach a point where
you can no longer treat them adequately, resulting in the need for an
alternative.
What inspired you to create this
implant solution, and how does it
work?
Gellrich: In addition to the clinical aspect, which I have just described, we
have been focusing on computer-assisted surgery for 20 years. We have
been dealing with modern biomedical technology, patient-specific implants that are functionalised and 3D
analysis. Based on this experience,
we realised that it could be advantageous to attach the bone anchorage for our patient-specific implant
made of titanium at a location remote from the point where the post

passes through the mucosa into the
oral cavity. This avoids lengthy treatment protocols that would normally
require at least three surgical procedures and a delay of one year before
a patient receives a prosthesis.
Our initial goal was to be able to guarantee prostheses after six to 12 weeks.
Today, we can even offer immediate
care on the day of outpatient treatment. Although this means that the
planning phase for the implant is
somewhat more intensive, the time
for invasive clinical procedures for
the patient has become significantly
shorter.
Rahlf: Also important to mention is
that our patients are spared a major
operation in which bone is usually
transplanted from their iliac crest or
the fibula to the jaw, because these
operations often result in complications. With the IPS, we can operate
independently of the bone volume
because the bone does not necessarily have to be situated where conventional implants would be placed.
Instead, we can analyse the existing
bone structure, which determines
the implants, and we can have the
posts emerge from the bone where
they are needed for the prosthetic
solution. Soft tissue must, neverthe-

less, be transplanted in many cases,
for example to ensure the unrestricted movement of the lips. However,
this is not a problem, as the human
body has sufficient soft tissue and
this transplantation is not very difficult to perform.
Gellrich: The clinic’s cooperative
partner is the KLS Martin Group, a
globally operating medical technology company based in Tuttlingen
in Germany. IPS Implants Preprosthetic is a registered trademark. KLS
Martin now also offers the digital interface to prosthetic solutions.

of the implants has failed. I hope
that, with this method, we will also
be able to show that, for example,
even very old people can still live for
many years with a high load capacity
and chewing comfort. The oldest patient we treated was 91 years old and
the youngest 19.
Is the IPS implant currently only
available in your clinic?
Rahlf: We organised an event where
we showed other university professors in Hanover how the method
works. We have also been to international congresses and have presented the method at these. The demand
is great, and some colleagues have
already planned and implemented
their first major cases.
Gellrich: In Germany, colleagues are
now slowly starting to use the method, some of them at a clinic in Kassel.
We also have guest students who
come from other clinics to watch live
operations at our hospital.
Are there clinical studies on your
implant application available?
Rahlf: We have already published
a clinical study1 that described the
kind of system it is and how it works.
In a current follow-up study2, we
have been investigating whether the
quality of life of our patients was improved by the IPS implant.

Would you like to add anything
else?
Gellrich: To avoid any misunderstandings, I would like to emphasise
that the IPS, contrary to some misconceptions, is not a reinvention of
the old subperiosteal implants that
were used from the 1930s or 1940s
up until the 1970s. The great difference is that subperiosteal implants
were an alternative to normal implants at that time; however, this
does not apply to our implant solution. Firstly, the indication is different, and secondly, subperiosteal implants were not functionally stable at
any time. The IPS implant, however,
is a functionally stable anchorage
that a normal implant restoration
could never achieve in a comparable form in one of our patients, even
under the ideal conditions of a perfect bone graft. It is a line extension
in implant dentistry, an individual
supplement, not a replacement, and
certainly not in competition with
conventional implants. We consider
it a useful addition for a specific patient group.

How many patients have received
the IPS so far?
Rahlf: The first time we considered
this idea was in 2014; the first treatment followed in 2015, and the current number of patients is 41. However, some of them have received
more than one IPS, which means
that we have placed a total of 45 implants.
Gellrich: We treat sick patients, patients who are often also undergoing
irradiation and patients with congenital malformations—meaning
all of them have poorer medical preconditions. However, so far, over a
period of more than five years, none

IES 2020: Watch expert lectures on demand

Innovative, interactive and informative – this is a succinct description
of the online version of this year’s
International Expert Symposium
(IES), which was organized by Ivoclar
Vivadent for the dental industry. The
symposium lectures are now available online – free of charge and at
any time.
On five consecutive days at the end
of May, a series of online lectures
was presented in line with the symposium title of “Succeeding together
in tomorrow’s dentistry”. The event,
which involved 17 well-known dental
experts from all over the world, drew
an audience of more than 8,000
people. The speakers talked about
new technologies and current trends
and developments and they interacted with the audience in live panel
discussions hosted by Dr Jean-Pierre
Attal from the University of Paris,
making the event a truly unique experience. Aljan de Boer from TrendsActive, a respected trend research
agency based in the Netherlands,
presented an enlightening keynote
lecture, entitled “How to attract and

manage talent”. He provided the audience with interesting insights and
practical tips and tricks on a topic
which affects everyone, including
the dental industry. The online symposium was presented in partnership with the dental firms 3Shape,
Dürr Dental and Komet.

inexorably linked, and they provided
